Thanks again to you both. Having been told I had prostate cancer in 2007 many years of anguish followed. Watchful waiting ,countless PSA tests, 3biopsys psma scan Feb 2024 No metastatic disease but a large lesion periphery of prostate. 9 of 16 cores cancer. 4+3. Brachytherapy on my radar. Pain on and off in my hip. ( Builder for 46 years). 17 years with a diagnosis that was probably not needed ,only made me anxious with every little niggle. Wanted to extract prostate 17 years ago . Urologists need to keep their hand in to get better at this procedure. Glad I passed up 17 years ago. Now I need to get the best advice at this particular time in my life. Early 70s now.

@@stevemarriott2788 I'm glad you didn't extract your prostate. That was a cutting edge decision you made back then before the advent of 3t MRI's and psma pet scans. Good for you.

I went in for scheduled back surgery for 2 compressed nerves and while I was recovering one of my attending physicians was concerned that I had a bowel obstruction. A CAT scan revealed that I had metastatic disease caused by Stage 4 prostate cancer. I saw an oncologist within 15 minutes of the diagnosis and started second generation hormone therapy after my hospital discharge. My PSA has dropped dramatically and my oncologist is pleased with my progress but I’m still playing “name that pain” as I’m still recovering from the back surgery. This video helped a lot. Thanks.

Keep watching as many of these videos as you can. I take notes and mark the time, in case I want to go back and check the details. Often, they offer hope when you’re feeling hopeless. And they arm you with the knowledge you need to fully participate in your prostate cancer treatment plan. Best of luck to you! (And make sure you get a PSMA Pet scan as soon as possible!)

In regards to your previous discussion of Pluvicto, you had mentioned if having a good response to consider taking a holiday from treatment to stretch out the time you can use Pluvicto.. I suggested it to a friend of mine and they had always asked if they could do that. Apparently medicare won't cover the treatments if do this. Must follow protocol

When I was doing hormones my pain was stronger . Could radiation from Cyberknife case pain in the bones . I have pain in my butt cheeks. My psa is 0.20 with no treatment now and has been the same 0.20 for 8 months . I am 68 years old and my testosterone is 315 .

I too, am in the UK with the same diagnosis of metastatic prostrate cancer, and I cant find videos which match our therapeutic culture. But this is part of a wider culture. Uk experts in what was my own field of science, seem reluctant to communicate their experience to the wider “every person world” outside their universities and research institutions.

@@iaintoms7071 I wonder if it's because in the US they have to pay for their treatment (through insurance) so there are probably more options and those otions probably limited by the insurance coverage. When I mentioned PMSA scans to my oncologist it was brushed aside just like my questions of any related pain. At 80 yo any twinges I get I put down to old age

@@flyhalf2 Yes, there’s a lot to be said for your more cynical view. I’m 79. They are reluctant to show and discuss the scans that take, but, at my request, I was shown the Technicium scan. I know that we have the right to see the scans and radiologist’s interpretations, but I dont think the patient is seen as part of the team.

Dr. Schulz, I am a patient in Taiwan. I have been following your videos, lucky for me as I have English literacy, ever since I was diagnosed with prostate cancer in April 2023. I did the guided MRI and then biopsy at National Taiwan University with Gleason score of 6 (3 and 3) but PSA 21, the doctor was trying to suggest active surveillance first but then advise active treatment. I had the bone scan at the same and was cleared. I went to the Veteran General and did the prostatectomy in May. I recovered quite well but my PSA was 0.14 in August then 0.16 in November, 0.17 in February, then jumped 0.211. My doctor recommended me to take the PSMA pet scan in June, at which time a PSA was made again at 0.189, the result of the scan in June was said to be inconclusive (after they have internal meeting) but no metastatic spread. Then I took the latest PSA only two days ago which was at 0.171. I took healthy diet and dropped by 7.5 kilo since May, no drinking more. When I saw the doctor this time, he said there was a shadow in the PSMA in June when he pulled out from the computer. He seemed having forgotten what he had said the last time. In a way, I can’t blame him as he saw at least 150 patients on the weekly basis. He suggested the salvage treatment but he did say the effectiveness of which will be the same between .02 and .05. I reminded him of my latest down trend below 0.2 as he seemed to missed my latest PSA reading. As I have been watching all your videos so I know he is following all the protocols, and then I suggested that I want to wait to monitor for a while, i e. another 3 months to which he was ok. I did not tell him that I was thinking to wait till .25 or .3 to do another PSMA for the salvage treatment. I think I can live with this status psychologically, it was not easy in the beginning though, so I will see. I am no illusion that I had not a relapse but I would like to have a bit quality so is there any other things I should do? By the way I am 69 years old and my father did have the prostate cancer. Gleason 6 and psa 21! Was that psa verified with repeated tests? And if yes did you doc ever suggest that with such high psa maybe you need to re evaluate your biopsy slides at another place to verify the accuracy of your Gleason score? (I am not suggesting, just asking to know)

I had a prostatetechtomy in 2015 and had good PSA .About 2 years ago it has been rising at a faster rate to a little over 5.Took a PET and it shows no cancer but my Dr.still wants me to do radiation.How does this justify in treating the prostate bed with sight unseen.Does this make sense?

​@@T.Z.M4N With a psa over 5 I wd assume that any metastasis would light up in the pet scan. A doc may usually suggest radiation at the general prostate bed area in cases that psa is gradually rising above .2 (or even .1 sometimes) to values over .3 or .4 and does not want to give any more time to the tumor to grow till it becomes detectable in the pet scan. Let us know what you finally did anyhow and why you waited that long for your psa to get to 5. Thanks.

Can a biopsy possibly spread cancer outside the prostate? Prostate removed. Margins clear, meaning cancer had not escaped the prostate. 4 years later, prostate cancer found in the lungs. So how did it spread, from the biopsy or cancerous cells left behind from the prostatectomy?
