I think this goes even deeper than clock’s birthday being forgotten, like many people have theorized, remember, it came from the sky, where we also saw the channel number in episode five. So I think the present is basically this universe’s version of announcing whether or not a show has been renewed based on the company’s and viewer’s interest, and the gift being empty means that everyone has stop caring for ITFT as a whole, so the world and life as clock knows it will end soon, that’s why they become manic and depressed at the end, that’s why they were so attached to the gift, it was something that literally meant life to them, and that’s also why they couldn’t bring themselves to open it, in case they were met with devastating results. They spent the whole episode delaying the inevitable and telling themselves everything was going to be fine.

building off another comment that mentioned at around 15:22 that it spelled out perpetual, i have a theory about it you know how tv programs often have reruns after a show ends its original run, what i think is happening here is that channel 57 may just be one big rerun, however, something happened so that clock either remembers everything that happens and is just hiding it, or they were never intended to show up in the first place back in itft 1 they say "i have been here the entire time, it seems like YOU might not have been very observant" in my opinion, this can imply that clock (the regular object form at least) has watched reruns of whatever show was on channel 57 that included the cast, but seeing how things began to unfold in earlier post-credits scenes, the weird things happening eventually leads to clock becoming what they are now, somehow ending up in channel 57, and then making the show as a result this potentially throws things off as it technically means that it's no longer a rerun of the original program, but now its own thing (aka itft)
