Yeh he really was a great character. When the actor had that interview where he talked about the real life gangster that Dr Narcisse was based on, and described him as some kind of noble anti hero... Really killed it for me... Fans of the show love Dr Narcisse because he's a scumbag, but an eloquent and vicious one, one who's intelligent and vindictive, but generates great PR and puts on a pious front for the public. He fit in well among the characters of Boardwalk Empire. In a way I kind of saw him as being incredibly similar to Nucky Thompson- Both had a great public image, popular with their respective communities and did their part to contribute to them. Narcisse sold Heroin to Harlem, despite being a member of the American Association of Negro Improvement. Nucky sold booze to Atlantic city, despite being a public and vocal supporter of the temperance movement during season 1.

Nucky never gave up chalky. He always thought it was business. But in season 3, he realized it was friendship when chalky came to his aid. In this scene, he just tells the doctor what he wants to hear. He later met with chalky and they started planning. The mayor fucked them over. He told the Dr. About their plan, to get chalky out of town. When chalky comes back, nucky had Richard waiting to snip the Dr. It's not his fault Richard was too mentally and physically wounded to finish the job.
