Depression is one of the most common and most debilitating mental health disorders, affecting some 17 million adults in the US. It also continues to be a misunderstood, often hard-to-treat illness. Researchers have worked for decades to better understand the neurobiology underpinning depression. For patients with severe, treatment-resistant depression, spending months or even years searching for good treatments can be totally disabling. The prevailing hypothesis for years was that depression was regulated by the neurotransmitter’s serotonin and norepinephrine. Eventually, data began to suggest that maybe something much larger and more global was involved in the brain to account for depression, which led researchers to begin working with glutamate and GABA, the most abundant neurotransmitters in the brain. These chemicals are involved in neuroplasticity - the brain’s ability to adapt to change and protect itself against stressful events. Neuroplasticity is a physical thing, too: it manifests itself “in terms of synapses, how these neurons are actually touching each other and communicating with each other,” explains Gerard Sanacora, PhD, MD, Director of the Yale Depression Research Program. “And we know that in depression, the number and strength of these interconnections decreases,” says Rachel Katz, MD, a professor of Clinical Psychiatry at Yale. Ketamine - originally developed and still used as an anesthetic - works on those two neurotransmitters and was discovered to have rapid antidepressant effects. Some experience an improvement in symptoms in 24 hours or less. “We think that one of the things that Ketamine does, that helps to explain its antidepressant effects, is help the brain to regrow the synapses, the connections between nerve cells,” says John Krystal, MD, Chair of the Department of Psychiatry at Yale. Because Ketamine seems to work on a totally different system in the brain than typical antidepressants, “that may be why Ketamine works so well, even for patients who have not had success with a number of standard antidepressants,” says Dr. Katz. Ketamine as a treatment for depression gained FDA approval last year with the release of Spravato (esketamine), and for now, it remains a treatment reserved for patients who haven’t found success with at least two other antidepressant treatments. Dr. Sanacora explains that other treatments, including cognitive behavioral therapy, are important to use in conjunction with ketamine, “to develop new ways of adapting and having new cognitive strategies of dealing with future stressors to prevent relapse.” New therapies like ketamine are changing how depression is understood, and may pave the way for future research that helps us understand the brain in all of its complexity.

Why is this not used as a first treatment for depression? Regrowth and interconnections in the brain sounds better than relying on constant psychostimulation using SSRI for potentially years.

Several reasons. It's not approved by the FDA. Therefore, it is not approved by any insurance. I am getting one infusion a week currently. It costs $500 for each infusion. I will eventually go to once a month. It's insanely expensive, and let's be honest - mental illness is not exactly that important to the medical world, there's still a huge stigma in society towards people with brain diseases, and the medical system in America is just plain greedy. But there are those of us that are tired of it, and we are working on fighting the system.

Wicked wonka it is FDA approved. In Colorado, Medicaid covers ketamine therapy.

Because BIG Pharma doesn't want a cure or real treatment that will reduce or eliminate depression, they want you to be on meds for the rest of your life. These SSRIs do help for some but not for many, plus there's evidence that these meds may not even help, many cause suicide ideation and some have been linked to mass serial shooters...

Just had my first treatment. I’m a combat vet and have been struggling with severe MDD with psychotic features/PTSD for years and I’m here to say my life has changed. The day after my treatment I woke up at 4am and for the first time in at least 30 yrs without crippling anxiety or depression as well as anger. I was completely relaxed with hope for my family and my future. I have 5 more treatments and I’m excited to see what’s to come. I was nervous as hell, to be honest, but, it was one of the best experiences of my life. I felt so connected with everything my wife was with me in support. It was such an emotional experience. It’s very expensive and not covered by insurance in the US but worth it 100 percent. Had my second session and It was an amazing experience, different in some way I felt a bit more out into the mystic I now call it. I had a moment of WTF but it wasn’t frightening it was amazing. I noticed that as if the spiritual realm I was in allowed me to accept awesomely incites into what my life truly was meant to be like free. Free of depression, anxiety, pain. Very comfortable. I was feeling nauseous as hell the next day but it was manageable somewhat considering my experience. Also, I had a Flood of overwhelming sadness related to my combat experiences in Iraq. For the first time in almost 20 yrs, I was able to grieve the loss of my best friend as well as other war-related issues I had. After talking with my DR, and therapist I was able to process that. Incredible as if the piece of my soul that was left in Iraq was being returned back to me. Od_Arts 10 days ago My 3rd treatment was so powerful. I feel as if all exposures of trauma I've experienced are being reset with truth and recycled into my subconscious. My Dr. hooked me up with nausea med through drip and has worked wonders. My anxiety and depression have subsided big-time. I do experience some anger but it disappears quickly and turns into these moments of clarity and electricity throughout my mind. No constantly nagging intrusive thoughts. If I have one I deal with it as false and move forward and that's huge. Suicidal ideation was a kind of ebb and flow with MDD my least favorite part. I've had 2 again I notice it and deal with it as false and move on. This has been an incredible journey so far. I've had to deal with some shit moments but expect that on the in-between speaking for myself only. I can see how each journey builds onto one another. I have hope now and feel like I can take on the world. This experience also reverberates throughout my family. Look forward to the next. 4th treatment today was awesome! Very linear rather than organic I feel on the right track it’s been a minute) 5th treatment was so awesome! My depression and anxiety have completely subsided. I have newfound motivation my artwork has transformed. I will be one year sober from alcohol and I have had no cravings. The real work I have noticed is the days after treatment. I can tell the effects carry on to my family and that’s most important to me. I'm not stuck anymore so I can be more supportive of my loved ones instead of being crippled with mental health issues and the cycle of addiction. It’s not easy, I had to be open and honest with myself really wanting to do the internal work that it takes to move past trauma. This works. This works. 6th treatment was epic. I had a rough couple of days after my fifth treatment, I had a flashback due to an anniversary date what I noticed was even though it was a rough experience I was able to get past it in a healthy way and the event didn't force me down a hole for months or even yrs on end. I am even optimistic about my future in taking better care of myself as well as taking my art to a new level. Overall this treatment has worked very well for me. My wife, my Dr., and his wife have been the best support during this time. I would absolutely recommend CBT or some one-on-one therapy will getting the infusions to help make sense of the transformations that come with each unique experience. There is no more crippling depression and suicidal ideation. I feel at ease and at peace with myself. I know I will have to go back for booster infusions and that is ok. The only downside that I can say doesn't have to do with the effectiveness of the treatment but the cost. I consider myself in the lower rungs of the middle class as of now I hope one day soon this can be covered by insurance so there are no barriers between economic classes. Everyone should have access. I am also on a 6 round treatment, 4 down and 2 more to go. I noticed a huge breakthrough two days ago when I was really busy with a lot going on. I was on the phone with my husband and I said "I am so stressed out, I want to go home" then I paused and said "Wait, I'm actually not stressed out at all! That has just been my default setting and something I would say that it just came out of my mouth by habit, I actually feel fine about having things to do!" That may sound silly to some, but when your default mode is verge of tears constantly, it's an amazing feeling when that's gone. I also noticed when I went to my GP appointment I wasn't rocking back and forth in the waiting room, a nervous tick I have is to constantly fidget. I get my treatment at Revitialist in Louisville Ky. I get a 2 hr infusion and they're 375 bucks each. It is a lot of money, but so far for me it's been worth it. I got sick 11 years ago and both my physical and mental health deteriorated. I am a mother and wife, I spent so much of my time feeling like a failure bc my body and brain let me down and therefore I was letting everyone else down. I am lucky to have an incredible husband who has been by my side every step hoping the next treatment will be the one that works. I had bacterial meningitis, sepsis, and MRSA as my first diagnosis. I was deathly ill in the ICU for weeks. My life changed in one day, I lost who I was that day. The vancomycin along with the illness made my immune response spiral. I have been diagnosed with 4 autoimmune disorders since then. The first being Crohn's and the last diabetes insipidus . Pain meds have failed me for my severe nerve pain, and antidepressants failed me for my depression. I was relying on vitiams and food alone. It just wasn't enough. I am excited to see how I feel after all 6 treatments, I am a bit nervous on how often I will need a booster treatment, but I will cross that bridge when I get there. I just can't recommend it enough. I am so glad it has helped the OP as well. Best wishes to you and your family.

Psilocybin saved my life. I was addicted to heroin for 15 years and after Psilocybin treatment I will be 3 years clean in September. I have zero cravings. This is something that truly needs to be more broadly used in addiction treatment.

The Trips I've been having have really helped me a lot,I finally feel in control of my emotions and my future and things that used to be mundane to me now seem incredible and full of nuance on top of that I'm way less driven by my ego and I have alot more empathy as well

@@Chill_With_The_Ads_KZbin I’m actually done with treatment. I’m off all antidepressants, anxiety meds, and reduced my cannabis intake by 90 percent. I have at home prescribed ketamine I use to last for 3 months until (or if) I need another infusion. I had the routine 6 (3 per week for two weeks) but had to have a few after- it turns out I’m a rapid metabolizer and so they had to work on the dosage and up it significantly. I got more from the at home version where I could dose myself and they respond to my progress accordingly. But it’s nothing short of a miracle for me.

@@georginarandolphgirl66 I had major depressive disorder severe, as well as CPTSD and panic attacks. I didn’t have to wean off meds beforehand, but I wish I had. I was on lamictal before hand, and I titrated off after switching to a different clinic. It seems that inhibited my progress because they work opposite of one another with respect to glutamate and dopamine (the ketamine and lamictal)

Yes it’s been used as by anesthesiologists for surgeries for decades. But the doses and methods are completely different for depression treatments.

Ive been on antidepressants and/or anti-anxiety meds for the most of 20 years. I suffered a lot of trauma when i was younger so it's caused some issues to say the least. On friday I start my first treatment with a doctor in conjunction with my therapist. I'm so sroked and I've been looking forward to this for months. I've read lots of great comments and it's making me even more excited. I cant even remember what it feels like to "be happy" or "at peace" for real. Wiah me luck!

I’m currently half way through my ketamine therapy. As a combat veteran, many things linger. After 10+ years of doing, psych therapy, various antidepressants and so forth. I’ve found ketamine to have an unexplainable success with some of my most serious issues. I relate it to electricity when people ask me how it works, I explain I don’t know how ketamine works, just as the same as I don’t know what exactly electricity works however I enjoy the benefits of it. I wish I would have known about it sooner.

Been mixing this with lions mane, ginseng, and ginko biloba extracts. Ketamine therapy has always helped me but now I'm seeing huge rapid changes. I'm trying to get a job now for the first time in years!

I didn't know, but aparently ive got slight manic depression, work in hospitality, after loosing my job girlfriend house and few more things I don't want to mention in public, i discovered Ketamin, its like a positivity BOOST... for me literally instantly iam able to think clear, productive, logic creative, this stuff works, i have got my life and routine back❤

My first infusion is in 6 days. Im honestly terrified. Psychedelics scare and intimidate me, but my family and my doctors think it'll save my life. I want this to work so badly. Not only for my psychiatric disorders, but I have pretty bad chronic pain.

@@Usedw0tm8 I'm glad it's helped! I have ptsd and am scared try ketamine because I've had dissociation before and depersonalization and that was very anxiety provoking. So I was scared to see it can cause dissociation and an out of body experience.

Praying for a way to afford it. I’ve had 7 infusions. Money is becoming an issue at $450 a session. It helped me and was very noticeable in every aspect of my life. My major depression has robbed me of far too many years. My first treatment was 4/29 . I became a new me about the 5th treatment. I want that back. It felt so good to live!

@@doo6z I'll have to go to currency converter to see how much 20 pounds is in U.S. dollars. Is a gram the standard dosage given in one treatment in the U.S.?

@@serenahm like 35 dollars. 1g is astronomically high but from what I’m reading I’m guessing a dose is anywhere between 100-250mg maybe even lower if intravenous

I’m comforted to hear a prestigious institution support ketamine use. I had a course of it awhile back. Short vid. But no one greeted me with “my you look better” afterwards. I think it helped but I’m still dragging many times.

Ive been having Ketamine IV infusions in between therapy sessions and while still confused about the meaning of this world, Im okay and not at a default sadness. I will also say that I took Kratom daily for 5 years, gave myself an opiate addiction by doing so, was on Suboxone and two ketamine treatments in, I have no desire, physical or mental to take the Suboxone in my cabinet right this moment. Bare in mind, I couldn't physically sleep without having my opiate receptor activated, ie suboxone, kratom etc.. my first session I slept the entire night without taking anything and I haven't felt the need to since. Its crazy.

I was in ER for a hand fracture. To set the bones, the doctors gave Ketamine through IV. As soon as the tranquillizer rushed in, I was rushing out into a colorful spiral -- a world full of wheels and gears. Noises were disturbing and the visual effect was overwhelming. I didn’t like what I was experiencing from the get-go. It was not fear but an uneasy feeling of helplessness in a restricted space. The speeding world or rather mind was like a fast, persuasive 3-D ride in a theme park -- just that I wanted no part of it and had no joy in the trip. At the end of the run of concentric spirals, there was momentary peace but that didn’t last long. At this point, I had no idea what was going on in the room. In the hallucinated world, I had multiple personalities and that was persistent during the entire episode. I felt I had become a hollow object of white matter with no body; I desperately wanted to get back to normal life. Then, I was able to switch to another plane and relive there. Parallellism was evident. In one of the nether worlds, I had died, and there is a group of doctors trying to revive me; my family has been called for a final sendoff. And I was telling the doctors that I died due to overdose of ketamine. Later, when I partially woke up when the ketamine was wearing off, I was really surprised that nobody was around me in the room. Nor did I want the medical devices stuck around me. Some of these hallucinations likely occurred during the latter half when drug was wearing off as I could remember the room, see the time, converse like a drunk and yet I could enter the demonic world and continue where it left off when I closed my eyes. Whoever came and spoke to me in the ER room gave me a wonderful reassurance. I wanted more people around me. I craved for jokes as the whole psychedelic experience was too intense. Whenever I opened my tired eyes and saw the nurse was doing something else instead of attending me, it made me depressed. My cousin had joined me by then in the room and wow, what a company that I relished from then on for the next 30 minutes! I repeatedly said I didn’t care if they didn’t fix my fracture, they just need to reverse the tranquilizer. It was overall a spooky and tossing experience. I am not sure if I am brave to go through this drama again. I hope NDE is a bit more peaceful for all. I think a relaxed mindset and an appropriate setting is important for a positive experience.

What a terrifying experience you had, I’m sorry you weren’t being supported during it, it was irresponsible that they left you there. Quite understandably, you would t have been in a peaceful state of mind with a painful injury and the frantic ER activity around you. @@fc4660 Thanks for the reassuring words. The ketamine was a little bit overdosed in the first place which was evident in a later-than-anticipated wake up time. I'm sure the experience may have been less intense if the potency was lesser too. Also, the medical staff knows about the drug, so left me to my own recovery time when I was fighting the mind. Now I definitely appreciate the experience -- to get a window of opportunity into how life can end (if we leave non-violently). Looks like you are in a related field and is nice to hear from experiences that the end can be peaceful!

I too would like to know! I've been on some type of benzodiazepene for 30 years. I don't get into REM sleep which causes short term memory loss. You file your memories away when you're in REM sleep). There's studies showing a direct link between taking benzodiazepenes and dementia. So if ketamine can help protect me against benzodiazepene dementia ... it's an added BONUS!!

A week ago I did half a gram in a single dose I broke down crying during my experience and since this I have felt content and at peace with myself struggling with depression for as far as I can remember ketamine has helped me a lot I tried this with mushrooms and it helped for awhile but when I recently tried ket it’s made me really mellowed out I feel happier and peaceful before I was always in low state of mind and angry I can’t feel any bit of anger at the moment and it’s been a week since I last tried anything

Any treatment that can actually change something, not just cover up symptoms, is worth pursuing. Especially, if there is some experiential element to it. In other words, not just taking the pill and expecting a miracle, but having some sort of realization in the process. I hope that this will eventually become a standard, and that some medicines that are nowadays used (and misused) - such as benzodiazepines - will be illegal in the future, because these often cause a lot of harm.

Your right. I was on oxazepam 100 mg per day, and lorazepam (2 mg) + temazepam 20 mg per night. To getting of the oxazepam was very hard, my anxiety became much worse then before, i had extreme muscle tensions over the entire body. But I did it, since two months oxazepam free. I am now slowly tapering of the Lorazepam (from 2 to 0,5). They should forbid it!!! Ketamine is much more powerful.

I have had four IV Infusions for Major Depressive Disorder. Two more to go. The K experience was very beautiful and inspiring. But so far I am not experiencing a remission of my Depression. There seems to be this 'hype' around Ketamine as a Depression cure in the media. I wonder what the actual data is on people being 'cured' of Major Depressive Disorder with Ketamine.

The data is available freely on PUBMED. Scavenging the many papers I saw 70-80% success rates, but a lot of these studies do not follow patients long term.

I had tried 10 different subtypes of antidepressants before I started ketamine last year. I still need it once every 3-4 weeks, but I can work again and can keep up my life. It’s lovely. Only advice i’d give is look up what your medications do with ketamine. A lot of SSRIs don’t interact well with it. I don’t take my other medications, and i have better outcomes now with it alone

Doesn't your psychiatrist work with you on what medications you can still take safely and which ones you need to stop? I would think she would be the best judge of that. Sounds like you need to go over that with her.

I'd suggest talking to your doctor or pharmacist if you're on antidepressants or anti-anxiety meds. I'm working with my therapist and a doctor in the same office at the same time and I think it's the safest way to go. Best of luck and take care.

I would definitely not recommend using this to self-medicate. Not trying to be judgy here, just reminding you that you need a therapist to help you process all that comes up and integrate these experiences to get the real benefit of the treatment, plus a psychiatrist to monitor your treatment to make sure you're doing it safely. Self-medicating with anything is never a good idea.

So my moms boyfriend has been in a severe depression for months if not years.... He is a veteran and has had a lot of trauma in his life. The main one being losing his wife in his early 30s to cancer. He's mostly been useless as a human since I've known him, but I feel for him. Its been years and began to depress my mom and now theyll both sleep until 2-3pm every weekend, hes daily. I get it having gone through depression. Im so excited the hospital and himself are willing to take this jump and its all through the VA! I hate the VA because I've watched his treatment for the last decade and they keep them on a cycle of drugs. This is the first time theyve offered him an alternative to their treatment and said theyd pay for him to go to the state school to get real treatment as long as hes approved.... I dont know what to say. I made them watch videos and evidence before. They ignored me. Now it's government approved and theyre all about it.

Please let us know how this turns out for him. I hope it works! With him and your mom in such a codependent relationship, maybe his improvement will help her. She needs her own therapy or support system like Al-Anon, though. But I hope the ketamine treatments will be a new start for the whole family.

I've been going to the same VA psychiatrist for my PTSD/MDD and have been on a never ending but frequently changing cycle of psych meds since 2012 and only just got offered and approved TMS therapy last Friday.

I have been on medications from every drug group, along with psychotherapy. They’re suggesting to switch from TCAs to SSRIs - which is moving two steps backwards. They’re not ruling out ECT, but I live in Australia, and Ketamine therapy is probably privatised. Haven’t heard anyone mention it and I haven’t come across people who’ve had it. I don’t think recovery is going to happen if they keep doing this to me.

I mean, it's privatized everywhere that im aware of. If you want even mediocre medical care in the US, you pay. If you want quality medical care in the US, you pay way more. It costs thousands here and it's not covered, even when you're paying for insurance, just like every other effective treatment. Health insurance here is expensive and pretty much only partially covers ER, imaging, medication and some surgeries but nothing that actually improves one's health.

I can’t help but wonder if ketamine can help stroke patients recover mobility during rehab by re-wiring neural pathways. And stroke patients often have depression, sometimes from brain damage. Perhaps ketamine could help there too. I’d be interested in participating in a study on this.

For 4 months I used it. 1/2 mg per day. Not only did it help my clinical depression, but helped my Fibromyalgia. Only reason I don't currently use it is because I'm worried about fentanyl and the fact that for no reason its still not used except by infusion. 28 g every 2 weeks. Why? When 1/2 mg everyday works!

That is a really good point. How are they measuring this... it is not clear. I got a bit of a worried feeling when the woman at the end talked about how it's great to see the change on people's faces... because that could just be social conditioning too. There is the expectation that you'll feel better, so maybe you play it up to act positive, or maybe you believe that whole "smile and you'll feel better" thing, or just plain politeness, any number of other things. I spent some time in talk therapy and I didn't feel like I really made any progress, but my therapist waxed poetic about my general mood/demeanor at the end versus at the beginning when I first came to see her. I didn't have the heart to tell her I felt exactly the same. It was just that in the beginning, I decided to go with resting bitch face as my default. And when I got to know her I let it go. It didn't mean I actually was doing better, just that I decided to relax around her. I have to say that it felt really shitty to hear her say that. Because it felt like the only thing getting her out of bed in the morning was to see people get better, and my "getting better" was a boost to her ego and her career and sense of self-worth. So because I didn't actually get any better, I sort of failed her. And I couldn't be honest because it felt too rude. This is what happens when you are the fawn of the 4 F's - fight, flight, freeze, fawn. Thanks for coming to my TED Talk.

2? lol I have been on at least 15 different medications for my mental health including adhd. I have had 1 antidepressant work over the years which was Prozac, but that stopped working. Zoloft has kept the deep pit at bay, but I have been on everything including lithium Which was the worst. Maybe if I wasn’t diagnosed with bipolar when all the e time it was undiagnosed adhd then maybe things may have been different. I would love to try ketamine. A friend of mine who is a psychologist as treatment resistant depression and he has been micro dosing for about two years. The difference is incredible, I know he still has his moments but I can see such a difference, even the expression on his face. The irritability and anxiety has really decreased and his mood stable, better than going down right? We need to make treatments more available to people. I’ve struggled with this horrible illness for 30 plus years , of low mood anxiety and and have had two major depressive episodes both Itimes I had very bad suicidal Ideology with attempts. I can’t believe I am still here to be honest, my kids saved my life. They are my blessings c

How your traumas were being reset but did they come up and you successfully gave them first to get healed. We can’t mask the healing we have to address it and that mean feel it and face it to get it out. And does this medication take away your sense of emotion or make you zombie like or are you better because you don’t feel anything to react to. I see you eating works but what is the person going through for it to work. . Like is it a bandsaw are you able to mourn your frown by suppressing the sad feelings along with it? I need more info on what it’s actually doing to help not what it helped.

I’d run for the hills. Like you said, your brain is still developing. My parents put me on anti depressants at 15 for what was normal teenage angst, and my brain has not been able to function without them ever since. I’m 45. Not to mention they don’t work anyway.

@Glowi Main Some of the antidepressiva has a warning that suicide is the Side effect ;LOL! cause you get so flat you dont care. my mother is a negative person, on common anti depressiva. Sitting like a Person who waits yo die while wathing tv. i hope these Micro dosing comes soon. Cause old people cant have this Tripping effect, it scare them away. I have permanent brain damage from ketamine that for some reason I only experience symptoms when I do it. Absused it non-stop for about 6-7 months and some fuses up in my head blew now nothing makes sense when I do ketamine literally nothing my brain starts sending signals down the wrong pathways I even started convulsing last time I used it. God I miss ketamine

I’ve received many high dose (100mg IM) injections. Really the only major side effect is that it’s hard to do vigorous activity after treatment on the same day. So take it easy and slow. It can be very tiring as well. But it also makes things look crisp and clear. And the best part of it all is the incredible antidepressant effects

@James Becketts I've used DMT and mushrooms and LSD to regain neuron pathways after my brain injury. It really helped to recover memory. It's just expensive to use in the way I will get the Spravato. Twice a week for the first 2 weeks, then once a week, with monthly doses available as upkeep.
