I’m 31 y/o male very active just sustain a knee injury. I’m currently going through exercises program to reduce inflammation, maintain muscle strength and restore ROM. My plane is to take an MRI a month after my injury. In the event that my ACL is torn would you recommend to go through an ACL reconstruction? I appreciate your advice.

After my ACL surgery, my affected knee was always a bit "looser" with still a fair bit of tibial movement relative to the femur, compared to my healthy knee. This has led to cartilage damage and somewhat chronic imflammation. I fear I am developing arthritis

Hi dr...do we need yearly prp injection as maintenance to slow down OA post ACL injury?

Yes! Maintenance injections are recommended. Think about any other "chronic medical condition" like high blood pressure or diabetes. You take a medication every day because if you don't, your blood pressure will go back up. Same thing with arthritis. Once you get it, you got it forever. So you need maintenance therapy to keep things under control.

pubmed.ncbi.nlm.nih.gov/32779915/ As this study shows, results of the effect of PRP injections vary, and the use of PRP injections should be discouraged. Can you please explain why you think that the use of PRP as a maintenance therapy to slow down OA is a good thing to do?

@@medstud6931 Yes! As our understanding of PRP improves, preparation and protocols have resulted in better outcomes. Subsequent RCTs and then systematic reviews and meta-analyses all show that PRP is superior to alternative treatment options such as saline, corticosteroids, and hyaluronic acid. @@medstud6931 I don't think it's ready for mainstream use and should really just be recommended by providers experienced in orthobiologics and cellular therapy. There is still too much unknown and many potential bad actors out there that will take advantage of patients to make a quick buck. This is why more studies need to be done and for the FDA to play a bigger role in monitoring and cracking down against sham clinics. One of my goals with these videos is to share the science and medical literature to educate all those wanting to learn more.
