What does the “plus” mean on a chord? Is it always the same thing for every chord? I mean, surely not. What a stupid question! Ha ha!

Not at all. That's a good question. It's short for +5 or sometime you will see #5. The symbol means augment, or go one half step higher. In this case, you would augment the 5th note in the chord. So the G chord G B D becomes G B D#
