We've been using the adapter piece for quite a while in EMS for running an inline neb with BVM. I think it is something that can be easily made with a 3D printer instead of ordering from a manufacturer/supplier. Have you tried attaching the setup with a CPAP mask so you can be hands free from the pt if necessary?

Souce: I work in Pulmonary and any grade hydrogen peroxide used in a nebulizer no matter how diluted is caustic to your lungs and can cause pulmonary fibrosis. www.atsdr.cdc.gov/toxfaqs/tf.asp?id=305&tid=55

Study after study has proven that only about 10% of the mediation gets delivered via Hand Held Nebulizer. Even with the specialty breath actuated nebulizers, the parentage only improves marginally. The vast majority is ventilated to the atmosphere. The drug and device manufacturers have taken that into account decades ago, hence the huge dosage difference between nebulizers vs metered dose inhalers. If you want about 80% delivery, use a MDI with spacer and proper technique. On a side note in this time of pandemic, a virus will very happily float around the room attached to exhaled particulate aerosol for someone else to inhale.

Hello sir I have a question is budicort 0.5 is reuseable like I open the bottol and I use half of this so can I use it tomorrow
@phillyphakename12552 жыл бұрын
it generally isn't recommended to save an ampule that has been opened for later use due to the possibility for contamination. The wet environment provides a home for germs, which will soon be inhaled directly into the lungs. Not ideal. Then again, it is reasonable to ask how risky it really is. Do you routinely clean and sanitize your hoses and mouthpiece and medication cup? Do you dry your equipment after use? If not, the breeding ground probably already exists in places other than the unused medication. Also, another thing to consider is the burden medication waste puts on you. Does it mean you are significantly less able to afford your medication? Does it mean you have to get your medication more frequently? Do you have easy access to the pharmacy? Non ideal medication is almost always preferable to no medication, so if it means you won't be taking the medication if you don't reuse, you should reuse. Also, talk to your pharmacist or doctor about this issue, people on the internet don't know you or your situation, they can't give great advice for you.

If you're old enough and not severe enough to do without a mask, I have found that simply putting the normal mouth piece end at my nose and breathing through my nose works just fine. There have been many studies and although not in peer review yet...The science is basic when it comes to aerosolizing and Covid-19. This does indeed aerosolize your exhaled lung volume..... *WHY* put anyone else at a greater risk and just not use a nebulizer in the first place?? Why wouldnt you just use an MDI? Or ask the question if Covid-19 even require the pt to need a bronchodiolator in the first place?

MDIs are in short supply. Our hospital is already running out of them. Again, we need to be thinking of all options!

@@lmellick I work in Pulmonary here. Ours are out of MDIs as well. But we do know full well that using SVNs do aerosolize exhaled lung volume. The data is not there as peer reviewed of course, but as WHO and other studies are saying now any aerosolized TX (bipap, vent, HFNC) be treated as more dangerous and be considered an airborne precaution and negative pressure room. Of course we both know we are out of those rooms. I would say just as a precaution do not order nebulizer treatments at all. I have not personally seen any relative effect from these covid-19 patients and bronchodiolators anyhow. At least that is what our team is doing currently. Stay safe!
