I’ve got severe back and sciatic pain with numbness and weakness from L4/5 and L5/S1 disc prolapse and desiccation. The prolapse is putting great pressure on my sciatic nerve root. I’m 26 years old. I remember having this issue since I was 17, but only recently diagnosed. I’ve lost the ability to walk further than a few hundred meters, can no longer do any of my outdoor hobbies and now my working life is being greatly affected.

The medial branch and spinal nerves are always at the side of the neck where the transverse process are right? if pain is on the back of the neck where you showed the spinous process is then it has to be referred there by the nerves or its the disk and facet capsule located at the midline/spinous process that are likely the culprit and not the nerve? Especially if its triggered at the midline by shrugging up and down movements?

I'm currently 56 and did physical labor for more than 30 years in construction, regularly overworking my back while doing so. I'd already been diagnosed with chronic degenerative disease 15 years ago, and feel the same sensations (cold and continuous pain in lower back, buttock, side of thigh, side of calf, and top of foot) as I did then. I'd recently reinjured my back at a different job, and have started a prescription of nerve desensitizers and steroids. Is it possible to convince the new staff at the clinics that (for me) the injection is the most direct and most effective rout for relief, rather than starting over with mobility limitation testing and physical therapy? Both of those put more stress on the already injured and painful areas, and last time it took months before they sent me for an MRI, which did result in me getting a cortisone shot.

I’ve had MRIs, bone scans, X-rays, blood tests and nothing is clear as to what’s going on. I have depression because of it and I blame my parents and all the doctors/physios who have failed me by dismissing me. I have deep resentments to authority. Academically, I’ve managed to succeed, with a 1st in Chemistry from Oxford. I now run a small consulting business, where I am profiting personally around £80,000 a year…. I want to quit it all as the back pain is just too much and too persistent. I blame everyone for neglecting me.

@@XLDJWHAT I’m so glad you asked. So insurance sucks. An xray is not sufficient for this diagnosis. So what I believe was actually the problem, was my ULTRA tight hamstrings. I have been doing physical therapy with my hamstrings and have been noticing improvements in my back pain. I believe they were so tight, it was pulling my spine inward and creating the illusion of DDD. But then again, I’ve tried to get an MRI 3 times and been denied. I’m going to get different insurance, but wanted to reply and mention… things aren’t always as they seem, and sometimes a second opinion is best. Also… sometimes doctors cannot treat the actual problem. Peptides have also helped a lot. I hope this helps you and gives you hope.

@josephsardella8047 I was diagnosed with this over 10 years ago. It started 20 years ago. I am now 55. I have ruptured so many disc's I am basically crippled. I have a back surgery also 10 years ago that did no good. They will not treat me at all unless I get more surgery. But they won't prescribe ANY pain management at all, even after. This has ruined my life. I never get to go anywhere, and live a life of complete isolation, and reliance on my husband. I finally could barely afford the diagnosis, the surgery, only to be left untreated and insulted if I ask for pain management at this stage. I have to remain mobile to use the bathroom so I dunno what I am going to do.

Spinal decompression May temporarily alleviate some of your symptoms. Traction will help bring the disc away from the nerve temporarily. Ultimately your body may or may not heal the disc. Thank you for providing this information. I appreciate it. Is DD disease looked on as a no surgery disease if there is no direct nerve impingement, by the medical community or does it differ in prognosis outcome from surgeon to surgeon? Is it hard to differentiate between the different disc levels for a definitive diagnosis? Would you ever recommend discectomy with fusion for DD disease in today's world where it is almost impossible to get any pain medication and the patient has suffered for many many years with this disease or injury? How do you diagnose this by looking at a MRI as so many times the Xray is so detached from the actual pain that the patient has. And finally and most importantly, do you feel that there is any procedure out there right now that can help a person with DD disease that shows marginal degeneration, but is suspected of having a crack or tear in the disc? I would think that if there is severe degeneration these types of procedures, like DiscSeel, would not be possible to use? Or is it worthwhile to attempt to seal the disc and pump it up if possible? Sorry for the silly questions, but many of these are questions that I am trying to deal with as my situation now has become urgent as far as the need for some type of clinical action is required. I cannot go on much longer with this discomfort level and the inability to sit or do any type of physical activity. And I do realize you are not making any diagnosis but only answering general questions, if you do respond. thanks

It typically does not cause tingling in the legs. That is typically more related to stenosis. There is a video on that.

I love your videos. I have had low back pain off and on most of my life. It has gotten worse over time. I turned 62 yesterday and it is constant now. I had a CT scan done a couple of weeks ago. Among the finding of Spondylolithsis and a string of other things at L4/L5/S1 it was also mentioned about vacuum phenomenon. Could you explain that? Also, could trauma from decades ago be the cause of this starting? I had trauma to my back in the mid 1990's in this exact area and was wondering if that could possibly be the cause. Thank you for your time.

Thanks for this. I have degenerated discs at L5 and L4. I’m only 26. Is there any hope for them to regenerate and any treatment besides a spine fusion surgery? I also have a extrusion at my L5 and that’s what I have pain in my buttock on
@DrBrianSuTheSpineGuy3 жыл бұрын
You could certainly try PRP or the new relievant procedure which ablates the nerve endings at the desk. Both are somewhat experimental but are not harmful to try prior to disc replacement or fusion. However, if your buttock pain is what bothers you the most you may want to consider an epidural steroid injection to see whether or not the problem is coming from nerve compression or the disc degeneration. You can always make a zoom appointment to see me by calling my office. As a disclaimer this is not considered medical advice and you should always seek the consultation of your spine surgeon
@jliu24883 жыл бұрын
@@DrBrianSuTheSpineGuy I saw a spine specialist today and they basically said you have 2 choices for your condition....microdisectomy or epidural shot. Dear dr. Su, Could you please give your considerations with regard to indication for and operative approach to endoscopic foraminotomy in cases where a lumbar foraminal stenosis is (mainly) caused by spondylolisthesis (grade 1 or 2), as compared to a (mainly) discogenous stenosis? In such cases, does the removal of bony material needed to widen the foramen carry a risk of aggravating the spondylolisthesis, and/or of increasing the instability of the spine generally? Thank you!
