problem areas I recommend: (1) knee massage for people with previous torn ACL, etc. (2) hip flexor therapy for runners, (3) isolated infraspinatus therapy, (4) elbow therapy for athletes/lifters, (5) hammate bone therapy, (6) tennis elbow therapy.

I try to start with the least invasive options first. In the case of low back pain, I would start by seeing how they respond to hip/glute work, which often gets me the results I'm looking for. If some low back stuff sticks around, that's when I'd be more likely to start with abdominal/pelvic work. Not a hard and fast rule though.

Yeah, I was kind of worried about that. This may have been a job for yoga clothes. I'll try to have a pelvis palpation video in the near future where it's actually possible to make out landmarks :)

I would not expect an inguinal hernia to heal on its own: healthy.kaiserpermanente.org/health-wellness/health-encyclopedia/he.inguinal-hernia-should-i-have-surgery-now-or-should-i-wait.za1162 You and your doctor might land on less invasive care options, and massage might help manage pain, but I have not come across evidence of inguinal hernias healing without surgical intervention. Hey Ian, just curious as to why this video focused on iliacus as opposed to psoas? I read your comment that you're ultimately killing two birds with one stone, but could you explain when it may be necessary to work on one versus the other, and how might you isolate the two? Psoas works most when lifting up the leg in a straight position. Iliacus works most when lifting up and to the side. Like when you are getting out of a low lying car.

I could easily make contact with it from here, but I'd need to shift my contact from the area medial to the ASIS to the area just inferior and slightly lateral. Definitely an important muscle to work with when considering anterior hip pain!

If you're doing self-massage, I'd recommend that you work much more broadly than is done in this video. Iliacus massage is great, but it's easy to overdo, and it's best to have some training when doing pressure point style massage near the abdomen. If you'd like to do some self-massage, I recommend gentle foam rolling of the hips: kzbin.info?o=U&video_id=-Y86TCXKS9w If your pain is persistent or becomes worse, I do recommend that you see a doctor for evaluation. I find this useful for any sort of complex hip pain. If the person has several symptoms that seem to center around the pelvis (low back pain, IT band pain, inguinal pain, etc), I start to suspect the hip flexors and gluteus medius. While I can often get good results without targeting these muscles, I find they're often involved when the pain is otherwise treatment-resistant.

Hello Ian I had a question about massage but off topic from this video! I am a LMP as well as was wondering if you had any advice for a client having an emotional release on the table, anything that may help comfort them while its happening.

I think it's easier to start with what not to do: Don't feel like you need to ask questions or help them delve further into their feelings. Don't try to fix anything or offer advice. There are lots of right things to do. Here's my usual approach: "I want to acknowledge that you're going through something right now. Would you like me to continue with the massage, or would you like to take a break?" If they choose to continue, I might slow down just a bit to not be too distracting. Once things seem to have blown over, I'll offer them a tissue and some water and ask how they're feeling. It's good to stay in communication afterward, because the client might feel a little awkward about having had an emotional event. Silence can seem like reproach, so make sure to check in. Then, keep on keepin on :)

We can't do anything about the disc degeneration, but it's important to keep in mind that all sorts of non-invasive treatments can reduce pain and dysfunction without intervening at the level of the disc. As massage therapists, we can soothe the muscles of the hips and low back, massage and stretch tight muscles, and reduce sensitivity by providing painless contact. I would spend extra time with the external hip rotators.

I'm of the opinion that any work with the iliacus will also effect the psoas. Not only are they near neighbors (eventually sharing a common tendon), but they undoubtedly have some fascial and nervous system entanglement. Still, my intention here is just to contact iliacus and glut medius.
