The literal giant is called Devil Rebirth, he was in the original series. He's called that because of him being a literal giant, being over hundreds year old, and survived numerous execution attempts, that they just gave up and lock his ass in a bomb shelter. No idea whats his story in this game though.

To be honest, I prefer the 2nd opening with the song Silent Survivor over the 1st with Ai wo Torimodose. That intro doesn't need to throw a lot of crazy shit at you, it's just calmly badass like Kenshiro himself. Also, Ken doesn't hate Shin at all, despite everything Shin did to him. He still considers Shin his friend after Shin died.

Yeah but he was living in the city with Yuria prior to the bombs falling. Remember that Toki was going to be the successor before the war, not him (and was going to turn it into a healing art to help people), so he was free to live a normal life.

It's more a case that this stuff simply wasn't in the radar of mainstream Japanese culture at the time. The only way little Shota could get away with reading manga like Devilman or Violence Jack in the 70s or Fist of the North Star in the 80s was because Mommy and Daddy didn't really think to check out it's contents. In fact FOTNS was probably one the first series that actually caught the attention of parent groups in Japan and were horrified that a show with as much violence as it did was being targeted at ten year old boys. The result of this is that the movie had its violence censored in later editions. Even today the reason that there is so much fucked up shit in anime and manga is that the mainstream simply doesn't know about it. And in Japan people typically share their hobbies with the mainstream world so it stays underground and unregulated.

I feel like I’m in the worst position watching this in that I know nothing about fist of the North Star, I can’t understand the language, I much prefer the wackiness of yakuza than this (apparently) much more serious tone and scenery, and I’m fully aware of how unfair it is to make a real judgment with these in mind.
