My guess would be if you located a provider on the North American menopause society website, the provider could provide provide you with a source. I just finished my personal training certification and I plan to partner with some providers myself. After going through the perimenopause weight gain myself having paid 4 trainers who gave me cookie cutter workouts and nutrition plans designed for women in their 20s 🙄I decided to get certified and at 48 My physical fitness and overall health has drastically improved.

I am SOOOOOO excited! I just went to my appointment with my gynecologist and she HEARD me! She listened and with compassion and empathy prescribed me testosterone, progesterone and estrogen! Then- she referred me to The Pause Life! I have been following Dr Mary Clare for about six months already- which is what empowered me to ask for a referral back to my gyno who was my provider when I was younger and going through issues with fibroids. I feel so loved. It is wonderful to feel validated and supported. My terrible pcp did me one solid and granted my request for a referral. She is a woman and about 30 and looked at me like I was pathetic for wanting to have a sex drive, keep my muscle mass and bone health and not have reoccurring utis and urinary issues. How dare I not want to shrivel up like a prune at 51??? She told me I needed to temper my expectations because I wasn’t going to feel 20 again. 1:30

Please let me know how the patch is working for you…I have an appointment with a gynecologist (haven’t been to a gynecologist since I gave birth to my daughter 14 years ago at 39 years of age!😬) in April and am really hoping I would be a good candidate for estrogen patch! 🙏

I started with the .25. And didn’t work And then up to .05. And felt a difference . Stayed here to so far….. I still get hot flashes but not near as bad as I was

I wish so badly that my mom could still be a candidate for HT. Sadly she was deprived of it 20+ years ago because the prescribed meds for her was SSRIs and I am so angered by that. By coming across Dr Haver and all these information coming to light now so I can begin to take care of me so that I can be as healthy as I can be and be there to support her and my dad. Thank you for fighting and giving voice to the need for HT in women’s health!

This is fascinating! I'm also having a SAD EPIPHANY of the CULTURAL BIAS I have as an American in my 50s when listening to a language difference between doctors. It makes me feel like crying. Time for a podcast series ladies! Both of you together have so much to share and it’s essential to get this info out there! I’m an RN and have learned so much in this one conversation! Also, what if a post menopausal woman is not obese but immediately gained 15 lbs after being the same weight her entire life and cannot shake that extra weight! VELCRO! Meds not an option? Only food and exercise?

In this scenario you have the opportunity to lose weight with lifestyle. Eat meals and avoid snacks. Make sure you get protein, fiber, and healthy fats at each meal. That's a terrific place to start. No night-time eating is important too.

@@heatherawadmd7858 i tried that it is not sufficient ! in France they advice progesterone more than oestrogene for post menopausal women ! why you did not talk about progesterone treatment only ? there is so many approaches that women are lost ! we do not trust the pharmaceutical industry anymore too ! Whilst WHO has recognized Menopause as an illness, it is not given the right attn by the medical corps saying one thing and the other every other year !

I still remember the phen-phen craze in the 90s and all those young women taking those drugs to lose weight and ending up needing heart-lung transplants to live due to those drugs. I will wait this out to see how this goes over time.

I hope it all goes well. I love my HRT been on it 14 years, have reversed type 2 D but it keeps coming back as I just cant exercise much due to leg hardware and my appetite is voracious, always has been. How did you convince your doc to give Wegovy to you? here in Canada they say I'm just borderline and eat less exercise more...well it works to a point. I just cant lose the last 40 lbs...

The doctor will prescribe it if you meet the criteria for Wegovy which is BMI of 27 with comorbidity such as high bp or high cholesterol or BMI of 30 and up, But my insurance wanted me to prove to them that I attended a weight loss program, which I did with my previous Dr. Anyways I’m experiencing weird side effects with Wegovy. I have severe muscle aches and fatigue which are nowhere mentioned as Wegovy side effects. I just realized it was from Wegovy. I’m planning to stop it. I’ll just continue exercising and try to diet. Of course not everyone is the same. I wish you luck with your weight loss journey.

HRT definitely put me back to my pre-40 shape, lost tonnes of weight in one year , maybe a bit too much, but I was always trying to keep weight on. So this explains perfectly why I've always had issues with keeping weight on me pre-40, it was estrogen !! Tha KS ladies xx

Great discussion!!! Due to my family’s cancer history, my oncologist and gynecologist (she introduced me to Dr. Haver 💙) agreed HRT isn't an option. So diet, exercise, lifestyle, and building muscle are paramount to navigating perimenopause and thereafter.

Have listened to so many of your podcasts. I lived in France,I am old to start HRT (77) but my Doctor reccomended it bec. i broke several bones inc hip and femur since starting menopause. Felt so much better since starting HRT re sleeping, bladder control etc. Then moved to London and had an NHS medication evaluation by pharmacist, she said, do you know "HRT causes Breast Cancer and stroke"w/o a family history. "R u willing to take the risk." Was so upset and scared after speaking to her.

when it comes to menopause treatment i feel France is so much ahead ! I am French and was living abroad and it is a nightmare how they approach menopause! It is just unbelievable the worse ever are the dutch who consider menopause not to be a chronic illness and will tell you we will not test your hormone level just accept the change and deal with it! they just recommend to take vitamines that is on top you are paying from your pocket not covered eventhough recommended by the Dr! Women are treated like in the medieval system in Netherlands , only country in the world i believe who push women to natural deliverance during pregnancy wto morphine or anything;..so much stories there the world must know ! all the foreigners go back to their country origin for treatment coz Dutch women are so used to the system they just accept the system as it is !

I am 53 going on 54 this year… I am so relieved to hear that I’m not going insane and there’s a reason for my madness. Last year i got my periods in April and this year in January. I thought I was done with them!🙄Both last year and this year have been really tough… lots of weight gain, even when I continue to walk 2-2.5 miles a day, do 20-30 minutes of yoga daily, experiencing hot flashes and night sweats, feeling achy, sad and sometimes frustrated. Whenever I mentioned my symptoms to my endocrinologist she prescribed gabapentin. I do not want to take that medication which is for depression and has many side effects! I scheduled an appointment with a gynecologist that still is a few months ahead while I continue to suffer😢 I am hoping that she will put me on HRT. I hope that I am still a candidate although I’m almost reaching post menopause. I have added more fiber, less carbs, and more protein in my diet. I have been taking vitamin D for years! I do not have diabetes or hypertension. My cholesterol is a little up but do not need medication. I am hoping HRT will help stabilize me!🙏

Excellent episode! Thank you! I am on 😮 tomorrow I will be putting on a estradiol patch and a tiny bit of testosterone gel for the first time. I am excited and hopeful it will help with my post menopausal symptoms. I will turn 58 at the end of the month and had a hysterectomy 2 months ago.

Iv learned from research as well that the hormone insulin raises as our oestrogen declines, insulin is a storage hormone and when we are in a chronic state of high insulin levels not only dose this put us at risk of insulin resistance but also the risk is diabetes. Fasting for 16-20 hours can really assist with keeping our insulin in check by draining our glycogen tank between meals/drinks. Weight loss is a lot easier when we fast and there are studies that suggest that fasting can actually increase our metabolism, improve inflammation, heal our body. It’s worth incorporating every tool we can to help our bodies working with them for the most positive outcomes.

I started using HRT patches because of very low Hormones and Brainfog. After a week on HRT i had so many side effects like worsen Brainfog,Depression,Nausea, Memory loss,Slow thinking,Tiredness and Headaches. The only benefit that I got from using it was better sleep. I have stopped using the HRT patches for a week now and all the symptoms disappeared.I guess it is not for everyone.

You need to watch macronutrients (make sure it’s accurate for your weight and weight loss goals), stay hydrated and sleep, sleep, sleep. HRT is not the holy grail on its own. You have to weight train .. get protein right at 1g for every pound of body weight. This, along with HRT will work but noting on its own❤

@@sussannekeith5676 I’ve been watching macros for quite a while not and average 90-110 g per day and prioritize fiber as well. i’ve tried not to get too restrictive on other macros is I kept protein and fiber in check. Maybe it’s time to cut calories…?

I'm not lower to take HRT because of borderline ovarian tumor. My total oopherectomy and hysterectomy was 6 months ago. I've been doing clean keto with intermittent fasting. I did two extended fasts for 5 days of water only. I've had much success. I haven't had major hot flashes and my sleep is still good. I don't want diabetes or bone/muscle loss. 😢

Probably bc this is how our bodies are made to be as we age. God designed it to be this way but I agree with the doctor that if there’s a way to minimize the symptoms so we can live our full best lives and it doesn’t jeopardize our health Why Not ? Right

The slowing down of the metabolism is associated with the drop in testosterone as we age, and muscle loss is also associated with the drop in testosterone as we age! Estrogen slows the metabolism down because it directly influences the thyroid, estrogen slows down thyroid activity and we burn less calories! It's absolutely true that estrogen reduces and prevents visceral fat though, because it regulates insulin!

My oncologist told me no HRT because my tumor was triple positive. In fact, the prescribed estrogen inhibitor. Did I understand correctly that HRT is ok despite breast cancer? I fit your description exactly. 1-2 lbs a year. I definitely feel muscle loss. My old workouts don’t seem to work anymore.

The answer varies by individual with the exception of vaginal estrogen which is safe for everyone. Other types of HRT are a more nuanced discussion to weight your health risks versus your benefits. After watching this video it is so dishearning to know what I have been through during me pause that could have potentially been prevented. At 51 I stopped having periods and 3 months later had extreme panic attacks along with anxiety. Along with the classic symptoms I had one that doctors could not figure out for 9 months. I had stinging, burning, electric shocks 24/7. I was sent to every specialist you could imagine. They put me on anti depression drug, an anxiety med that I cannot get off of and I am 67. My gyno unfortunately is not a proponent of HRT so I suffered 7 years before I found a doctor who would put me on HRT. I refuse to be without it now, unless there is a health issue that will not let me continue. Breast cancer does not run in my family. I wish I had this k owledge sooner. We need more educated OBGYN's who special woman's health AFTER having babies. That's all most of them know. How to deli er babies!

Do you want to get off your antidepressant (Effexor, etc)? Your MD needs to be prescribing a ‘Cross-taper Withdrawal’. With the way Effexor half-life works, it’s typically 1 dose of fluoxetine. The half-life of the latter is so long, it can actually be taken once every 7 days. In contrast, venlafaxine has a very short half-life, so that’s why the acute withdrawal (‘discontinuation syndrome’) hits hard when people try to stop. The Pharmacist may have to the explain this ‘Cross-taper Withdrawal’ protocol to the MD so they can Rx it.

Very educational! I was 25 when I got a total hysterectomy so 3 years ago, due to severe endometriosis. My doctor was a male so for the first year he wouldn’t prescribe me any type of HRT. My hot flashes got in the way of work & daily life. Hot flashes would make me loose my breath. Now I’m struggling with the tummy fat!! This gave me a lot of insight. Thanks so much!!

Losing your ovaries at such a young age puts you at risk for multiple health issues later in life unfortunately. This subject includes osteoporosis, heart disease , dementia. You should be on hormone replacement until the normal age of menopause. You need to find a competent doctor quickly.

Just reread your post, do you mean that your in informed( idiot) doctor waited a year after your total hysterectomy and THEN gave you hormones??? They should have given you an estrogen patch as they wheeled you into the recovery room!!!

@@lindajones4849 ohh yes! i quickly found a new doctor who explained all the negative effect s of having 0 Estrogen ! it is crazy how much can be affected! i have since been put on HRT and feel absolutely AMAZING! my quality of life has done a complete 360. now i can live like a normal 28 year old :)

If you don’t have bad symptoms of menopause should a woman still consider HRT?? To support bodily functions- skeletal, etc? I have minor hot flashes. Should one consider only taking supplements instead (D, mag, collagen, probiotic, Fish oil, etc.?)

I have been on BHRT testosterone and progesterone for 5 years. I started Estrogen a little over a year ago because I started having hot flashes. I have a new Dr. and they took me off the Estrogen because my mother had breast cancer. =(

How Dr. Haver described the way semaglutide works in helping with weight loss is exactly how I'm experiencing its use. That is, semaglutide is not a magic pill. However, it is allowing my body to actually respond to reduced food intake and exercise to actually impact weight loss. I tried EVERYTHING and since going through peri and post menopause, NOTHING worked to lose weight. In fact, it seemed before taking semaglutide, the more I tried, including fasting, the more the weight would pile on when I did eat. I'm actually encouraged to eat better and exercise with using semaglutide as it is allowing my body to respond to lifestyle changes more easily. It was exhausting constantly thinking of what and when to eat with no results. I'm grateful for semaglutide because I can actually LIVE and just feel like a normal person who eats healthy 70-80% of the time, exercises a few times a week and not constantly be in diet mode.

The problem is that you can't stay on them forever, and the weight comes back when you come off those drugs. They didn't talk about that.

Thanks for this incredible, informative conversation! These are powerful tools, hrt & glp-1, that can change the lives of women. My struggle started right as I turned 50 - with physical back pain, sudden 12-15 lbs weight gain, and a loss of "sense of self" - which were summarily dismissed by my 35-ish female primary care as normal aging. One year later with copious research & self advocacy , I am symptom free with hrt (combiaptch) and lost weight with 4-5 months low dose, carefully titrated treatment of compounded semaglutide. Thus, I am a great example of protocol and wisdom shared here. I encourage all you beautiful ladies to keep going!! Also, would love if the community could share more about smart, careful ways women with moderate weight loss goals can leverage more affordable and accessible compounded semaglutide to achieve goals and quickly break the 'compounding' effects of weight gain at midlife.

I have heard synethic estrogen causes weight gain and cancer advised to take progesterone to help balance all hormones. And taking compounded HRT is the healthy way. I see a natural Dr that told me this as well. I just can’t afford to go to her anymore.

This is an extremely frustrating issue for me. I have history of a blood clot after a hysterectomy in 2023. Blood clot was gone within three months. I have yet to find a doctor who will prescribe HRT for me because of that blood clot. I’ve even brought up YOU and the things you’ve said about this not being the case (and that I just need a specific kind of HRT), but they still refuse to prescribe. I am beyond frustrated and upset, as I’ve put on so much weight since my surgery. I’ve tried restricting calories. Fasting. Exercising more. The pounds keep coming on. I went from a size 8 to a 14 in one year. My joints hurt. My acid reflux off the charts and barely controlled with meds. My blood tests show I’m pre-diabetic. I’ve been in tears at office visits. My hematologist just referred me to an endocrinologist, who she hopes can put me on some kind of weight mgmt medication. I have to wait a month for an available appt. Sigh. I hope it’s not a waste of my time when I meet with them. I cannot continue like this.

Great information. I am fortunate that I have affordable access to both HRT and GLP-1 treatment. I did not understand the difference between medically supervised obesity treatment versus the off lable/medi spa treatments. My OBGYN did not have a solution to my weight gain, she told me the HRT would only treat my night sweats, and that perhaps once I was getting enough sleep I would lose the weight, that did not happen. I was able to get a medical weight loss appt because of my BMI and borderline A1C & elevated CHO, both which I never had in peri-menopause. I live a healthy lifestyle so the weight gain was surprising and weight loss resistance very frustrating. I still am curious to see if the healthy weight loss from GLP-1 while maintaining/building muscle post menupause is maintained if you go off the GLP-1. Thank you both!

Dr. Haver is not a fan of pellets because the dosage can be difficult to control and they are not FDA approved so you don't always know what you are getting. ~ XO Community Support Team

This is all great to know, but here in Toronto I can't even get a family doctor to see me never mind believe me that I would like HRT. The last walk-in clinic I went to, sure enough prescribed me anti-depressants.

I had to fight for a year and a half went to gynaecologists General practitioner who would not listen spoke to a menopause specialist and she confirmed that all my symptoms were perimenopausal I wanted to cry. Thank god I didn't take the anti depressants.

Hi I live in Toronto! Prosper menopause on line is a great platform of nurse practitioners that will give you a thorough consultation and prescribe you hormone replacement. I have an appointment with Dr. Damian’s Castleman. She is an ND that is also certified as a menopause practitioner and can prescribe health Canada approved products. These are great options, however, depends on what benefits you have because these are out of pocket costs. However, the prescriptions they give you are covered under any drug healthcare plan.

I could be wrong but what I thought I heard was them say that testing isn't as important as the symptoms because one person at 50 is happy while another is in hell. I was frustrated when my gyn said the same thing to me.

I would suggest asking your Dr to order the blood work so you can get a baseline for future reference. If she outright declines ask for reason. If she makes you feel uncomfortable, questioning your own decision etc, you might have to switch providers. That holds even more weight if you are experiencing symptoms that can not be explained by other things. Listen to your gut, Doctors don’t know everything, smart one know that and are willing to learn.

How can you find a provider that will listen…I’m in menopause..I work out 5-6 days a week which focuses mostly on weight training…I have gained 20lbs and cannot lose the weight…5’3” and have maintained 177-120 until 50…now 140 but told I don’t fit the bmi requirements to qualify for the glps…so am I stuck until I reach the obese category?

My doc wants me to start hrt for-my bones due to having osteoporosis at only age 33. I am terrified of the weight gain people have said they have experienced when starting hrt.. I’m confused as you say hrt actually helps with trimming belly fat, but then others say it increases….can you clarify?

I went to my appointment to see my OBGYN i am 42 my i went from having a normal cycle to having a cycle thats irregular i was having pain on my knees hot flashes and crying spells agitation but more crying spells she put on estradiol patches 5 mg pegestrone and estradiol cream 0.1 mg for frequent urination

I eat mostly keto to carnivore at least twice a day. My blood glucose is basically 82. I do kettlebell workout. Estradiol pill and progesterone at 47yo. I'm good 😊

Technically I am not an HRT candidate anymore because I had stage 0 insitu that was determined to be estrogen fed. I have packed on the visceral fat in my trunk 😢. I have seriously thought about stopping the anastozole, but I don’t think anyone would let me use the estrogen patch again. I already had osteopenia and type 2 diabetes- not over weight. Very frustrating.
