I've been dealing with metatarsalgia. As someone who has been very fit for the last 8 years, I'm terrified of this condition making me unfit eventually and taking years off my life from all of the movement I've been missing because my foot hurts so badly. I dont see it getting any better because ive been to 2 different podiatrists and the course of action is almost non-existent.

I am pretty sure I have a severe case of Metatarsalgia on my right foot. I went to see a specialist and he recommended aetrex memory foam Orthotics and some Physical Therapy. Physical Therapy is treating me for Plantar Fasciitis. They also recommended I wear Metatarsal pads, which I have tried. Without a metatarsal pad and using memory foam insoles the 3rd and 4th toe on my foot feels tingly every time I walk. As soon as I put a Metatarsal pad on and walk most of my toes becoming tingly, then my foot feels like it's swelling up. I have gotten MRI /Xrays/had EMG/had cortisone injection in Plantar fascia area a few weeks ago of/on my right foot and all test came back negative for any kind of Neuroma or Nerve Damage. I also had an MRI of my back as suggested by my podiatrist and read by an orthopedic surgeon for backs and he stated that my foot pain is not related to my back. I tried the Orthotics that my Dr. suggested, but they made my feet red/tender and my toes tingly/numb. I am really at a lose for what to do to make me feel better any suggestions would be greatly appreciated.

We can't comment specifically on your case, but theoretically if we are able to take full pressure off the front of the foot with a cast or shoe, we can confirm it is the foot. Something to think about. We usually find it.is always too much pressure on the front of the foot.

That is a vague answer for sure, but just because one orthotic wasn't powerful enough, does not mean further padding can't get it done over time combined with PT and increased flexibility and loss of weight, we find patients are most successful with this treatment.

One of the most important things to remember is that nothing is better overnight! Even with great shoes, great orthotics and offloading weeks can go by with minimal to no relief, but at that point things start to get better and should stay better. Think about this like breaking your ankle, you have to wear a cast for a few weeks or even a month or two before the pain starts to go away. It's not going be completely fixed in just one night, but it will gradually get better. If it does not then you should probably be seeing your doctor anyway!

So true. The base of my second toe hurts every time I flex it a certain way. Xays showed nothing except my toe starting to cross sideways from a bunion. Dr suggested wearing toe separators and to change my shoes. The pain subsided after three weeks (no painkillers) i thought it wasnt going to work but I think i can work out again now. Altho I'm wondering if wearing these toe separators are gonna be for life because of the bunions

I have been dealing with Metatarsalgia for 8 months now, particularly in my left foot. I have been non weight-bearing on it for 7 weeks now and even with a good shoe (Hoka) and inserts I am still unable to walk without burning pain. I have developed the beginning symptoms of the same problem now in my right foot since using a knee scooter to rest the left one. I have been referred from one doctor to another because no one seems to know what to do. My current ortho doctor ordered custom inserts and titanium plates in my shoes and sent me on my way telling me he'd see me in 3-6 months. I've tried all the treatments they suggested which you also mentioned in the video. Nothing is working and I am in constant pain. I also have numbness and tingling in my left foot and leg as well as cold sensations that start in my foot and run up my leg. What should I do when there has been no improvement for such a long period of time? It has been absolutely debilitating! Is it normal to not see any improvement even with treatment? Would you recommend physical therapy or massage therapy? I don't know where to turn! Thanks for any thoughts you may care to share!

We would love to be able to give you a more specific answer, but unfortunately we cannot do so. These are very complex medical problems that do require specific evaluation in both physical and imaging modalities. Please see a podiatrist near you for further evaluation. We would be doing you a disservice with general advice for such an important issue. Your podiatrist or primary care doctor can likely help much more than we can in this instance. We really do appreciate you as a fan and we hope to bring you more detailed videos that help more in the future!

I have to wear safety toe work boots. I just went up a half size and went from EE to EEE width. It seems to be helping all the streching and laser and massage treatments seems to be a losing battle against 10+ hours on concrete.

There are some studies showing possible injections into he ball of the foot with harvested fat, but to me that seems extremely excessive and dangerous to simply avoid wearing better shoes for heavy duty activity. Surgery is not risk free and you are always rolling the dice. Our philosophy is that if it's not causing severe pain and it can be fixed with inserts and shoes, do it that way.

Metatarsalgia is really putting the squash on my hiking and trail running! I routinely hike 10 to 16 miles or jog 3 to 7 miles (strictly dirt). I am into my 3rd bout with this injury. From one foot to the other and back again. I know it's probably from overuse. I've recently added jump rope too. I wear Altras (trail runners) for just about everything, and when I dont have Altras on, I am usually barefoot. Trying some Oboz inserts in my Altras to see if this helps. I have a super high arch. Paws crossed. I'm sick of this recurring injurry. ☹ Thanks for the video. It was helpful. 😁

The idea with a metarsal pad is not to put it on the bone, but to have the pressure spot hang over it like a cliff so that there is no pressure on the bone

Very helpful & informative! :) I am in healthcare and work 8 hrs on concrete floors for almost 30 yrs I used to run and walk very far distances but I havent since I hurt my same heel twice :( I have custom made orthotics for metatarsal...got my first cortisone shot ..( I have heel spur.)...nothing has helped so I'm trying to heal them in other ways to avoid surgery Once the covid is over im going to the podiatrist and physio to get laser treatments on it I wear a good sneaker and I'm not overweight so praying new orthotics and other treatments will help :)

A good shoe brand should easily accommodate the insole. Loafer or non-laced shoes may be tougher, especially dress shoes. Proper fitting shoes and higher quality shoes usually fit better, last longer and don't have that issue. Hope you feel better!

i went to get some xrays and ultrasound done today.....I've been having these same symptoms for a month but until i don't get those tests back i won't know what's up....riding my bike helps ....swimming helps...... resting helps I've tried padding on my shoes and it hurts my achilles heel...i hope time will make it heal and i hope that's all i need. i work in a factory so standing up for long periods of time has taken its toll on my foot

feet have chronic condition (both 2nd and 3rd toe). Tenosynovitis, mortons neuroma, bursitis and capsular synovitis. I already had my 2nd shot of steroid wc worked but lasted for 5month only. I have 1 shot left and dr. Refuse to gve after the 3rd. Im afraid if it doesnt work, ill be carry ths pain forever. There are a lot of factors in play, but most of our patients see a lot of almost immediate improvement with that type! There is always fine tuning that can be made with evaluation, but tough to do without seeing the foot or having someone walk for us.

I wear custom orthotics that have healed my plantar fasciitis. I finally started doing cardio again and now metatarsalgia is killing me. I've requested a podiatrist appt but I'm pretty sure they're just going to offer another injection. So frustrated.

Pain while sleeping could be the soreness that developed during the day of walking on a sore foot. It's common for people to not get sore while walking but then at the end of the day have a painful throb while on the couch or sleeping. But always get it checked out by a professional!

Great video message even if the audio is lame. Intermittent music is way too loud and not needed. The voice, though in a whisper makes me feel the sincerity and intimacy of the message, which is great until I turn my volume up and the loud, blaring background music jerks the children awake to scream uncontrollably and the pets start to bark and howl, and then the neighbor's pets begin their . . . but then I finally got bailed out of jail and watched the rest of the video thanks.

my foot pain came after a week of low activity, I was fine in the morning, when I walked out of work (where I sit all day) it felt like I'd been standing on my feet all day. That was 2 weeks ago. I limped around for 10 days, then went to the doctor. I've got inserts on the way... and got a cortisone shot. I'm over 48 hours out now and it's only helped take the edge off. This seemed to come out of no where and is not going away fast. Rest almost seems to make it worse... I'm usually fairly active outside work and it's really getting in the way. X-rays were fine... how long does this take to go away? #Frustrated

It depends how much pressure you need relieved, the inserts and really good running shoes have generally yielded the best results, injections are only meant to take the edge off, not to fully solve the problem, good luck!

@@DrTomBiernacki minor update. I started using a CBD oil rub and noticed immediate relief. It didn't disappear, but was muted. I also now wear Hoka One ATR 5 shoes. They've completely relieved the pain.

I don’t work for them but can 100% recommend the brand Sole for both their inserts and shoes especially their products with built in met pads. It hasn’t cured my metatarsalgia but they’ve made it more manageable.

Im pretty sure the fact i walk to the ball of my foot is causing my metatarsalgia cause one of my legs is half an inch longer than the other, i dont think i need an xray to know ehats goin on

The bones are broken and remain in use and any muscular construction spasm become nerve noise so while those clever toes are weakened by disuse due to pain their function in the musculature supporting the medatarsal arch and the big side arch turn into shoe paddles holding a body up. I think a support for all the toes and medatarsal and lengthening oppertunities to streatch and flex awakening and engaging the toes intrinsic muscles of the foot and the as ending musculature of shins and medial thighs so these clever phalanges become reconditioned to muscular skeletol function is the universe where feet are feet not just miserable shoed in market targets. And not a podiatrist either, just a biped and so sick of the foot pain in general.

Went to 2 podiatrist 1. Said i have arch pain The other one said i have plantar fascist. Both did xray Howver i feel like both of those diagnosis dont fur However after doing my own research i belive i have this

Not specific to you, but we find what helps our patients the most is icing and inflammation early on. Only later does it make sense to do some of the more advanced treatments.

"Why this is not working" is an inconvenient truth that a lot of doctors don't want to tell patients, because it cuts into those nice profits of conservative care. Those $60 orthotics the doctor got for free from a partnership deal, that $500 shockwave for a total of 30 minutes work, those steroid shots that are counter productive but they get reimbursed hundreds from insurance, none of them get to the root cause of why this problem becomes chronic, and that's that collagen doesn't heal well. Our bodies have not evolved to heal tendons and ligaments the way we heal bones and muscles, and once they exit the inflammatory stage they lay down disorganized, uncomfortable scar tissue. There is absolutely nothing you can do about this except have surgery, after which you'll be FORCED to stay off your feet, and because of the gigantic increase of blood flow due to the incision itself, you will heal better after surgical intervention. Some people MAY get lucky from conservative care in that they have a pain reduction as the scar tissue remodels over time, but most patients in the comment sections of these videos are months to years out and got 0% improvement despite trying a dozen "conservative care" options at a time. I'm going to try the Tenex procedure next month to hopefully just suction out some of the diseased tissue. I'm done messing around with this.

Absolutely right, the custom orthotics are always preferred if possible. But most people can't go from zero to a massive insert instantly, it's better to ease into a mild or moderate insert than from zero to 100. Not to mention most people don't want to go straight to $300+ without knowing it's going to work.

Michigan Foot Doctors you are so right. I tried just the running insoles at Walmart (to try it out) and those, along with resting my foot, helped me to finally get better. It took a few months but so much better now. Plus, found great sports shoes. I’m a teacher, so on my feet for long periods of time.
