ABOUT THIS VIDEO:
When we get a diagnosis of insulin resistance or metabolic syndrome is given, patients often want to know which is more effective: lifestyle or metformin. The NEJM published landmark research in 2002 showing that lifestyle is clearly more effective IF it involves intense lifestyle changes. This included 1-on-1 training for 6 months, a 7% weight loss, and over 150 minutes per week.
Today we have found that HIIT, resistance training, and low carb diets are also especially effective for preventing diabetes, heart attack, stroke, ED, and dementia.

🩺ABOUT DR. BREWER
Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on the run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer provides services via telemedicine or in-person if you're in the Lexington, KY area. We find a lot of undiagnosed prediabetes or insulin resistance. Treating unrecognized risk factors like prediabetes allows reduction of risk and prevention of disease.

I was in aspirin 162mg. Metaprolol. Lisinopril. Metforman, glyburide, lovastatin. And tamsulosin. I was a walking pharmacy with stage 3 kidney failure. I had a triple bypass two years ago as well. When I went on the Keyto diet I quit all my meds except the tamsulosin. I lost 20lb over 4 months and my A1c went from 8,5 to 5.9 Hdl went from the 30's to the 40's, trygliserides went from 174 to 100, and my kidneys improved to stage 2 (eGFR from 55 to 72). I think my diet and medication was killing me.

I like these older videos better than the newer ones on this channel that are snippets taken from the live events that seem to jump around a lot. The live events are great but these types of videos like this one here provide lots of great information.

@@PrevMedHealth hello there from England UK, I have been type 2 diabetes for over a year , i have just suffered a branch retinal artery blocked occulous, my dad suffered 12 strokes , which killed him , I've gone back on metformin, artovastatin , my cholesterol is sky high , I'm taking aspirin 75mg , my vision loss in part of my eye is permanent they tell me, im eating porridge oats every day , im disabled in chronic pain in my legs , a double groin hernia, a damaged coccyx tailbone, pilonidal sinus disease, anal fistula , and fissure in my rectum and anus , burning stiff legs when I walk and lose my balance sideways constantly. I'm on atenolol , lisoprinol , esomeprazole, buprenorphine patches and zapain 30 /500 , baclofen, amitriptyline, doxasozin, pregabalin, and nefopam , metmorphin prolonged release , ,one of my kidneys is damaged. My coccyx is stuck in my rectum ,pushing hard against my anal leviator muscle. I'm also taking naproxen but I try not to take them every day because I read they can cause stroke

That is an excellent point. It’s what I do. And it’s what I encourage for patients that need it (most do). But it is a big emotional leap for a lot of people to start a diabetes med. They have to get over a lot of denial. And the question always comes up. My policy is to be totally transparent, provide the facts and encouragement, and support the patient’s decision.

I'm entering the Dean Ornish program January 9th. You ruined my weekend lol I was happy cause MY A1C was 6.1 couple years ago, now 5.7 and glucose was always between 90 and 100

I’d be concerned about A1c of 6.1 and 5.7. Have you had an Insulin survey? Or at least an OGTT?

the studies actually show animal protein paralyzed insulin. But we shall see, I know last time I went plant based Mt blood sugar went down, but I also lost 10% body weight

Stan Efferding say there is research stating that a 10 minute walk, 3 times a day, after meals, is more effective then Metformin. True? I don't know. But that guy seem pretty knowledgeable

Try taking the walk and taking metformin. I have noticed a 10 minute walk after a meal is a big help in preventing a big spike in my blood sugar. I also take metformin, which I have been taking for 35 years. I am 80 now,,,,,,,,

my A1C for last 10 years has been between 5.6 and 6.1 I just had blood work 2 weeks ago and it was 5.7 glucose was 100. I was expecting much higher since I've been eating a lot of junk 6 months. I have heart disease, I can't become diabetic on top of it.

HI Dr. Brewer, so after all my results came in Amy decided to not start me on metformin and added Berberine instead. I guess we will see if this works to somehow get some consistency in my blood glucose. I'm hoping it does! It's a challenge to figure out what I can and can't eat. Because, even healthy things (low carb whole foods) spike my glucose levels at times.

Managing our blood sugar as we develop IR is not easy. We want to minimize time above 120. But dropping below 70 can be dangerous. Be patient. I see John’s sharing helpful experience in this area. Have you read Blood Sugar 101?

I don't know if I had OGTT, I just had blood work done for my cardiologist. I'll look at my blood work tomorrow, what should I look for?

I'll keep you posted when I start Ornish program. I still have yet to see high fat diet reverse CVD. Would to see it reported in The New England or Lancit. And didn't you say lifestyle trumps medication for diabetes or did I misunderstand

Lifestyle trumps metformin. But it has to be effective; e g like losing more than 10% bmi.

OK. when I was plant based I started at 160 lost 15 pounds. then I went back to eating animals and I back around 160

Like you have said, the Cardiac Stress test is overused by cardiologists. I know a few people that have been found to have had 99% blockages in their arteries and just a couple of years before that they passed the stress test!?!?! So, how helpful was that in identifying their growing blockages and preventing more blockage or an event?!?!? Whoa--not helpful at all!!

Good point. Tim Russert (Meet The Press) passed a stress test in March. Then he died of a heart attack in June. This was well publicized.

shouldn't drugs he last resort, it's obvious most chronic diseases are from the foods we choose to put in our bodies. then instead if changing food people rather take a drug

Thanks for the request. Mine’s 87.3. It’s in the 58th %ile for a 60 yo male. And I plan to do one. It’s very different from CIMT. But we do use it. A very different interpretation.

well.these guys always refer to countries that have the lowest rates of heart disease, type 2 and cancers and state they are mainly plant eaters like Asia Africa. Of course Asia has now adopted the standard American diet and there diabetes rates have skyrocketed

The traditional Masai also have excellent health and they eat a lot of meat, dairy, and drink cow blood. Probably the biggest thing you can do is stay away from processed foods.

OK thanks like I've said a million times Asians, Africans and south Americans live on starch based diets for 1000's of years with extremely low rate of type. But if you eat animal protein and lots of carbs, yes you can be in trouble. But I'll say it one more time, Asians live on primarily white rice at every meal and before 1980 had 1% diabetes, when they took on THE SAD way of eating their diabetes went though the roof. Did you ever do a plant based diet. I don't believe potatoes are bad for people, unless you put butter or sour cream on it

John Lorscheider owe shall see, according to many Dr.s diabetes is not caused by sugar but by animal fat clogging cell receptors. Dr. Neil Barnard published studies that showed plant based diet lower A1C as much as medication. In addition the Asians used to have lowest rates of diabetes about 1% living in mostly white rice and vegetables. But we shall see, only one proven way to reverse CVS and that's with a super strict plant based diet
