It’s not the job of the FAA to “mitigate the impact to operators” it’s their job to bloody well regulate US aviation! That means grounding stuff when safety is at risk.

Nope. According the the FAA, Vulcan doesn't even need to be investigated after the side booster blew up, so apparently they're not that concerned about safety either. Just how good the lunches bought for them are.

@@andywomack3414 exactly correct. The same exact thing is happening at countless major US companies and has been for several decades now. As long as the incentive structure continues to reward maximizing immediate profits above all else, executive compensation is not in any way tied to the long term viability of the company, CEO's come onboard knowing that they can take a golden parachute out the door at any time regardless of what they do to the company, and everyone involved continues to escape any sort of culpability legal or otherwise for even gross criminal misconduct, this will continue to get worse and worse.

Such crooked organisation apparently recognises that the CEO has a slim chance of ending in prison, but let's face it ... due to corruption and the amount of money, they are untouchable. If the world was not corrupted, then no newer Boeing products would be allowed to fly due to their complete disregard for safety as long as there is profit in doing so.

I remember the two rudder hard over accidents from years ago that United and U S Air had A HYDRALIC unit that would fit in the palm of you hand had tolerances so tight of the parts in it that it would bind up and there was nothing the surprised pilots could do about it! It just shows thats its the little parts that can cause an airliner to end up as a smoking hole in the ground!!!

It was a little worse than just that. Regulations said that there must be dual systems for the rudder, but Boeing didn't want to pay for 2 valves, so they just put 2 spools 1 inside of the other to simulate dual circuits. When 1 spool jammed, the other went haywire. 2 things to learn from this.. Boeing was always into cutting corners, just not to the same degree as more recently and the FAA let their buddies get away with it.

The thing about the two fatal accidents with the icy cold power control unit trying to function with very hot hydraulic fluid was that the NTSB got lucky when the third Boeing 737 to experience the same thing, but did not crash with no survivors, so they could ask the pilots what happened and inspect the PCU.

Greetings from Colorado Springs. I witnessed the immediate aftermath of the United Airlines 737 crash here in Colorado Springs. I never want to see that again.

That was a terrible day. That flight had departed Stapleton about 20 minutes before the accident. 25 people who had been alive and within walking distance of where we were working just 20 minutes before had lost their lives. Some of the guys I worked with were cut-ups who were always joking, but the mood was as sober as a Supreme Court session when we heard about the crash. Even to this day, my heart is in my throat, just thinking about it. The local news covered the accident, and for weeks we heard every detail available regarding the tragedy. As with most aviation coverage on television, there were some terribly inaccurate things being reported, and the coverage was presented for maximum emotional effect, instead of being confined to truly factual reporting. They interviewed a child who was near the crash site, and he said that he could see passengers pounding on the windows as the plane dove into the ground. That would be, simply, impossible, but then again, I can sympathize with the child; he was processing having seen something horrible and it’s not surprising that he would “remember” something he could not possible have seen. My guess is that the passengers were probably aware of having been thrown into the back of their seats, but little else. I’ve personally flown into COS from the same direction and the terrain below the approach path is rugged in places, end eventually passing over more populated areas. It’s fortunate that the plane didn’t impact any residences. As shockingly horrid as it was, it could have been much worse.

When you operate any vehicle, especially large vehicles like planes and boats, you have to be The Wolf from Pulp Fiction. I need to know everything about the vehicle. I need to know the tire pressure, engine type, operation hours, and maintenance log. I need to know if one of the switches is a little sticky or if a seatbelt is broken let alone the actual flight systems installed on the craft. Boing cares about money above all else... not the families getting on their planes.

The fundamental structural problem here is that the FAA has conflicting objectives - to both regulate and promote aviation. When regulation sits in the same organisation as promotion, regulation suffers at the hands of promotion. That's why in the UK, for example, there was a move to separate out the regulation and promotion functions of several organisations. For example, the solicitors regulation authority is now separate from the law society that promotes lawyers interests. That's what needs to change - the FAA needs to be split into separate functions for that reason.

FAA is supposed to do only regulation. There's no need for any split. But we see other gov officials pressuring the FAA to help speed up SpaceX's R&D.

It's right there in their mission statement - "safest, most efficient" - those two are competing demands, and that's the root of the problem right there. The demands of efficiency often override the demands of safety, especially after a long period of time without a major headline--grabbing crash Efficiency means expediency, and there are numerous examples of where the NTSB'sll recommendations have been ignored by the FAA because it would impede the efficiency printer directive, and which has then resulted in further deaths. For example, the failure to properly oversee the redesign of the cargo door in the DC10.

For a while I was enjoying the s*** show because I work for one of their competitors in the military industry, and I kind of had an ax to grind about one of their past (in our opinion) dirty dealings. But as a frequent flyer, and not wanting to see Boeing's workers (and their other American based suppliers) lose their jobs, even I'm wishing they could get their act together.

Something this story isn't mentioning, that there is a nose wheel steering lock out while the nose gear is in a weight off wheels condition. This prevents the nose wheel steering via steering tiller and or rudder pedals, being actuated while in flight. But it shouldn't affect normal rudder operation, but it would affect normal steering on the ground if the lockout didn't disengage for some reason.

@@ericrupert2687 I agree with you, I was stating there's other things in the system as well. That steering system was omitted from the story. I'm far from scared from flying, I've been an A&P for the past 4 and half decades, and 36 yrs experience on the 737 alone and 3 different generation on the 737 family, so I do know what I'm talking about.

Why yes, one mental pilot managed to deliberately crash his German Wings airliner into some mountains in France. Killing everyone. Another Air Egypt pilot crashed his airplane into the sea and another Malaysian Air pilot made his plane disappear and crashed it into the Indian Ocean.

@@JB-yb4wn Actually cause of Air Egypt 990 crash was probably not suicide-murder, but failing of control surfaces in 767. FBI investigated the case and concluded that it wasn't suicide. It was covered up by NTSB at that time. So Boeing was Boeing in 1999 as well.

@@user-yt198 The NTSB concluded that the crash was caused deliberately by the relief first officer, Gameel Al-Batouti, who was alone in the cockpit at the time of the descent. They cited the co-pilot's repeated statement of "Tawkalt ala Allah" ("I rely on God") on the cockpit voice recorder just before the aircraft entered the dive.

I will never forget the USAir 427 crash. I was flying 737s for USAir at the time. The NTSB, media and FAA rushed to blame the dead pilots. Fortunately, the Airline Pilot’s Union of America was able to replicate the rudder malfunction in a lab. The power control unit was at fault. A new PCU was oddly previously developed for the brand new (at the time) 737NG series.

As a frequent flyer I have given up worrying, as there is nothing to do. I do however spent a minute before roll out contemplating the plane may crash.

The FAA has a dual mandate of regulating safety AND encouraging air travel. So, unlike other federal agencies, it does cost-benefit analysis as to how quickly a repair, fix, or modification should be done. The NTSB has been suggesting for years that the FAA mandate children under two have their own seats, rather than the option of lap children. The FAA argues that if it bans lap children, more families will drive. While driving is more dangerous than flying, should the regulatory agency agency be weighing air travel against other means of travel, in deciding its regulations? Mary Schiavo has said the dual mandate hurts aviation safety, but Congress seems uninterested in abolishing the mandate to encourage air travel.

FAA has one set of rules for Boeing, Northrop Grumman, and Lockheed, and a separate set of rules for the rest of aviation and SpaceX.

The 737 is junk. Anyone remember the rudder hard over under certain conditions? I say avoid this plane. In fact I’m glad I live in Atlanta and most of delta airplanes are airbus.

As you say. The 737 had a series of stuck ‘rudder hardovers’, which crashed two aircraft. The problem here, was those early 737s only having one actuator. All 737s were modified to have two actuators. Again, the sticking actuators were caused by the cold. R

I'm hopeful that Boeing will have a bright future because the new CEO is an Iowa farm boy, Boy Scout, and engineer. I love the values of the Boy Scouts, and the work ethic and "can do" attitude of Iowans that I know. I feel that these videos that shame and humiliate BOEING are helpful to create the necessary pressure to influence decision making.

The hydraulic actuators for the rudder are lifetime components which should never have to be replaced. Not sure, but I think this stems from the piston within a piston for the rudder actuator which caused serious incidents and accidents of the 737 aircraft decades ago. Am I remembering this correctly? I guess Boeing doesn't replace parts which they have previously stated do not have a problem. In other words, they never figured out how to test it to make it jam. Others did.

The "Too big to fail" has to be tested out in full. I'm sorry to say it, but expect another 737, or even one of the other planes touched by the MDD merger management, to fall out of the skies one day due to Boeing's unsafety culture. Sad state of affairs with Boeing.

Configuration control is definately an issue with Boeing's aircraft...(oh we will just leave this part on the A/P since it is installed even though it's for ANOTHER Airlines configuration at no extra cost so we can meet the delivery date. You know, because we have a schedule that is more important than Passenger safety...) Damn the torpedos full speed ahead...Iceberg? What Iceberrr....

It seems the FAA needs a major overhaul and some of their powers need to be transferred to the NTSB, and the treshold for what makes an upgrade major enough to count as an entirely new model must be lowewed significantly

come on --it was a component installed on an airplane for a another customer, and then United got it instead, and it is a component that is used on speciale ILS landings and was deactivated - but it turn out, that if water got into the unit, it could freeze up, and the lock the system, even though it was not activated ! The error was that the firm that made it (and it was not Boeing) had sometimes installed the sealing wrongly, so water could get into the system of the unit. United knew that the plane had the system, but it was deactivated as United does not use that system. No one knew it could lock the system if water got into it. Well not until it froze the system of another jet and that was shortly before this incident! Mentour pilot has a video about this

You need to look at the recent ULA engine explosion 20 seconds after launch that apparently doesn't need investigation according to the FAA. If the debris had gone the other way, in, instead of out, it would have destroyed the rocket and released 2 solid rocket boosters, still with 60 seconds of fuel and no way to steer, destroy or shut them down

Good to hear that Boeing now has a CEO who will rigorously wipe everything under the rug, to cover up his mistakes from earlier life....👍👍👍 if ever possible I book airlines which do not operate Boeing planes even if more expensive.
