Are you sure that during inspiration, the drop in intrathoracic pressure creates pressure on the vena cava? I agree that inspiration leads to increased venous return, but this is because the right atrium pressure decreases and makes it easier for the heart to fill. I dont understand how a drop in intrathoracic pressure leads to an increases pressure on the vena cava. What you are illustrating with the syringe is the pressure on the thoracic wall from the atmosphere, not the pressure on the vena cava.

@@Giraffinator fun fact! Eliot Philipson is actually the first person to come up with the idea of CPAP and initially invented it for dogs with respiratory issues like bulldogs and pugs to help them sleep better in 1970 with impressive positive results. Hey brother, great job. I found out today that I have sleep apnea. Of course, I had the test 2 years ago and apparently someone forgot to notify me. Lol, I'm not kidding. I was originally told they would only contact me IF I had sleep apnea. Chronic fatigue brought me back to the doctor and a review of my test results. Great job explaining CPAP and keeping it entertaining. Thank you.

On a scale from 1-10, 10 being super chronic, how chronic was chronic? And how are you feeling now?
@notsosilentmajority13 жыл бұрын
@@alaefarmestatesllc For me chronic is actually really bad. I can "sleep" for 8 - 9 hours and get up in the morning, have coffee, and start to nod off within 2 hours if I am not physically moving around. My CPAP pick up has been delayed several times so I haven't received it yet. I had an eye surgery unexpectedly rescheduled that postponed the pick up and then they made a mistake in scheduling and I showed up on a date that only the secretary was in the office Then I had to fly out of state unexpectedly for a family matter. Hopefully, I will be getting it this week. I will let you know if/how much it improves my fatigue. I was an extremely active person that required very little sleep throughout my whole life, it is only the past few years that I am always tired.

You got the Venous respiratory pump backwards. Down diaphragm causes intrathoracic pressure drop, expanding superior and inferior vena cave and pulmonary arteries drawing more blood toward heart's atria which passively fills Right Atrium and lesser amount to tight(due to thicker Pulmonary Artery walls) enough normally to give stretch receptor response. Bag valve or mouth to mouth increase thoracic pressure while pushing down diaphragm and also compressing Vena Cavae, and pilmonsry vessels, pushing a little blood into atria but more effectively back flowing Vena Cavae and Pulmonary artery blood, causing greater deficiency in Atrial filling for effective ejection fraction. The aorta is to thick walled for breathing assisted Thoracic pressure oscillations, until chest compression and recoil get involved, where a pulse bolus of blood will push to Coronary arteries, carotid arteries and descending Aorta past diaphragm. ---somethings not taught, but proven in field studies and sensible to engineers are: 1- Vena cave return flow is minimized due to cardiogenic or neurogenic shock autonomic relaxation of veins walls, dilating them up to 200%n stagnating blood outside chest cavity, needed to return flow and fill heart(eso in CPR). Elevating legs and even mild to moderate Abdomino-pelvic pressure creates a compression of dilated veins, returning over 50% or more stagnated extrathoracic blood to central core(thoracic/heart). This is the princiole of Mass Trausers in ATLS training and G-Suits in pilots/astronauts). Just don't do severe pressure to upper abdominal to force stomach contents to asoiration risk, but legs contain 500-1000 ml blood reserve in relaxed veins and abdomino-pelvis another 2-3000 ml. This greatly counters the thoracic return flow to heart dilemma and the gravity and peripheral pressure overcomes part of the Vena cavae/Pulmonary vessel compressed effect, from passive recoil or non- rescue pod use breathing assistance. Trendelenberg positions, even partial helps, along with audio gravity assisted cerebral flow. One must not elevate head in COR or you create a gravity resistsnce to pulsed compression flow, unless there's a closed head trauma, where you don't want venous congestion, amplified by gravity, to cause more cerebral edema or cerebral venous resistance, increasing cerebral atery pressure in bleeds. 2- Just mild lower ti mid flack abdominal pressure while directing upward toward chest(cephalad peritoneal visceral.mass wave) can push diaphragm up increasing thoracic pressure and causing Pulmonary partial expiration. When releasing, abdominal.contents slosh back by gravity and elastic visceral tissue cinnennections, causing a fluidic surface tension adherence to diaphragm to pull it down(causal rebound mass wave draw) causing negative intrathoracic pressure that not only draws in air through brinch trachea tree, expanding alveoli naturally, but also draw-dilates thoracic Vdna cave and Pulmonary vessels as well as atrial walls, giving natural diaphragmatic recoil cardiac filling ti some degree. Thus is tge Iron Lung effect without the added Thoracic Wall Rib Cage expansion. However, engineers applied tge old drowning resuscitation techniques of elevating the rib cage/expansion and lowering diaphragm for inducing negative thoracic pressure by lying th patient supine, grasping both forearms and pulling arms straight above head while slightly lifting torso off groundN then relaxing to settle the body ptone with relief of all tension on arms, even taking them down to front of chest crossed and pressing to anterior chest. Then repeat this in cycles. The mechanical effect is: the pulled hyper extended arms passively pull the the shoulder girdle muscle taught, with connections to pectoral and serratus muscle chest wall connections and actuate rib liftT,chest expansion. At the same time, the dkight pull to cause torso lift/angulation, allows abdominal content shift caudal, drawing down diaphragm as well as lung weight gravity pressure, expanding lungs through induced negative intratoracic pressure mimicking conscious breath or Iron Lung effect through tethering effect vs outer atmospheric pressure oscillation. The elevated arms also return half a pint of blood to Superior Vena cava under elevated hydrostatic pressure. The release of arm tension and supinating the torso reverses all, giving chest passive and gravity contraction (collapse) expiration with an added emphasis to clear deeper alveoli, which improves turbulent convective O2/CO2 exchange, ready for deep refill next arm pull/lift. Field comparisons of the Drowned man method without assisted breathing or compressions had a statistically equal near 4% survival compared to Standard AHA CPR. ==== A wise rescuer can learn and apply any of the above engineer-anatomical blood flow effects, gravity and compression return flow, diaphragm sloshing and even arm pull. --- Note, the opposite actions are desired in fulminant CHF, where you want to blood to stagnate in limbs and abdomino-pelvis to unburden failing excessively dilated heartthrob inclining chest and even partially tourniqueting limbs to restrict return volume overload.(opposite of Mass Trousers/G-Suit) Human bodies are Engineered machines...analyze them in. Same principles.

I am a little confused? I was always taught that during normal Negative pressure inspiration as the thorax expands and comes under negative pressure it actually "SUCKS" blood up into the Vena Cava from the abdomen increasing Pre-load and it is when we exhale creating positive pressure in the thorax it is then that the preload is pushed into the atria? When we intubate or use a BVM and initiate positive pressure ventilation you loose the "SUCK" and reduces the preload consequently lowering the blood pressure? Furthermore are there any objective studies not funded by the manufacturer of the ITD that prove its efficacy?
@jakobstensasen9776 Жыл бұрын
I think the physiology in the video is wrong. He states that negative intrathoracic pressure increases the pressure on the vena cava and "drives" blood to the heart. What happens is that during inspiration the pressure in the right atrium drops and makes it easier for the heart to fill. A negative pressure in the thorax doesnt put pressure on the vena cava. What he illustrates with the syringe is the pressure from the atmosphere on the thoracic wall.

@@raymondelsayed4445 so u lost an additional 15 pounds? Good job. Yeah that cigarettes gotta go man. I’m one to talk. That’s the last habit I have and it’s the hardest to get rid of. Quit cannabis long ago. Don’t regret it.

My problem is breathing in. I'm overweight and due to my airway collapsing on the inhale I have an APAP. That said I did notice prior to getting my machine I was waking up with my heart beat in my neck like it was gonna explode and a migrain so fierce I was afraid I needed to go to the hospital. Because of modern technology and medicine I'm sleeping like a baby these days.

Who came here after seeing William Shatner doing a cpap commercial ... I have a severe sliding hiatus hernia, Gerd, COPD , 6 months ago had my duodenum part of my stomach removed and who knows what else ( besides that I'm doing great} I'm trying to figure out if this CPAP is for me ?? This video did not answer this question. OK I'll ask it differently, what is the CPAP mostly used to treat what ailment ??

Excuse me, but if the diaphragm drops down, the thoracic cavity expands, descending the pressure in the heart. So instead of the vena cava being sequeezed it happens the opposite, and what helps blood return to the heart is the pressure difference between the vena cava and the rest of the circulatory system.

The only cure to sleep apnea is double jaw advancement surgery with CCW movement. Waiting to expand my jaw with MSE and MSDO before I get the surgery and finally cure this disease. I can't sleep without CPAP anymore, it's night and day difference. I recommend you use a nasal tube only and once u get ur ideal pressure tape your mouth shut.
