I think it's less surprising for the Pistols because they're technically a Colony Stand, and those tend to be more sentient than the regular punch ghosts like Star Platinum, The Hand, Silver Chariot and Gold Experience (kinda like how Keicho's Bad Company could shout army speak to each other or Cannolo Murolo's All Along Watchtower like to bicker with each other and form pyramids). Obviously Echoes is an exception, but that was pretty much the only one before Spice Girl.

As someone studying psychology, I really like the boss/Doppio. Sure, schizophrenia and DID aren't represented completely accurately, but I'm not the type to care too much about that (besides, this boy was born in an all woman prison after either a two year pregnancy or via immaculate conception). Basically, form observation, Doppio has DID, with the main personality, Doppio, being unaware of this (I say he is the main personality because of this fact), and the boss being aware of this, and being more dominant. He goes through some pretty extreme physical changes when he switches personalities, which is like an exaggeration of how people with DID can and will change their entire posture and bearing so drastically it can look as though they are different people. He also displays signs of schizophrenia, and I like how they do this because you see it in both personalities in different ways. Some psychologists believe we should break schizophrenia up into subtypes. One of which has hallucinations being the prevalent symptom, which we see obviously in Doppio. In the boss we see a more delusion centric disorder. He is paranoid, believes everyone is out to get him (even before they actually are) and goes to great lengths to escape this perceived persecution. He also makes statements implying he believes he is immortal, or will become (literally) immortal once Trish dies. Also, there are two explanations I've seen for Trish's stand talking to her. One is that that is just a trait of stands of people who aren't very confident (Koichi in part 4 has a similar moment and his stand spoke to him, but spoke minimally afterwards like Trish's). Another is that she may have gotten some bad genes from her dad which may be bleeding into her stand.
