I've always felt there is a time-line without Claire in his life and Jamie died at Culloden. In this time-line he helps create the circumstances for her to travel through the stones and her presence in his life saves him. Also, Jamie has some kind of psychic ability and perhaps astral projects in his sleep as a "ghost" and has seen the 20th century several times in his dreams.
