Patellar Fractures - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes Patellar Fractures, its types, and fixation.
Movement of the patella across the knee is normally a gliding, smoot movement.
A patellar fracture is an injury to the kneecap. These fractures are commonly caused by a direct blow to the kneecap or from a fall.
Patellar fracture types
1-Transverse fracture: fracture can be non-displaced or displaced.
nondisplaced fracture or minimally displaced fracture: the patella can be pulled apart by the attached thigh muscles. The patient is able to extend the knee with full extension.
Displaced fracture: the patient is unable to extend the knee.
2-Upper or lower pole fracture: fracture at the site of attachment of the patellar tendon.
3-Comminuted fracture: multiple pieces. Fracture can be non-displaced or displaced. This fracture type shatters into three or more pieces and is very unstable.
4-Vertical fracture: fracture is nondisplaced and stable. The fracture fragments usually stay in place during healing.
5-Osteochondral fracture of the patella usually associated with acute dislocation of the patella. Osteochondral fractures are located in the facet area of the patella.
Treatment
Transverse patellar fractures
Nondisplaced or minimally displaced transverse fracture: knee immobilizer or brace.
Displaced fracture:
•reduction of fracture with reduction clamp
•k-wires are placed perpendicular to the fracture.
•Figure 8 tension band wire is applied for compression of the fracture.
•The wires convert anterior distractive forces to compressive forces at the articular surface.
•Cannulated lag-screw with tension band: wire passes through the screws and across the patella in figure 8- tension band.
Partial patellectomy
•Usually involving the distal pole, smaller fragments are escised.
•The patellar tendon is reattached anteriorly with sutures.
Total patellectomy
•Indicated for comminuted and displaced fractures that cannot be reconstructed.
•Bone fragments are excised before reattachment of the patellar tendon.

Thankyou for the explanation. I didn't fully understand what the doctor was saying last week. Your illustrations clear it up in my mind. I fell at work and landed full weight directly on my knee. I have a displaced lower pole fracture, unable to extend my knee. The doctor says I need a partial patellectomy to repair the knee, plus several weeks of physical therapy so I can walk again. Until the surgery happens I am unable to walk at all.

15+ years ago i fell on my patella and it fragmented, i could move that piece around without any pain and had no ROM limitation. over the years it disappeared somehow =) ANYWAY, not here to tell a story but study for traumatology exam ;) thank you for the videos

Hello, nice video! I have non displaced transverse fracture on the lower part of my patella. Made CT Scans, consulted with 2 doctors and they told me to avoid movements .. I have knee brace ... My question is, may I step on my leg? what exactly should I avoid? I feel no pain at all and I feel perfectly fine ...

I just fractured my patella on Monday and went to the hospital due to extreme pain. I have a a big chip/fracture in my lower patella. I'm in a brace right now and for the next 2 and a half weeks. I hope I feel better soon. I feel like a pirate with a peg leg

Im 18 years old and just had a vertical patellar fracture and was put in a knee brace with crutches..i know it should take around 4-6 weeks to heal. Will i be able to climb any ladders this summer or be able to water ski? I know it depends on recovery process but just wondering what your thoughts were. thanks!

I had transverse fracture which was surgically repaired with 2 cannula red screws and a tension band wire. Hardware removal December 2012. I have EXTREME pain & patellofemoral arthritis. What are your recommendations for treatment at this point? I'm 37 and very active. This has been a debilitating injury. Thank you.

How much time was there between the first surgery and the hardware removal? Have you built back the quad muscle you lost from the surgery? I have also had the same surgery and recovery is very long!

I was longboarding down a hill and was going about 15 mph. I started wobbling then I bailed and fell straight onto my knee and rolled. I don't know if it is fractured becuase I was in extreme pain and there are multiple bruises on and around my knee. It's been 9 hours since the incident and it doesnt hurt as much as it did but it still hurts a little. Can someone tell me if it is fractured?

Hello, Greetings I am an athlete run long distances.I have very bad pain exactly below my left knee cap, this happened after fall from bike...MRI scan was normal but still i have a very bad pain, when i run for long distances. Doctor said knee cap is tilted slightly looking at X ray. But i am able to bend my knee and this issue is been for an year now. Please suggest. Thank You

I broke my patella 4weeks ago. It is in 5 pieces so my doctor recommended letting it heal without surgery saying after removing the small pieces there wouldn't be much left. Is this common? Also I can only walk backwards with a walker. Can people actually lift leg and walk forward? Have been wearing a brace to keep leg straight. Thank you in advance. Would love feedback.

Have been told many times to amputate, although two doctors from state Fed. And three local I respect as well will not dispelled. I have to go to to the physician whom insist on a mutation tomorrow. I'm scared .even local podiatrist whom also won't name doesn't understand my right leg built car an power plants.he treats me as if I don't hear .that's not right, still after 2 years I can't walk I am ashamed but it took him half as long to admit my ankle was broken whole time and never gave me rehab had insurance most my life but not at prepping Have government paid over 2000000 still can walk still owe won't be making car or power plants again never been good at begging got to be there tomorrow or they will make it work against me maybe I'm scared but going .please help !
