Awesome 👏 coverage again. I’m 77 and totally 💯 am in the cycle but I have improved a great deal since I was 70. I use a word “ WAISTING” when I point out that it’s not happening to me. You are so tuned in.

Fabulous comprehensive summary of sarcopenia. I was actually surprised when my friend asked me to do that sit to stand (from cross-leg position without hands) and for me it was easy but for him it was quite hard and I found that surprising because he can do many pullups and pushups with ease and used to be a sprinter/runner . Maybe just needs to practise resistance exercises for lower trunk muscle groups and core/plank exercises. His grip strength is far better than mine though....must be from all the pullups. I personally think pushups are a lot easier than pullups.

Muscles are actually consume not a lot of energy per mass unit. IO like liver, brain or kidneys are way more metabolically active. It's just we have more muscle mass than liver mass.

Implications for someone with muscles that cannot convert glycogen to glucose and as a result the muscles become marbled with fat deposits? Missing (complete absence) of the PGYM enzyme. Anaerobic activity is virtually impossible without elevating creatinine kinase levels > 3,000 UI and (often) myoglobinuria. Was told for 60 years that I just was lazy or not trying hard enough, no pain/no gain, and more.. Getting weaker and weaker.

I'll take better care of my mitochondria . I also se a talk about mitochondria and cancer . The damn things are important .Do you have any opinion on rhabdomyolysis and occlusion training ? How dangerous is this and how prevalent ? How to avoid or should BFR training be avoided in your opinion? What is your take if any?

This damage protein that yiu are talking about the synthesis of protein and the breakdown of the protein can be enhanced with Fasting right 😊 I know I am not a expert....just I guess a question to you

You don't have to be an expert to ask questions, Juliat - otherwise, this whole channel would be pretty pointless. :) Can you point me to a timestamp on the video you're referring? My understanding of your question is that you're asking if fasting increases damaged proteins from protein synthesis, but I feel I'm misinterpreting. Correct me.

@@PhysionicI think I know what he was trying to ask. We have heard that older people need more protein due to the loss of the ability to perform protein synthesis as well. But with insulin resistance, we know that fasting can reduce insulin resistance and increase insulin sensitivity to a more youthful level. I think he wanted to know how f fasting would also increase protein synthesis again by fasting from heavy amounts of protein and thus bring your ability to perform protein synthesis again as if you were younger

Does this apply to everyone as they age or only to those who have not and do not do any exercise?

Definitely not everyone, no. The review mentions something like 13% of people in their 70s, and as much as 50% in their 80s.

So would the reduction in protein synthesis and protein degradation send more protein to the kidneys possibly leading to protein overflow and advancement of kidney disease? I am a 60 y.o. Surprised by my kidney function decrease working manual labor, who still consumes a fair to large amount of protein.

A lot of muscle damage and inflammation can happen due to heavy resistance training, why doesn't the muscle becomes fibrotic under these circumstances?

I really love your questions, Mario - you always bring up top notch ones, I've noticed. So, you're exactly right that muscle damage does occur with lifting and inflammation does occur with muscle damage, but the distinction here is the time elapsed. In sarcopenia, inflammation is A) Chronic, B) may involved a different population of immune cells, C) may involve different cytokines, and D) may recruit less activated inflammatory cells (immune cells have stages of activation, a bit like a dimmer switch). With muscle damage, the immune reaction is transient, targeted, and local - so, while in the big picture it shares commonalities, the details make all the difference. Brilliant ask!

@@Physionic thanks for the compliment Nic, your videos really make me rethink some things. So there are important differences between acute stress (so called hormetic) and chronic stress, right. That's one reason why I only take Sulphoraphane once a week, or less, instead of daily like Rhonda Patrick suggests. The other reason is I hate the smell of broccoli seeds and the resulting stomach ache. 😅

Would this be applicable for malnourish or disease related muscle loss before the age of 50?

No - definitely not nutrition related, and likely not disease related. Those would be separate issues.

At what point can the process be reversed? Would age be a factor or the extent of muscle loss? How could this most effectively be reversed in terms of macro ratios and timing?

According to my doctor, there is no (reasonable) limit to muscle growth as we age. Per experience, it seems to take more work/exercise to grow muscle compared to when I was younger. And skipping just one week causes a serious reversal.
