Hi Melike, no worries! The tests that you can do are Biceps load II, O'Brien's and Speeds, so basically every test that stresses the long biceps tendon. You should realize however, that the shoulder is usually instable (anteriorly) when you have a labrum lesion and the other way round: Labrum lesions often have shoulder instability in their pre-history.

the positive of the test can indicate the anterior instability or impingement?? an anterior instability causes the subacrominal impingement?? and how the posterosuuperior internal impingement happened??

A positive test is indicative of anterior shoulder instability. Cook et al. (2008) described the test to differentiate between internal (posterior pain during the test) and external impingement (anterior pain during the test). Anterior instability can be a contributing factor for both kinds of impingement (internal and external).

We'd suggest to see a local physio for a good rehab program. In general closed-chain exercises work well for the instable shoulder.

Are you only referring to the classical Bankart lesion anteriorly? Then the the Apprehension and Relocation test are very good tests and there is no need for other tests. It doesn't usually affect ROM too much - the problem is rather instability in late cocking phase of throwing etc.

90° abduction in apprehension test. 110° in posterior impingement test. Plus more horizontal abduction in the posterior impingement test.

Great videos. What is the normal range of motion for external rotation of the shoulder in this test? If one is unable to achieve this rom, what problem might this indicate? Thanks!

around 80-90°. But the outcome measure for this test is not Range of Motion but "apprehension of dislocation" hence the name "apprehension Test". The patient will be fearful to reach endranges of external rotation as the shoulder might subluxate, indicating possible anterior shoulder instability

Avoid the exercises that put the shoulder into these "extreme" positions of external rotation. Strengthening the rotator cuff in order to ensure good support of the glenohumeral head

Not that black and white, but the patient with anterior instability will most likely not want to perform the exercise. It's not that the exercise itself is contraindicated but the patient will want to avoid it

I have a question, here is an article using a test battery/algorithm for Bankart lesion testing and among others, the crank test is mentioned and the apprehension test, is there another crank test than the one in your video and what are the differences between these tests? Hey! thanks for the comment. Indeed they reference a paper (citation 33) that describes both the apprehension test in 90° abduction and external rotation as well as a separate crank test. "Clicking was further assessed in a subset of 22 of the study patients with the "crank" test by internal and external rotation performed in a position of maximum forward flexion with axial load along the humerus" Will check it out further and make a video about it! Thanks for the heads-up! There may be some inconsistencies in the literature regarding the instability tests!

More in general where some refer to the test shown in the video as both the apprehension and crank test whereas others like the one you mention lists them as two separate tests
