100% at least it appears that he would have been unconscious at the crash and carbon monoxide poisoning is not a terrifying sensation - you just feel like you’re a little bit puffed and short of breath then you feel dizzy and sleepy and you drift off to sleep, so at least the family can have that to know that their precious son probably wasn’t terrified in his last moments ,doesn’t bring him back but possibly give them a little bit of peace😢

@@rc70ys aah! Hopefully I can qualify that and explain the difference, so here goes !anxiety and thinking the worst is an internal thing. Not based on any evidence. Gut feeling ,in my opinion, is actually the voice of your subconscious, the subconscious, just being a part of your brain that below the surface observes, everything collect the evidence but doesn’t make any value judgements, ( just like a computer analysing data)so to decide which you are acting on take a moment to academically consider, and if it is just anxiety, it’ll just be either an undetermined fear or some random inexplicable aspect . if you quietly think and realise a logical aspect then, as you let your mind relax, the subconscious thoughts will come to your conscious mind, and you will then say ,okay ,it’s because the pilot joked about his skills,slow. getting with a clearance. I noted that there was an oil leak on the front Strut, the flight and was hairy, and then he got confused with the approach. They will all be things that at the time just passed unnoticed to your conscious mind, and then your brain will collect them all and present them as a “gut feeling” for want of a better word not sure if that seems sensible, but that’s how it feels like it worked to me when I’ve considered it.

I once had a gut feeling. I ferried a flight to Nottingham for maintenance. Kept seeing fuel tankers passing me. Saw two small kids about 2 years old playing in the cafe park and they were adorable. They both hugged and kissed and then looked at me and pointed to the big tanker, run over to it with their parents. I said im going to check my fuel for the flight back (left on full tanks, always gets me there and back) i checked the tanks when the aircraft had finished its annual. I had 2 gallons of fuel. I would have taken off, and had an engine failure. I still to this day always think those two kids were angels sending me a warning. Never ever skip on checking fuel and oil, EVER.

Thank you for covering this crash. Here in the UK the case highlighted the casual approach to safety & regulation by the "Private" flying community which was truly shocking. Not mentioned in your summary was the fact Ibbotson had been found to be breaching the regulations in an earlier flight with this aircraft & the owner had made it clear they didn't want him using the aircraft again. This fact & the limitations on Ibbotson's Private Pilot License were said to be known to Henderson who booked him to do the flight not with standing. Henderson was prosecuted and found guilty of arranging a flight without permission or authorisation and of endangering the safety of an aircraft.

It seems likely that Ibottson was offered a sum of money much greater than he would normally make for a flight. He was filling in for the usual pilot (Henderson) who regularly flew players around Europe and so probably earned quite a relatively large pay for those flights. So when Henderson quietly gave the flight to Ibbotson along with the money to be made, all sense went out the window. When I see such repeated stupidly dangerous decisions it's usually money at the root cause of it.

It's very interesting to read the news articles coming out of the UK around the time of David Henderson's trial (the operator of the plane). The woman who owned the plane (within a trust set-up) had specifically told Henderson to not allow Ibbotson to fly her plane again after previous problems. Henderson Knew about Ibbotson's lack of a commercial license, the fact that he couldn't fly at night and more. Henderson ran a shady operation (and it sounds like Sala's agent was pretty shady too) and did so because of business (profit) goals (of course). Henderson even texted associates as soon as he heard of the crash and told them there would be a big investigation because of passenger's VIP status and not to talk. Also likely that Sala was unconscious at time of crash due to leaking carbon monoxide from tailpipe into cabin and that Ibbotson, though still flying, could have been adversely affected too. This whole thing was a sh*tshow from beginning to end but it looks to me that the buck stops at David Henderson.

This was massive news in the UK at the time. The 2nd search wasn't paid for by the family, it was funded by a Go Fund Me campaign, and the AAIB (Brit NTSB) helped by providing their submersible. The reason why the 1st search was called off after 3 days was because it was conducted by the authorities on Guernsey. Guernsey is a tiny Island and as such I'd guess they don't have a lot of resources. It's a sad tale all round really, a story of pressure on both men, who probably should have said no at some point.

David Henderson was taken to court for arranging this flight. The return trip to Wales was supposed to be earlier than planned hence the VFR flight plan. The pilot and Sala were exposed to carbon monoxide rendering them incapacitated before the impact.

In the voicemail call the soccer player made during the flight where he talked about his concern for the condition of the plane, he also said he was feeling very tired. I heard this in a more complete version of that recorded call in another video elsewhere a couple years ago. This could have been from the CO in the cabin already beginning to reach toxic levels and making him feel drowsy.

When in Nicaragua about 10 years ago with some friends, they were invited to go sailing with someone they met at the beach. This wasn’t a professional charter. My friends made plans with the fella and they asked if I wanted to come along. I felt uneasy and uncomfortable with then thought of going. Well, as soon as they got out of the bay and into the open sea, they capsized. Luckily there was a fishing boat not too far away and was able to save them.

yeah, if you're trying to be a jet setting high roller it seems like you need to go ahead and commit and pay a premium and make sure there is a thorough background check on the pilot, the plane etc, otherwise drive or fly commercial. id want a pilot to have a minimum of 10k hours on the specific plane we are flying. the dude should know everything about the plane and be very anal about safety checks, maintenance etc.

For an additional reckless pilot fatal accident (if you haven’t done it already) I would suggest you look into the airplane crash that killed Black Sabbath and former Quiet Riot guitarist Randy Rhoads back in 1982 in Leesburg, FL…a flight that never should have taken place by an unqualified and impaired pilot.

That is one thing you can count on with Hoover's analysis. Pertinent facts. I too used to watch other incident doc's but not anymore. Amazing to me how some pilots (the cowboy pilots) can throw caution and safety to the wind and that's when an incident occurs. I liked one commenter that was a very experienced (over 20000 hours) state that he was very paranoid about flying, and did not take any chances.........Probably the very reason why he never had ANY major incidents. The difference between having a long flying career as opposed to OMG,OMG,OMG........

The biggest problem with the super stars and the pilots whether is a car or a plane is that some of the pilots feel the need to show off and impress the super stars many times taking extreme risks and eventually ending up crashing. Then again it was also the player that could have traveled in a much more secure way. Thank you so much for shedding light in this case. Maybe I’m just too poor to understand how this is a better option than an upgraded commercial ticket. I’m sure it’s more convenient being able to avoid the masses but boy is it sketchy. Add to the fact that these small prop jobs are not comfortable to fly in at all . . .

I remember this tragedy and the issues it raised with private licences, sadly nothing appears to have stuck, as I haven't heard anything regarding updating the rules around the private travel sector. Well, the rules already forbid private pilots to conduct commercial flights, so it wasn't an issue of the rules, but an issue of enforcing adherence to these rules. The get-you-home limited IFR thing though should IMO go down the drain. We train no vis / lo vis escaping during PPL or LAPL training (i.e. flying a 180° escape turn away from adverse weather) using foggles or a hood, but that just increases survivability to a couple of minutes at the best. Without regular training, it's impossible to keep those skills current and ready. And to me their last 180° turn looks a bit as if they had tried to return to Guernsey to avoid weather ahead. But if they had already lost visual orientation (i.e. no horizon), it's terribly difficult for a non-current pilot to execute such a turn without losing altitude or getting disorientated. I fully agree with Hoover, this flight should never have taken off; there was no way to conduct it safely. NVFR over water is a terrible idea either, though technically legal.

Night VFR over the ocean is pretty much IMC conditions. Even though the pilot might have been clear of clouds, flying over large bodies of water it is difficult to see the horizon at night because of the lack of ground lighting. I use to fly cargo in the Caribbean and after take off at night you were on instruments despite being clear of clouds. Flying in complete blackness is a strange experience and has killed a lot of inexperienced pilots.

Night time over water is the one time I got real spacial disorientation. That is such a strong and strange sensation. Thankfully I realized what was going on quickly and fell back on training, but wow, I can see how those events happen

Even for airline pilots it's not easy to fly at night over the ocean. I remember the AF 447 flight, were pilots lost orientation on flight and crashed.

@@jasonMB999 your looking at everything typically at night in VFR conditions just like during the day, but typically at night over the ocean or large bodies of water there is no ground lighting to determine the horizon. Stars at night can help, but if there is an overcast layer above you, and no ground lighting over the ocean, your IMC despite being clear of clouds. Lights on the ground can also give false horizon indications, so you’re always backing things up with instruments at night. You do not need an instrument rating at night to fly, but there are night conditions where your IMC despite being clear of clouds. The probable cause of the accident.

It sounds like you've never heard of the false horizon, or autokinetic illusion, coriolis etc. There is a reason for using instruments as main source of reference at night. You're NOT looking at " everything typically at night in vfr conditions just like during the day". Are you actually a pilot?

So called gut feeling tells you two things. One is if you are distracted or physically diminished then you feel not up to something. The other is when things around you don't make sense like going flying in crappy weather. Or someone acting odd. Your Spider Sense is activated. DON'T ignore it. I am a Private Pilot. Never regretted staying on the ground when things looked or felt not right.

Hi Hoover. Thanks for this. I’m a UK based pilot. I have both myFAA Instrument 5e UK Restricted Rating. Just for reference, the UK Restricted Rating is also not valid outside the UK. So that’s another breach he made if he used it to fly IfR. A very sad state of affairs indeed and the question is, how did the pilot even get to 3,500 or so hours with this level of recklessness?

That’s one of the most shocking reports you’ve documented. I can’t understand the pilot’s hubris in this situation as he clearly knew he was unqualified for night flying and passenger work. The fact that the plane was also in poor condition gave little room for error.

I am a UK private pilot, night rated, partial instrument - the actions of this pilot are totally unforgivable - single engine at night over water should have been enough for him to make a no go decision - his routing, instead of flying north east and crossing to England at the shortest point (Cap Gris Nez), exposed him to avoidable danger. He should never have been in the pilot seat. This was recklessness bordering on criminal negligence.

Man this just straight up depresses me, the pilot was such a reckless cowboy that he just didn’t care, ignored every single warning and safety issue, and killed a young man who was in the prime of his life and terrified, it’s just so cruel knowing this level of recklessness not only happens but the pilot made jokes out of.

It seems like most crashes are the result of a cascade of things, not just one. That heater muff is how old Volkswagen bugs are heated. I never really trusted them! Ahead exchanger is a lot safer.

This was such a sad outcome when this not qualified pilot ended the life of such a talented young man. Perhaps the FA and/or FIFA should maintain it's own list of qualified pilots for moving around all their talented (and valuable) football stars. RIP Emiliano and condolences to his family back in Argentina! 😥

That is the worst possible suggestion. What on earth makes you think that *any* organisation whose business is football should *ever* be involved in deciding who is or isn't competent to fly anybody anywhere ? Every country in Europe has a system for licensing and regulating every type of aviation organisation, and in the vast majority of cases lists of such organisations are available online via the relevant regulatory authority. The problem is very simple - a man who had *no* authority to act as a football agent (licence cancelled due to being bankrupt) chose to operate outside both the law of the land and CAA regulations by hiring a man who had *no* licence to operate an air taxi business, who in turn sub-contracted the job to man who was neither qualified nor licensed to operate *any* flight for payment let alone a night flight in poor weather conditions. This problem is solved by the agent behaving responsibly and professionally by employing *only* organisations that are properly licensed and therefore trustworthy.

I can think of many such accidents now and virtually all of them involved amazingly reckless behavior and organizational failures at all levels. Money alone is not enough to bring competence, it requires vigilance too.

You don't have to be /that/ rich to fly this kind of aircraft with this kind of pilot. Probably cost less than £500 for this flight. Nantes to Cardiff is ~400km, the PA-46 from the trace in the video apparently was doing around 173 knots IAS, so should be around a 1.5h flight. This flight almost certainly saved him at least 5h of travelling time. Thing is, if you're going to spend money on private flights, spend it on something that comes with 2 pilots, where at least the captain is fully ATPL certified (and obv. with current type rating on the aircraft). Anything below that level, you're into cowboy land. Any CPL pilot who takes passengers (in Europe) with money changing hands is at a minimum dancing at the edge of legality ("cost sharing") - and that's already the first warning sign! It might cost £2k to £5k instead of £250 to £500, but it's your life.

This reminds me of Graham Hill and his Formula 1 team. Graham Hill chose to fly his plane without insurance, and without instrument rating, in the fog at night from France to England . They crashed on a golf course near London, and all died. Graham Hill was probably the only driver to win the F1 World Championship, Le Mans 24 Hours, and the Daytona 500.

Hill was racing in an era when motor racing was inherently dangerous and careers frequently ended in a lethal accident. It encouraged a fatalistic "press-on" attitude characteristic of wartime fighter pilots, so it was perhaps inevitable that he would end like this.

The Malibu is a high-dollar aircraft and perfectly adequate for the short flight from France to England if in good mechanical condition. It wasn't the fact that it was a Malibu....it was that it wasn't being maintained properly and wasn't being flown competently that ended in their deaths. I worked across the runway from Piper back in the early '90's and knew people who worked there....and the Malibu did have a fairly high accident rate due to it being rated as 'all weather' which it seemed the people buying them thought meant that they could fly straight through thunderstorms...which is just stupid. Turbocharged pressurized aircraft need careful maintenance or they'll kill you just like any other aircraft and the CO poisoning no doubt was from the exhaust system fault that was being ignored. Whomever was supposed to be maintaining that aircraft should have been investigated and charged.

@@JustMe-fo4ev I think the OP was referring to the fact that they could afford to charter flight, and there are multiple charter companies that are more reputable than this one. It’s a tragedy all around, particularly given the fact that there are so many other options of available. As often as the case, the flying public does not have the full skill necessary to tell whether somebody’s confident, that’s why we partly trust on the authorities or regulate things. Yes, he has his gut instinct, but if you’re being reassured by all parties, things are OK. Many people simply go along.

Your coverage of this ghastly story is thorough and accurate. Here are a few more details. The crash aircraft (N264DB) was managed on behalf of its’ owner (Fay Keely) by David Henderson, an aircraft broker. The plane was registered in the USA and using it for any form of commercial flying into, out of or within the UK was a breach of both UK law and CAA regulations. Furthermore, those regulations state that aircraft hire for *any* purpose may be offered and executed only by duly licensed companies and individuals, and that all parties in the hire are jointly and severally liable for their actions. Neither Henderson nor his company held such a licence; according to regulation, both Henderson and McKay (the source hirer, see below) are legally liable for this breach. Henderson employed David Ibbotson as a pilot in direct and deliberate contravention of the owner’s instructions; those instructions were issued after Ibbotson was twice caught flying in breach of CAA regulations. Henderson was recruited by Willie McKay, who was acting as an intermediary (agent) in Emiliano Sala’s transfer. McKay was in breach of FA (Football Association) regulations because he was not a licensed agent. Cardiff was happy to pay for Sala to travel on commercial flights, but that would have prevented him from returning to Cardiff in time to prepare for the club’s next match, so McKay offered to arrange private flights - this explains Ibbotson’s strong desire to get back to the UK as soon as possible. McKay was breaking UK aviation law by hiring Henderson - quite why he has not been prosecuted for his actions is something of a mystery. Very soon after the crash, McKay disappeared. In June 2021, he was acquitted on two charges of fraudulent disposal of property (he was bankrupt at the time), and in February 2024, Cardiff sued him for failing to honour a legal agreement under which he was supposed to provide Cardiff with all the documentation he holds regarding Mr Sala’s transfer. For his part in this ghastly story, David Henderson was sentenced in November 2021 to 18 months in jail. Two employees of the company which operates the CCTV service in Royal Bournemouth Hospital were sentenced to 14 months and 5 months in jail for watching Sala’s autopsy, both live and from a recording. Ugh. There was an ugly row between Cardiff and Nantes football clubs about whether Sala was a Cardiff or Nantes player when he died and therefore whether or not Cardiff was liable to pay for his transfer. A court ruled in favour of Nantes. In the UK, there is unfortunately a well-established “grey” market, where unlicensed brokers employ pilots on a casual basis to provide flight taxi services at rates substantially below those quoted by properly qualified, properly licensed and properly regulated brokers and pilots.

Dave Grohl, of the Foo Fighters, has been posting pics on his private jet. I have been leaving comments that he needs a pilot who doesn't know who the Foo Fighters are. Probably thinks they are a bunch of hippies and is retired military or airliner pilot. One that maybe lectured them that if there is dangerous weather they ain't flying. Rock stars and athletes don't have a good history with planes. So you better have the best pilots who are willing to say no to you no matter how important the trip may seem to be. As far as this clown show this is why to use your Commercial in the U.S. you can't go over 50 miles or at night without a Instrument Rating.

Reminds me of this experience. I once went to the bank at 11pm to withdraw cash to go meet some friends and as I waited for the machine to dispense the cash, a man stood by the outside door, he wasn't even trying to get in and he turned his face away. I put the cash in my pocket and before I pushed the door open, I saw a car revving the gas ready to take off while the man stood outside the door hiding his face, I pulled my hand away from the door handle and headed back to the machine where I picked up a service phone and as I did that, they took off. Point of my story, LISTEN to your gut feeling. God bless.

An interesting review but it seems to point a finger at the authorities in the Islands for not doing more to assist the two people aboard this aircraft. I live local to the scene of the crash-site. The sea around the Islands is some of the most dangerous on the planet when extreme tides and very deep water. The area just off Alderney and close to the crash site is known locally as "The race!" Small vessels have been know to have been sucked under by the sheer force of the water running through there It is a very dangerous place to go into the water, even in a proper life raft. The Air-search service here in the Channel Islands, is a privately funded service (Mostly through "Nickels and dimes" being put in to charity boxes stationed around the islands on the island's businesses etc. ) There are limits to how long you can stay aloft with no Government funding. Likewise, the sea search service here is privately funded and again there are limits to the amount of time you can look for someone if no-one else is paying for the fuel for the search vessel. ( Again the funding is all, nickels and dimes donated from the public at charity events throughout the year.) Those of us who have lived in the islands like me, for nearly 60 years, know from bitter experience, that your survival time in the water where the plane went down would be a few minutes, not even hours. The plane in this incident was not recovered from the sea bed due to the extreme conditions in the area. Shortly after Mr Sala was recovered, it was reported in the local media that the plane had been moved again by the action of the currents. It is reported locally that the plane has been completely destroyed and the debris removed by the movement of the water. On the night in question, it was bad weather, very cold and pitch black! I remember it well. Since the end of the second world war, multiple incidents like this have happened here and the only survivors have been those who ditched in good weather, during the daytime and who were able to give their EXACT position prior to ditching. MOST are rarely found.

Thank you for your comment. I flew to London as flight attendant many times to visit my family. My dad was born in Wales. The weather in this area can be absolutely brutal. I remember one time going to Heathrow and we couldn't land due to freezing fog . The visibility was minimal. People like this pilot should never be allowed in a cockpit. Such a sad, sad story.

I spent two years, back in the 90s, flying Trislanders for Aurigny based in Alderney. Flying low over those waters you can cleariy see The Race and The Swinge churning away!! I was always glad for the three engines of the Trislander.

When I was learning to fly, my instructor took me up on a cloudy day, and we ended up in IMC just doing the traffic pattern, I got really disoriented. When we landed, he told me he had done this to show me not to f*ck with the weather. How could someone with 3500 hours, take the decision to fly in IMC at night without a night rating? Speechless.

They didn’t. They hired a competent pilot who passed his work on to an unqualified amateur. He should be held financially responsible along with the owner for poor maintenance of his fleet.

Cowardly, greedy connections/powers that be re that crappy plane, and Ibestson, the pilot, whose lack of responsibility was disgraceful. The thought that it took several take off attempts is horrendous. Should've been an aborted take off. If only Emilio was more aware and able to take an alternative flight. It will always infuriate me. A beautiful young soul was lost in the most horrible way. ⚘️

Looking a bit further, it appears the owner of the aircraft, Cool Flourish Ltd (a vehicle for a Fay Keely and her family it appears) had *banned* Ibbottson from flying the aircraft 6 months previously, after the CAA had reported "airspace infringements" to them. However, they had left the operation of the aircraft to Dave Henderson - mentioned in this video - who it appears continued to contract Ibbottson to fly it. One might surmise there is a case to be answered by David Henderson as to what kind of cowboy operation he was running, around both the qualifications of the pilots he was hiring, and the maintenance of the aircraft.

From other comments, it appears Henderson was tried for reckless endangerment of safety of an aircraft, and admitted other offences, and was sentenced to 18 months of prison in 2021.

While it's true that the IMC rating, now called Instrument Rating (Restricted), is an "IR-lite", the flying does include a great deal of what actual IR ratings include - but you just don't need to pass all 7 ground school exams. As Hoover said, it's a "get home" licence but while you can use an IMC in European airspace, you can file for a VFR flight and during the flight, once you're back in UK airspace, you can contact ATC for temporary IMC and "Traffic Service" (which I believe is something similar to "flight following" in the US) just to get through the clouds. After when you have to return to VMC/VFR. Either way, this accident hit the news here in the UK and it was a real mystery for weeks. Speculation: I have a feeling that Ibbotson may have survived the impact and tried to escape. As GA pilots know, when you're about the crash land, you unlock the hatch so he'll have been able to escape while Emiliano Sala will have been strapped in, in the back. Exiting the aircraft would have resulted in Ibbotson because swept away by the currents.

That’s exactly right. The club have been hammered in the press and social media. But they wanted him to fly commercial. This was declined. The flight was arranged the facilitating agent.

The NTSB doesn't investigate those in most cases. I've got a few videos where passengers have landed the plane, one where the airline pilot dies on takeoff and the First Officer lands the plane, and a few where student pilots were able to land with emergencies. Hoover makes great videos covering a lot about the smaller aircrafts, that doesn’t get that much publicity. I also watch Mentour Pilot who covers major accidents AND incidents (with a happy ending). I’d love to see Hoover and Petter (Mentour Pilot) do a video together as they both are great story tellers and extremely skilled and these cooperations are ALWAYS fantastic as 1+1=4 in those cases.

So the actual crash event would have been due to a VFR pilot flying IMC under the effects of carbon monoxide poisoning, straining to maintain consciousness AND control. Jeez that's rough, not to mention all the other issues Hoover noted. Talk about a doomed flight.

I am more qualified to fly that plane than this guy . No instrument rating flying in the UK is really impossible. There is a TVG high speed train from Nantes to Paris and he could have taken the euro express to London and Cardiff. He would have been there in the morning after a good dinner and it's much more comfortable than the Malibu - What kind of pilot takes off saying he's wearing his life Jacket ??
