Following two fusions L3 through S1 I’m now 6 months out. That muscle was so painful and with walking very far I get complete saddle numbness and then paralysis of both legs. I have to sit anywhere from 10 mins to an hour to get feeling back in my legs then I’m good to go until next time. Walking very far is limited and or sitting for prolonged amount of time. NOT one doctor has mentioned this muscle condition to me. My Neurosurgeon referred me to a Neurologist and even he did not mention this. I brought up to him. He has now ordered a Nerve Conduction test and we discussed CES. But Neurosurgeon stated he had no idea what is causing this. I’ve been on my own educating myself and watching you guys. I have a total of 3.5 years of this and until now from watching you I am going to try the exercises you have instructed. I’m a young 62 year old women who weighs 104 lbs and have always been healthy up until my back started in. I can’t thank you enough. I will continue to go through all your videos in hope to find more information regarding this muscle. I pray you both are the answer to my long suffering. Thank you.

I had piriformis syndrome with the pain going down my right leg. I followed the recommendations of many, and began to stretch and roll the muscle. It made it worse. Then I watched a video from gym instructor, who said that people often make the mistake of stretching the muscle, when it should be contracted, i.e. the piriformis is too weak, causing it to become inflamed. After soothing the inflammation, I started doing side-lying clam exercises with various exercise bands to build it up - and the pain disappeared. The conclusion is If you have a tight muscle, then stretch it; but if it is just sore, you may need to increase its strength.

@@honua420 I hope your pain does go away. Mine went away fairly fast. I have a tendency to sit cross-legged on side side more than the other, and I thought that might be to blame, but it was not. It may perhaps be caused by a muscle imbalance. I also had an issue in my right knee, with some mild medio-infrapatellar inflammation when I walked up a steep hill. I saw a physiotherapist, who diagnosed a weak gluteus medius - my single-leg balance was terrible. She said that as I cycle quite a lot, my quadriceps had become very strong at the detriment to other less-used muscles in the area, and was causing the kneecap to track more medially than it should. (Strangely, it never manifested itself when I cycled, only when I walked up a steep gradient.) I now do single-leg balancing, seated clams, side-lying clams, glute bridges (single leg and double), bridges with clams, lateral leg raises, lateral side steps, monster steps, single-leg kick back and single-leg squads (using the glutes rather than the quads). Start without exercise bands and then incorporate them gradually increasing the resistance. I now use the strong fabric bands. I hope this assists you. It can be frustrating establishing the cause. It could be down to a bad habit or something new you have been doing. Some people are unfortunate in that their sciatic nerve runs through the middle of their piriformis. Good luck.

I tried all these exercises last year, but I found that they irritated the piriformis. I had piriformis syndrome for a year and in the end the only thing that worked for me were the following exercises. Side clams, hip flexor stretch in lunge position, IT band stretch with band and quad stretch in prone. You can find them demonstrated on Ask Doctor Joe, 10 best hip stretches. I did them morning and night for six weeks and finally I was pain free. I love bob and brad but these didn’t work for me.

@@Lindys I think side clams were the most effective thing I did in getting rid of my piriformis inflammation. If a muscle is being asked to do something it cannot do, stretching it is likely to aggravate it. Stretching adjacent muscles may do some good, I dare say.

Been suffering from piriformis problems for years, which started with coccyx issues. Coccyx issues got resolved on there own after 6yrs and now piri has started for the past 4 years. Physio, masssages, accupuncture on off and stretches have helped. But oh wow, I bought the bob and brad massager for christmas and this has been a god send!! Thank you for the help! If there is a way to strengthen the glutes or a way to permanently work on not having the pain, is my next quest. For now grateful for reduced days of limping!

you guys are excellent! I've had this recurring symptom (which several professionals failed to diagnose) and has gotten progressively worse. My burning pain went from my sacro... joint through my butt and right hip and down around my outer thigh. I had physical therapy which helped a bit but plateaued and never went away entirely. I did prescribed exercises religiously every morning and walked several mile per day. I experimented by eliminating certain exercise and adding others. was doing much better except when standing in place for more than a few minutes - then my hip and sacro -joint would begin to hurt. I had a recent 3 week bout after a 2 hour kayaking outing. when getting into the boat, I kind of whipped my lower back when I went to sit against the seat rest but it had slipped away. it didn't hurt right away, but by the next morning I could hardly roll out of bed! Any way, I slowly have recouperated to the point where I could do my exercises again. I started reading articles on the net and found your video about the piriformis which totally made sense for me. Thank You!

Wish I could have seen this video before! I overcame chronic piriformis syndrome (2 years) just this season. I'm a young mountain athlete and I had piriformis syndrome in my left glute, it affected me when ski touring uphill and prevented me from training and racing effectively. It also affected me a little when mountain running. I tried a lot of things and couldn't get rid of it until I started Glute Activation exercises (hip extensions, donkey kicks) and realised in my case, my glutes were too weak and overtrained, so they were "asleep" (gluteal amnesia). Now after some weeks teaching them to switch back on, with strengthening too (once able to activate them in exercises, I do pistol squats, lunges and glute thrusts) the pain went away VERY quickly... didn't take long at all. After 2 years of sciatica-like pain down my leg! It hasn't come back at all. Hope this helps someone!

After a month of growing pain both piriformis are painful - though left múch worse. Real pain in the butt. And hams and inner thighs. And shin. Ànd all that jazz. Sciatica was bad. After the "flu" reading in bed things started spiraling down. Self massages are indeed challenging. So glad for PT's.

I do these stretches, along with clamshells. Very helpful, IF I do them consistently. I do the stretches daily, but not so faithful about clamshells. Also use a lacrosse ball to loosen up the butt cheek. Next stop: Amazon, for the massager. Thanks for the videos; they've been really helpful in reinforcing what I need to do.

At the moment I'm just using muscle relaxant medicine and hot fomenting. Its helping reduce the pain .but coming back again later. I tried to do the tests mentioned by you and pain is increasing when I try to bring my left knee towards my right shoulder. Please suggest what I should do..

The piriformis muscle presented in my feet.I kept going to the doctor a complaining of foot pain and he said it was a bunion or arthritis or something else. They even said it might be gout I was tested for everything with no results. Finally my whole foot blew out and I ended up in the ER demanding to see a podiatrist. The podiatrist sent me to physical therapy. The physical therapist saved my life

I’m currently getting graston therapy treatment from my chiropractor. He determined that it was sciatica however now I’m thinking it’s Piriformis. I have pain going to the front of my hip almost like a locking licking pain. Would this be coming from sciatica or Piriformis? I’m on Guam so my medical treatment is limited. Thank you both for this video! I've been having pain in my left buttock area that's been causing numbness & tingling down my left leg, which then causes my foot to tingle & go numb as well. My leg/foot gets to the point, at times, that I end up dragging my left foot as I walk, so I over-compensate my left foot stepping so I don't drag it. This video addresses this issue directly. I'm gonna have to steal the tennis ball from the dog and start my floor massages! Thanks again!

Thank you for saying in a previous video that sciatica is usually a matter of having a slipped / herniated disc...or a pinched piriformis. I've been suffering from severe pain down my right leg for the past 2 1/2 months caused by my improper stretching exercises I was doing while trying to limbered up to do splits like Jean Claude VanDamme. Well, after seeing a couple chiropractors and an urgent care office, I gathered from an X-ray that my spine was just fine...so it must be a piriformis issue. Much of my ache started around the back of my right hip and went down to the front of my shin. I got a Wahl percussion massager from Walmart (not cordless, but was 10 bucks less than the one in your vid...and didn't have to wait for shipping) and it about makes my ears ring with delight when I hit a couple pressure points in that general region of my buttock. I wish the doctors I saw had a better understanding of this condition (false sciatica) and not try to treat it with acupuncture, cold pack regimen, medication, or other "therapies" which are a waste of time and money. I'm thankful that there is hope that I won't be stuck with this condition for the rest of my life...as I really depend on my legs for my line of work...which I've had to slack off of a bit here lately due to the pain which ibuprofen and tylenol mix just does nothing for relieving this kind of (nerve) pain. About the only thing that's helped my pain go away is simply sitting down on a firm chair...and crossing my right foot over my left knee.

First time I had this,I also had a problem with my RIGHT KNEE.(piriformis right side).NOW,this time,3 years later,I have an issue with my LEFT KNEE and guess what...piriformis on my left side.I am not a doctor,just a carpenter but each time it was one thing that led to another.I hope this helps someone out there,but I followed these two lads videos and it helped the first time,so I am back for a refresher.It sure sucks getting old !

The pain really gets worse if I do any kind of running , usually last for 2-3 days then dies down to my original pain. Ive tried physio, chiropractor and acupuncture and nothing has helped at all. @@brydobhoy123 Not really what I experienced, and piriformis syndrome *usually* radiates pain from the butt muscles into the sciatic nerve and then down your leg, since it is actually pressing against that nerve/ very close to it. Because the piriformis syndrome is often tied into tight hip flexors, quads, and hamstrings, that is why those and the glut muscles are targeted to stretch and otherwise break up the knots and loosen. I am not a runner. Any good physical therapist - assuming your problem is not caused or made worse by low lumbar disc issues - should be able to use massage, stretches and ultrasound, and teach the stretches you can do at home. In time, it SHOULD go away. Acupuncture, while I believe it can help with most pain, I doubt seriously is going to help you with what you need to do to stretch and loosen up the glut, quads, hamstrings, piriformis and hip flexor muscles, which all contribute to this problem. IF you have not had an MRI of your lumbar spine, get one, since PS is not something that is uncommon and can be pretty easily resolved (even if it takes a few months), with the proper modalities of treatment. If the MRI doesn't show disc issues then you are looking at sacral and lower muscle tightness and again, a physical therapist (and maybe not running for a while, as that will probably keep it flared up). Again, you need to be stretching 3-4 exercises daily - morning and 1-2 other times, to loosen up quads, hamstrings, hip flexors and glut muscle groups. If your MRI shows low back disc problems, then you need to address those with your doctor and then a PT, and maybe a chiro, to treat that while you work on the exercises that Bob and Brad have shown for true piriformis syndrome and associated pain in the hip muscles and down into the affected leg in 3-4 videos. I hope this has been somewhat helpful to you. @Bob & Brad I hope will concur also.

ORIF here. Piriformis is so tight that I almost can't get the left ankle up to the right knee. Working on it though! Heading out to therapy as we speak.

I had a terrible problem with this and did the stretches for months. I discovered for me, it's postural. I love to sit and sleep in a position I don't know how to explain, like the slim Buddha. Especially on the left, with my heel at my crotch. I quit and the pain is gone. I didn't know about the massager, I might have to get one if I can sit like that again.

I appreciate these videos. I won't be able to afford to seek treatment for about 7-8 weeks. I have had SI joint pain and Piriformis pain in the past. I slacked off on my strengthening exercises over the years and now the pain is back with a vengeance. Do you think I can get these two issues resolved on my own just to the point where I can sleep? For the next few weeks until I can see someone?

Please help me ! I have the symptoms of Piriformis ( I think ) , but mine gets very bad sciatica pain when I walk . example : I walk into the grocery store not in much pain , after going up and down the aisles for 15 minutes I can hardly walk because of pain . I need to keep the grocery cart in my hands to help me walk . . I get in the car and by the time I am home the pain is gone , or much less . I wake up in the morning feeling like I was kicked in the butt by a mule . At the evening I have to lay down because of pain . I use Voltaren a couple times a day , and take Advil or Tylenol ( does not help much ) . I am 73 years old , a retired carpenter that has always been very active and physical , now I feel like I am crippled ! I do not want to see a doctor , this spring I had a terrible time with nerve pain from a bulging disc in my neck . Nothing helped , but after 3 months it dissappeared ( the pain ) . I do not want to go thru all the nerve pain ideas again . Been there , done that !

When ever I lift my leg (knee) above my hip I get a popping thud in my hip area. When doing leg raises hanging from the bar or even lying on my back doing leg raises. Standing and just raise my leg... Any idea what this could be and how to treat it?

@@michaelmaldonado2619 I've done some research on my condition. You should look up snapping hip syndrome. It's when your hip bone snaps back and forth along your tendon and creates friction. I've been stretching and strengthening my hamstrings, hip flexors and adductor, piriformis muscle and glutes. Give that a try for a couple weeks

greetings.i am ageneral surgeon from Egypt.i genuinely appreciate your effort and talent. i ve a query! to the best of my knowledge the piriformis works as internal rotator when the hip is flexed more than 60 degree. if this is true, so how internal rotation in the FAIR stretches the muscle though we are approximating the muscle attachments? or is it the adduction that does the trick? more and again if the adduction stretches the muscle thus the piriformis is to be regarded as an abductor i hope that my question is written in clear English thanx. looking to read an explanation

Great video for Piriformis in seeing it and what is does lot of good information base on a fix point and areas of the body. I take issue with solution to pain and range of motion of Pirifrormis and abilities a person finds them self in. I am talking about solution to the affect of Piriformis has on us and far more to it unless you just want deal with it repeatably. . What is missing to bring Piriformis back to function condition is the Fascia affect on ability of muscle to have range of motion and it support to movement.. What missing is solution is the connecting tissue or material that links all muscles together to move the body. To understand to change begins with functioning ability of basic what I call cables force like the Piriformis abilities is liked to the IT Band and if IT band is tight that affect what you can do with Piriformis and why it is connected that matters which is Fascia or ground substances that is through out the body makes up the body ability to move.. The solution are nice like vibrator and rolling aid in relief but it not reconditioning to make where you do not need to why it is Myofascial issue in range of movement and tensile strength for movement. You have strength to hold weight but tensile strength is the force of muscle during movement like walking. One can have strength holding weight but movement can me weak two different characteristically.. we are dealing with movement and what allows us to move and Piriformis muscle is a foundation and look where it is connect to on lag at that point only but what does it support in the lag that affect is ability to active range of movement . Without understanding Myofascial release which ins Muscles characteristics which is not simple but then add the relationship to Fascial characteristics and releasing is Facial material for it determines range of movement base on what it will allow muscle range to do. Understanding if your fascia has turned hard there is not movement support by it that limits in that areas of body. We are liquid what does that mean we are Fascia for when it comes hard your not movement will. Example if your having Piriformis affect on leg and back and need to release it which means the muscle has NO range of movement. What affects piriformis in leg is the IT band which is side of leg so linked directly to the Piriformis. IF the IT band which is on side of lag is tight that affects the Pirifromis pulls. The fascia shape linking between Piriformis and IT band has to be address if you want Piriformis to really have range. This is NOT simple for person has to do it and lot of realization process and releasing and affecting tinsel strength within movement. It fun once you. realize how to affect the whole body for direct actions. There is more for add core breathing to affect energy release tight is directed to shoulders and stress range muscle movement. Check out if your IT band is lock up, tight on side of lag and that part of leg that areas of leg cannot move that will pull on Piriformis muscle in movement that places pressure on Pelvis areas in many ways.that affect spine. NOT understanding connection internal forces as part of treatment limits solution of pain. Without soft IT band that is a bad sign for Piriformis but more here we dealing with ground force influences of muscle range of movement. What allows movement is Fascia connecting to the muscle itself and NOT releasing fascia you just working to keep pain away not having solution to range of movement as one get older. I talking about liquefy the fascia or ground force material. Heat affect things but to really liquefy say the IT Bands is facia challenges. They are really good ideas and solutions without dealing with Myofascial relations limits solutions to range of motions and types of pain. I hope keep put the good work. What is missing also is bore breathing to release process that influence range of movement base on stress in shoulders. if you have stress in shoulders that affect range of movement base fascia action. Just realizes it not simple and there lot person can learn to affect their body and mind relationship.

i have a really annoying pain down the back of my left thigh, so bottom of butt to behind the knee. none of the motions in the video triggered it, but when i walk or bend over it hurts. or if i do a leg raise but move my leg to the right. straight up and down doesnt trigger anything. definitely different than a normal "stretching" pain. no back pain except normal soreness if i stand a lot. im 25 and dont wanna spend money at a chiropractor if i dont have to

Hi bob and brad! I have some questions, I have an herniated disc in l5- s1 and a bulging disc in l4-l5. Most of the time I don’t feel a pain in my low back (maybe like once a month) but I always feel pain exactly where the Piriformis is and down the leg when I do this stretch. So what do you think I need to do? Every time I stretch this muscle I will feel the same the day after. I’m in this situation for more then a year! My hamstrings are tight too, always, not mutter how much I try to stretch them and working them out. Nothing works. Please help me. Thanks !
