I hope, Dr. Hash I and Michele, you can create an episode about SLEEP! I am stage 2 ckd, and I’ve suffered for years from poor sleep. I use melatonin, echinacea, etc. to sleep. I’d love to hear your recommendations on safe sleep aids beyond the usual typical advice on KZbin about sleep (less blue light, etc)

For MANY years I've had a terrible problem with kidney stones, in 2017 it was suggested to me that I take K-2 and D-3 as a preventative.... IT WORKS, for me anyway, in over 6-years I've only been to the urologist twice for a kidney stone issue 🙏

Taking vitamin D3 and sugar raised my gfr from 48 to 60…. And it helped my Osteopenia.

Good analysis. D is fat soluble so taking with meat it is better absorbed. Magnesium helps with absorption. D2 is plant based and not as well absorbed. A 25hydroxy test will tell you the D level out of the liver. The 30ng/ml is fine for bone health but insufficient for immune health, where above 50ng/ml is much safer. D is a hormone and controls over 2000 genes through the vitamin D receptor. VDR polymorphism is another issue where most autoimmune patients have dangerously low D. Low D = poor immune function. With regard to impaired liver or kidneys. You can get 25hydroxy injections if you have liver issues and low 25hydroxy. The kidneys process the 25hydroxy to 1,25 dihydroxy which is the active hormone. I don't know if there are 1,25 injections. If there are it would help compromised kidney patients. 5000iu D3 very safe for any adult, but get tested. Hypercalcemia can occur in high doses but rare and non lethal. Coimbra Protocol uses very high doses for autoimmune patients, up to 700,000 iu daily but requires medical monitoring.

I have Sjogrens Syndrome, an autoimmune. disease. Because of Sjogrens, I have microalbuminuria, protein ( albumin) in my urine. So… vitamin d3 would be ok for me to take everyday? My nephrologist said it would be ok , but never gave me a dosage recommendation. I did a little research myself and came up with 2500 IU a day followed by 200 mcg of vitamin k2 and 200 mg of magnesium. Do you think that’s safe to take and do you think it will help my kidney function?

Please address the issue of ckd induced insulin resistance. The renal diet is comprised mostly of carbohydrates which increase blood sugar. Is there a way to avoid becoming prediabetic/diabetic on a renal diet? Thank you.

The "old renal diet" which is outdated and recommends refined carbs/starches has a greater impact on blood sugar levels. Getting plenty of fiber in the diet is important for helping control/prevent diabetes, but also for people with CKD. We will have an entire episode on fiber and higher fiber carbs in the future to address in more detail!

Hi I have received labs that I have high PTH, normal calcium level, however I have calcium in my urine found after a 24 hour urinalysis. Vit D deficient 20. Ferritin level 5. I was told I don’t have kidney stones but I have calcium in the the kind way tubules. I’m worried and I don’t see the VA nephrologist until March. I found it odd that my dentist says I have calcium decalcification on one of my teeth. Never happened before and the endocrinologist says I have calcifications in my thyroid. Some thing is going on but what? Thanks for reading.

Your mother needs to be tested for Vitamin D3 to see if she is deficient. It is a simple blood test. If she is deficient then her doctor can figure out the dosage she needs. She should take Vitamin D3 and Vitamin K2 MK-7 since these vitamins work together. IF your mother is deficient and 50% of Americans are deficient in Vitamin D3 then taking supplements will help her kidneys, blood pressure and might help her diabetes if it is type 2. Your mother also needs to watch her sodium intake, not drink colas or sodas with phosphoric acid in them, no NSAIDS, no alcohol, watch her sugar intake and not smoke tobacco. Tobacco reduces your Vitamin C levels by 30 IU/cigarette. She also needs to get her blood pressure under control and the same goes for her diabetes. Stage 2 chronic kidney disease is not too bad. It is a warning sign and she needs to be mindful of what to do to keep it from getting worse because the kidney diet is very restrictive and once you start dialysis your quality of life and lifespan goes way down.

Thx Dr Hashmi and Michelle for the valuable Infos. It is recommended to take vitamin D3 with vitamin K2 (mk7) BUT not at the same time ( avoid vitamin D3 and K2 in the same capsule). For instance take vitamin D3 in the morning and vitamin k2 in the evening or at least 3 hours later). All the information given in your channel is outstanding. And admittedly a level of 50-60 for vitamin d3 is optimal but for a CKD patient or for preventing ckd disease even 30-40 is maybe OK.

People with CKD should have vitamin D blood levels checked and supplement (dose per physician or dietitian recommendation) based on those levels. It is very common for people with CKD to have low levels of vitamin D.

You will want to check with your doctor on the appropriate dose that she should take as it will vary from person to person. Generally 1000-2000 IU /day can be used as a maintenance dose. 4000-5000 IU to slightly raise Vit D levels. And for someone deficient, a higher prescription amount may be needed. So definitely check with physician!

IMO, 30ng is a poor standard. Immune cells lack a dedicated VD receptor on the cell surface; they rely on passive diffusion through the cell wall for VD uptake. They thus require a higher serum concentration of D in order to be D sufficient. It seems likely that an immune-adequate 25-D blood level is around ~70ng. The 1~2K maintenance dose she's discussing is quite unlikely to support 60+ng in any normal sized adult.

You only need to get your Vitamin D level checked once a year and considering how expensive it is to treat all the conditions that can be caused by a deficiency of Vitamin D it is a real bargain cost wise.
