Couldn’t get parts for my race bicycle, switched to running, ramped up volume way too fast. I’m a doctor, assumed it was a deltoid ligament stretch. This is proof that even doctors like me can be idiots.

I have had flat feet since I was young. Working on my feet has always hurt. The pain in my ankle ( PT now that I know what is injured) has increased and keeps me from much walking, I am trying to find the correct ways to rehabilitate the disfunction. Your video was very helpful to understand what happening anatomically.

No idea why mine happened. Possible damage from inversion table usage during treatment for sciatica and suspected bulging disk. All brought on by starting a kettlebell program too aggressively. Being over 50 seems also part of the problem.

I drive a school bus-big van size. I got mine from climbing out of the bus and landing on my left foot. I vaguely remember coming down on it pretty hard once, but it wasn't enough to stop doing it. When the pain got worse, I finally realized what it was. I started turning fully front-wise to slide out of the bus and land lightly with both feet. It made a HUGE difference. My body told me right away that a front-wise slide was the best way to get out, but my ankle still hurt, and I need to find a way to relieve the pain. I'm trying acupuncture, light stretching, and rest right now. I'm also going to wear my long boots when I go back to work.

Loved the video! I hurt my posterior tibial and then anterior tibial simply walking 4 years ago. It was slowly getting better after seeing a podiatrist, orthotics, many good shoes, and physical therapy (3 @ 6 weeks). I recently went on a hike without my orthotics and aggravated it. The last podiatrist suggested I was out of alignment and suggested a chiro and then physiatrist which I started getting nerve blocks, nerve test and now I'm going to see a neurosurgeon. I have a pinched nerve on my L5. Meanwhile my ankle big toe and balance are bothering me and a tight hip on the opposite side. I keep working on it though.

Thanks for the video. I’ve had this injury since running in a football match. Slammed my foot down and instant pain. It’s been 2 years now. Really long journey. Started with orthotics + physio. Then we went for the injection and air cast boot. Back to doing physio now. I still get pain quite often and haven’t been able to get back to football yet.

Went from working at a desk to working in a factory on cement floors. The pain went away with rest, ice and elevation... but came back after I survived sepsis.

I only have pain after waking up and getting out of bed or after resting my foot. After a few minutes it wears off and I can walk with no issues. Would a brace be the best way to go along with stretching? I don’t have a tear or rupture but I have a accessory navicular bone but that doesn’t bother me just the inner ankle behind the medial mallus.

My PTT hurts on my dominant foot after a lifetime of CP affecting the other leg. The university hospital I went to for years did not even have an option to do PT on feet, but they had PT for other areas. Explain that, one, please! Thank you for this video. It gives me some place to start.

Your video is the best I have seen concerning PTTD I had it real bad about 5 years ago, and there was very limited videos on this. Thank you for taking the time to not only describe the injury, but showing ways to rehab it.

Thanks for this video! My PTT was caused by surgery to remove a calcaneal bone spur (caused by plantar fasciitis from too much running). The surgeon saw my plantar fascia was like a thick piece of rubber and thought it wasn't doing me much good, so he took a chunk out next to the spur. It immediately caused arch pain that my chiropractor, PT, and finally podiatrist couldn't figure out. My sister-in-law, a chiropractor, said my naviculocuneiform joint was subluxing. I ended up with OTC orthotics after all that, my podiatrist wanted to fuse the joint. This was back in 1989 and I still have pain depending on my activity, but definitely NO more running or high impact for me. It would be great if I could go on a long hike again without pain. Really great advice! As a 30 year runner, small person, I got froggy and ran too many consecutive days in lightweight running shoes with no superfeet and bam got a lovely case of PTT. I'm struggling to run during my doubles tennis and cannot run at all. I have been following all your advice and doing your exercises. I love the idea of wearing the good shoes/orthotics while doing the stretches, makes so much sense AND to stay away from the anti inflammatory pills after a week or too. Thank you so much!

I woke up with the pain and am unable to walk. No injury or repetitive walking/running. Could it still be PTT? It’s definitely where the pain is. 10/10. Like I said unable to walk or move foot toward shin or toward floor.

Does the patient having a high arch exclude this diagnosis or suggest something else eg plantar faciitis?

Thank you for making you tube otherwise really relevant and thank you for helping a lot of people who do not have access to medical help as easily. What you describe is exactly how I feel but I am sure I did not physically injured my ankle. In fact I started having the symptoms and other joints pain when I got COVID. Can it be related in anyway?

Great information! Thank you. It would be great if Hoka made narrow shoes, but they don't. I wear a 16AA, which is incredibly hard to find in any type of shoe. Thankfully, New Balance at least makes B width shoes, which are still too wide for my feet, but they're much better than D width shoes.

Hi. I massaged my posterior tibial tendon in the calf area with a graston blade way too hard. Now my ptt really hurts. What is your recommendation? I’m icing now. I’m in CA, so I can’t make an appointment. I hope you can advise if icing is a good plan. I’m pretty anxious about this. Thank you for your help/reply.

I had a lapidus bunionectomy three months ago and I have had this pain thereafter. Im desperately learning how to walk again with a shorter great toe and I think the pain in the ankle is caused by the acute weight bearing on the foot after the surgery.

Wow I enjoyed the clinical Rehab aspect towards self healing. So, I believe I have a tear to tibial posterior tendon. R/O By MRI. My Ortho Doc did a simple test. Can't stand on my Rt toes. So I have Rehab and no surgery at this time. I did the long boot thing. Then a custom made Ankle brace. Not using as much. Kinda hurt alongside my ankle. It's hurt on and off . I'll my foot does drift to the side .that's why I have to wear brace. I want to go back to jiu-jitsu. Funny thing is my Chiropractor a Dr. Had already dx my ankle injury prior to orthopedic doc. He has help out beautifully. Thank u for encouraging notes.

When he pushes down, his fingers are under the toes. When he puts the orthotic and pushes down, his fingers are not under the toes therefore of course it won’t bend. Not that everything else is false, but you can clearly see that.

Hi I have overpronation which causes my feet to push inward therefore causing inflammation on my tendon. Is there any insoles you highly recommend to help I’ve bought a few and still in pain by the end of the day. I work with kids and on my feet a lot. Any suggestions would be GREATLY appreciated!!

Great video, I am sure this is what I have. Was in motorcycle accident 2 weeks ago then ran on /off over next 2 weeks. For then just went went evening after a slow walk. Cannot walk on it. Very inflamed, A&E diagnosis is reactive arthritis but this looks way more likely. Gonna get it checked out. Many thanks form the UK on this video. Insightful stuff. Ross

I broke my right foot 8-10 years ago. Foot now is very bad. Unfortunately did no physical therapy. Ultrasound shows that tendon is lax, no inflammation. Also doctor (In Sweden) said surgery not possible, too mobile ankle. Do you have any suggestion ? Ready to do anything. I

I had an Mri had treatment it felt better was on mobic. Then the pain come back in a whole new way. Got a new foot doctor cause the other doctor told me I had to rebuild the muscle. The new doctor sent me to therapy it helped but arch pain and outer foot on top pain continued. New doctor don't know what to do out side of more testing and more pills. I going to try your recommendations but I don't know what else to do.

I’m always confused about orthotics. How long do you have to wear them? I’ve had pttd for a year. It gets better and then it gets painful again. I’m using orthotics and an ankle brace. I’ll try a cam boot. What about deep tissue massage of the posterior tendon and the muscle from the ankle to the knee? Do you have a video on massage for this problem?

Orthotics and ankle braces are a piece of the puzzle but sometimes cannot fix the entire problem. In almost all cases, the root cause is a biomechanical imbalance, or lack of stability, causing too much pressure on the inside of your ankles, leading to overpronation. Orthotics and a brace can provide stability, but if the knee, hamstring, glutes, or lower back are not loose enough, this will only lead to more pain (which is why most do not get success). I always perform a biomechanical exam to address the source of instability or stiffness (or both). Even then, there are different levels of orthotics and braces. @@DrTomBiernacki was in a cast for 3.5 weeks. Now been in a boot whenever I leave my bed with crutches. It’s been a little over 2 months. I just had a follow up MRI now. Patiently waiting but not sure what to do. This has been the hardest thing mentally and physically for me as I’m typically very independent person who goes out a lot and works out everyday.

I’ve been trying to get rid of this for a little over a year. Tried PT, EPAT, and modifying workout. Podiatrist says get cortisone shot or prp….I don’t want a bandaid approach so what should I do? I’m a super active person and want to resume my regular activities. Please help!

I had this over 10 years since I joined the military. They never bothered to get me any imaging. Haven’t been in the military in in 5 years. I no longer run or even walk much and still have this problem. Is there likely to be a tear that’s never healed? I’ve definitely had enough rest time.

I tore my TPTT while marathon training. Had a 2cm longitudinal tear in the tendon, spent 6 weeks in a CAM boot and have be rehabbing out of the boot for 2 months following low impact plan. I am almost 4 months into rehab, havent ran since in that period. Seen a surgeon who recommended more time .Is this unusual for it to be so slow, i still have pain/discomfort (not intense but noticeable) simply walking

Thank you so much for this video! Very helpful! I'm in pain for few years, and recently it's constant pain. Have flatfoot, using custom orthotics. Waiting for MRI to get final diagnose . Meanwhile, sport injuries doc prescribed ankle brace, and some exercise. I used to be very active, at least 10-15,000 steps a day. Now just pain all the time, day and night. Can you please name the brand of shoes that you recommended?

What Hoka shoes for women can you suggest? Regular some walking type. A bad accident put me in trauma with a tibia and fibula compound open wound ankle fracture bone came through skin pins sticking out from knee down to ankle. Science project wrong. Arggghh! then tendinitis, in the same place described above I was doing so well with physical therapy and almost 3 years out of having the accident and then one day this hit me. Most excruciating to want it cut off. I recently came across this video and have been trying the suggestions and I can say it is been helping so I really appreciate what you do to help us. Chronic pain will drive one crazy and for me to be up in the bed with my foot ( after being in it at home for rehab and having to let it rest has done a step back in my brain. I thank you. G

Hey, thank you for this. I believe I have Post Tib Tendinitis as a marathon runner and have had to take a break from running. I’m able to walk sometimes 15 miles a day with no pain, but when I run, I begin to felt a tightening pain after around 2 miles. I have quite a neutral foot gait. Shoe wise, would you advise trying a support shoe or just building the strength of the muscle up? Any tips appreciated :)

Are you aware of any form of light therapy providing relief to posterior tibial tendonitis?

It can be a treatment for sure, we are not sure of any great studies showing how well it can do.

@@DrTomBiernacki Thank you for your fantastic video. I have an Arizona Brace, custom orthotics, shoes with a rocker bottom and none of it has prevented foot pain when I'm on my flat feet. An MRI and X-ray do not indicate tears or fracture but do show edema and a little arthritis. I may explore what research John Hopkins may have to offer here in MD because I don't know what else I can do. Simply miserable.

Tibial plateau FX fracture 9/11 20 ok now after completing a knee surgery 11/12 21. Now it 3/7 22 I should be able to glide across Amazon floor. But this excises help and pomegranate juice. Ouch.

I've been suffering all my life with these devil foot problems some foot doctors drive me crazy stating that it's just an arthritis now I've pain all the way to my toushy through my lower back sometimes I can never sleep burning pain me like I'm in hell hope that can help with the agony of this problems that can drive ppl crazy.

I suffered a foot contusion during martial arts training on my navicular bone and the ligaments over it back in mid-August. The doctor confirmed this. The MRI, more two months after the contusion occurred, did not show any fractures. Just scarring and still tender ligaments. At present, I am still in some pain but not nearly as bad as it was back in August when I had a noticeable limp. Made the mistake to walk barefoot for about a week back in late October and now I am paying the price for it. I think I regressed some after that barefoot week. I noticed my tendon flaring up since barefoot week. The doctor did not diagnose this but it sure does feel like it. Now, I am using Orthotics, compression sock, ankle bracelet, a thick sole shoe (Sketchers), some light PT, and ice. I aim now to aggressively rest my foot over the next two months and assess my improvement after two months. Want to still work out but I think I should stick only to lifting. But can I do barbell squats and deadlifts? Will that put pressure on my foot? Again, I have good shoe, ankle bracelet, and orthotics. Finally, what is the best cardio? Swimming?

It's usually from a combination of tight calf muscle and weak feet and arch muscle that causes lingering PTT. Get a night splint--it stretches your calf when u sleep to take pressure off the tendon to reduce swelling and allow blood flow. You would be surprised how good you will feel in the morning because it the swelling will be gone. When you walk, focus on using your arch and bottom of the foot muscles to push off to strengthen these muscles what will create natural orthotic for your feet--as your bottom of your feet muscles get tighter and stronger you will feel more relief. Most ppl have weak arches or collapsed arches after years of getting heavier and doing minimal physical activity and this weakness is exposed during overuse or acute injury to the PT tendon. When the pain is lessened and your feet muscles get stronger, start stretching them and working through their range of motion as part of your daily routine. I would seriously avoid ankle isolation braces and rigid orthotics ... it will not address the problem and will just make the condition underlying condition (weak feet muscles) linger longer if not forever. Always be mindful of how you walk--contract the arch and adjacent bottom of the foot muscles with each step. Also, wear tighter (instead of loose fitting) shoes as this reduces sloppy feet movement. You can gain a lot of mobility and reduce a lot of the pain but it takes daily work. Last, surgery, sugar/stem injections do not guarantee anything other than a bill. Remember that. Just look a proathletes, their surgeries often dont always work out and they have the best ones their millions can buy. Stick with strengthening your foot by being mindful of each step.

Great input, I just don't see why people discount braces and orthotics due to the minimalism movement. These are real injuries that need to be protected and healed. These problems are not usually healthy 20 or 30 year olds, we are talking older people improved almost immediately with good shoes orthotics and braces. I don't see why it became cool to bash anything supportive now, even though it is cheap, works 99% of the time with no risk. Must be the minimalist shoe lobby really pushing their shoes and the 1 or 2 minimalist shoe articles out there.

Hi, I’ve been getting pain in the area of the inside of the foot and it is a sharp pain and can only be relieved with ice. The thing I don’t understand is that I walk on the outer edge of my foot ( as I have club foot deformity) so all of the the pressure of my foot goes to the outside edge and also my ankle doesn’t touch the floor. I had all surgeries as a kid including Achilles lengthening , my foot was basically broken and reset to a rigid walkable position. I am now in my 50’s and manage ok , apart from chronic callouses on out side edge of foot sole. But I don’t understand what the sharp pain is, like someone sticking a knife in the inside of my foot. Thought it might be the tendon but if my foot is turning outwards then this is not what you are saying here. I believe in my medical notes that I had the navicular bone removed? Maybe it is this as this is the exact area of pain. I have made an appointment for the docs but this could be months. Thanks for any ideas.

I'm a huge fan of shockwave therapy, it is tough to use for the pt tendon for me though, it is great for things like Achilles tendonitis, plantar fasciitis, spur pain and more

Ive had 8 sessions of EWST and it gets better then hurts again. My podiatrist said it could take 3 months before I feel any improvements. Ive done it all : custom hard othodics, stretching, my legs and post tib have seen more ice than the titanic did, foot leg moon boot for 1 month, massages, and now 8 weeks of EWST.. Ive been watching these videos in hope to find something new.. Any suggestions would be great but I dont think there is much left to try.

​@@GamersHereAgain12 my tendonitis was on the interior side , for me it made a big difference and I only did 4 sessions, it is true that you dont see an improvment right away because every time they treat you there are new micro tears, before I had constant pain now I dont wake up anymore wishing to amputate my foot , I was treated by a very good chiropractor , I didnt find any podiatrist in my region who even knew what is EWST.

I've had PTTD pain for years. I have a small interstitial tear of the tendon as it courses medial to the talus. I'm quite functional and the tendon seems strong in that I can do calf raises, balance, and internal ankle rotation. My calf / legs are quite tight and I have flat feet that is worse on my foot with PTTD. I've worn orthotics for years and I"ve tried PT. Any ideas besides surgery? I've been told the tendon is unlikely to heal on its own.

We have seen certain people heal with a great shoe, great orthotic and potentially even a supportive ankle brace. The human body does have the ability to heal a small tear if held in proper position for long enough, but nothing is guaranteed. Sometimes there is more than just a small tear as well. Always trust the people evaluating you in person!

Try deep tissue massage on the tib post and flexor longus muscles. If the muscles are tight try to get that fixed and the tendon *should* heal on its own

Hey, can minimalist shoes be helpful? I have been diagnosed with PTT and use orthotics + supportive shoes (Nile Structure 23). I can run pain free for ~5 miles every other day. I have a generally good stride, and usually run 60+ mile weeks and compete att high level. But I've had this for quite some time but it hasn't been relieved.

I should add it's most likely PT but the pain is quite close to my Achilles tendon. Almost feel pain in the back of my ankle.

I feel extremely frustrated right now as I feel like it doesn't get better. No pain while walking around, in the couch or standing. I've also gotten a really strange feeling of "click" on the back of my Achilles tendon. Like if another sinew is sliding over it.

The knob of bone on the inside of my foot (medial mallelos?) sticks out more than is normal. I unfortunately wore a pair of stiff leather shoes for a very short time, but long enough to cause pain in that bone and the tendon that runs underneath it. I had an MRI and my podiatrist said I had tendonitis and to take it easy for awhile. It's now been 8 months and the tendon still hurts! I have cut out a hole in my shoe so that no part of the shoe hits my bone. I like to play golf and even though the shoe doesn't hit the bone, the tendon hurts after golf. Is it the rolling of the foot during the golf swing that makes the tendon hurt? If that's the case, I need to heal this tendon and stay off the golf course. Please comment on how I should do this -- I think with ice and stretching. I can't wear any kind of ankle brace because it presses on the bone and hurts it a lot. . . . Thank you.

I am a single figure golfer and have a micro tear in the post tib. Golf is what did it for me. The downward motion I the swing and resisting the force, in combination with walking on uneven surfaces is a killer for the post tib. Ive struggled with healing it for over 10 years. Only now with watching this video and ESWT 8 sessions and a brace has it started to come good.

I’ve been in chronic pain from this for over a year now. My most recent ultrasound showed my tear had healed and my fibers looked parallel. But I still have an inflamed tendon sheath. Also moving my foot in and out of inversion/eversion produces a grinding sensation right on that post tib tendon like it’s not sliding properly. Anyone experiencing this?

Yes I’ve been over 2 years dealing with this. I found KT taping for postural tibial tendinitis helped relieve the tendon enough to get back into running again. I cross train lots to help strengthen it as well. At times I thought I would be a gimp forever but you will find what works and might slowly make progress.

i had my foot ran over directly in the middle of my left foot, and ever since then i have had pain consistently that seems to move around to different spots. sometimes its in my toes and then goes to my ankle, then my calf. the pain moves pretty frequent. anyone have any idea what it could be? im at a loss and cant afford to see a doctor right now and this pain is awful

Someone deleted my comment. No it is still the same. Think it might be my flexor hallucis tendon. I cant put my big toe flat on the ground without really trying. And if i do it hurts like hell. And it definitely hurts a lot more when i lay down to sleep

Hello! Don't give up! I have many health issues some that cause excruciating pain on a daily basis for many years now you can get through this,,, my grandson says I have nine lives! I'm having the same issue in the ankle the pain is a nine out of 10 (** no one really knows a ten until you have a condition that causes that kind of pain,,and yes, this hurts- a ruptured tendon, but not a ten) anyways I digress an ultrasound which is not that expensive and can be done in your doctor's office should diagnose the tendon problem. Can't you ask for that? My primary Physician conducted an ultrasound within 10 minutes of my asking for one,,,? IDK why you are having problems getting diagnosed,,? I hope & pray that you will have proper medical care soon. In the meanwhile, get a good ankle brace, wear a good tennis shoe at all times, get yourself some crutches get your weight off of it! (** Or it won't heal!) and ice it down,, and as the pain allows try to do these stretches that he suggests in this video. I have over the years recovered from so many injuries,, since I am an avid recreational sports nut,, I've had every injury from A to Z but you are right-- this is very painful! Good luck! 🤕* Yes, I feel like my life is over, too. I just got, 3 injuries in a row, from trying to work out- jog after an extended period approximately 4 years of being injured & very sick & surgeries and bedridden,,,so I was ready to get out and now I'm bedridden again LOL. See, it could be worse,, you could be like me! ** Don't be that person,,(* me!) 🙄
