I appreciate you mentioning HRT. From all the studies that I’ve read and interviews with the doctors who specialize in bio identical HRT all of the studies that were done we’re done with synthetic HRT not bio HRT. I’m trying the bio HRT because I have osteopenia now after being surgical menopause for 12 years.

A correction to what you said about prunes. You said 100 g is about 5-6 prunes. That is incorrect. 5-6 prunes is about 50 grams. One of the studies on prunes concluded that 50 g/day was still beneficial, after the 100g/day group had a high dropout rate (understandably!) Hi, Dr. Doug - I just stumbled upon your videos today (not sure why they haven't shown up in my sidebar before now), and I'm binge-watching. You're actually saving me money because I had a few Amazon Prime rentals in my queue that I was planning to watch today, and now there won't be time for that LOL. I have ostopenia and gradually worsening bone loss after being on an aromatase inhibitor since September 2020 following breast cancer surgery. I am very interested and motivated to incorporate all of these tips to minimize or maybe even reverse my bone loss (understanding I may not see real benefits until after I stop the AIs). However, I've learned from a few sources that vibration plates are not advised for anyone who is a stroke risk or has had joint replacement surgery. I had a TIA event in 2005, and I'm on Eliquis due to A-fib. I also had TKR surgery on my right knee in January 2021. What is your view on the realistic degree of risk for people with either or both of these issues? Thank you for this wonderful content and for getting right to the point in your videos and without bogging us down with ad nauseam ads every 3 to 5 minutes and lengthy sponsor promos. I'm on my sixth vid and so far have not felt the need to skip through one second of your content. Yay to that! I do, however, echo the sentiments of others who have complained about the blinking/flashing sign. My strabismus-challenged eyes are going crazy, so I have to just listen rather than watch, sadly. Maybe you could consider turning off the lights for your vids? Thanks again for creating all these great vids with valuable content! -Karen 🥰

Too much running causes bone loss. I think he said greater ctx in blood from cardio causes bone loss.

Great question. There is mixed data on cardio. Running can certainly drive CTX up but not always. Too much running without enough fueling and resistance training can lead to bone loss.

Is MBP in whey protein powders or a separate supplement? I won’t trust raw milk in my area, but would try supplemental MBP..edit/.what is supplement called? Nothing listed for MBP. Also I do eat 100gr prunes daily and have for over a year. Is this a conflict with MBP? Also I count calcium and protein intake and consume at least 1200 mg calcium daily with food-mostly dairy. (No calcium pills) PLUS so relieved to see the results of plant vs animal protein. So many conflicting things out there, but now this settled it for me. The big impact was the report that vegans have more osteoporosis. It is largely ignored, but I think should be expanded on and reported more. Ignorance is NOT bliss, as I found out. Thanks for the clarity and help in these multiple areas. Great video, as usual

You’re so welcome! Answers. I don’t think MBP would be in whey protein powders. There are different degrees of purification and may be in some of the lesser purified products but not in any appreciable amount. We are still researching the right product to recommend here. It’s in a few combo products but I don’t like the other ingredients so….. still working on this. There is no conflict between prunes and MBP that I’m aware of.

Quick question: I was confused about the impact of running / aerobic exercise on CTX? Common question. I should have clarified that! Running can likely have either impact depending on several variables. Distance, fueling and resistance training all play a role.

A common question. Sorry for the confusion. Truth is running likely can be both a positive and negative for bone health. The devils are in the details. Distance, fueling and addition of resistance training. Running isn’t bad but running long distances, under-fueled without any resistance training likely will result in bone loss.

Doing sardine really fish fast for a week . I gained twenty pounds after being told to eat high protein. I could tell before facing the scale. Last July broke my femoral stem left side had anterior hip replacement. Titanium like your iPhone. On Alendronate now tomorrow morning is the time one day week. I am trying to get the protein I need. I can tell my insulin is down zero hunger. Doing boron not into high fructose prunes.Counting the protein grams of fishes.

Thank you for this as I have high P1NP (100) and off the charts high CTX (860) , so lowering the CTX is my focus. A few questions: 1) the study showing testerone can help with men because it converts to estrogen. Would we expect testosterone to help lower CTX in women in addition to estrogen ?; 2) I wonder if raw colostrum would work particularly well. I used go get this from organic pastures. I wonder how much one should drink of it or raw milk? ; 3) The study on WBV was comparable to powerplates. Would you expect the marodyne to work as well on lowering CTX or not as well? Thank you for these amazing videos, I'm learning SO much and it is SO Helpful!

Thank you for the informative videos! I am 52, years old general practitioner in the uk, been recently diagnosed with osteoporosis, usually, I am fit and healthy. I had a lot of investigations to see if there is any cause of my bone loss but nothing was found. the only thing is my BMI always been on the low sde 18-19 . I have started HRT 2 weeks ago after consulting with menopause specialist ( the gel and utrogestan tabs) now the rheumatologist want to starts me on the anabolic agent ( teriparatide daily sc injection) I am very nervous and against taking any of these medications. would really appreciate your advice in this matter.

@@Dr_DougLucas interesting viewpoint. My husband has been taking testosterone meds for a few years now. Every year having his PSI tested. During shutdown he could not get testing. After we were allowed back outside and see doctors. His levels were high and after a painful biopsy , he became radiation treatment for a year. He is now on hormone blockers. And getting regular scans to evaluate the cancer. A number of his friends who also were on testosterone had similar problems

Note on the vibration plates---if you have Glaucoma. Beware. I bought a plate, used it once for a few minutes and experienced eye pain for three days. Sadly returned the plate.

I was diagnosed today with osteoporosis at age 57. A complete shock but the Dexa scan doesn't lie! I have cardiomyopathy and did a lipoprotein (a) test recently which showed a very high genetic risk for CVD. I went whole food vegan overnight and have been that way for two months now. I really don't know what to do for the best 😢. All the plant doctors say be vegan for your heart health and you say eat animal products for bone health. Do I die of a heart attack or a hip fracture?? I'm a nurse and have no idea what to do!

That is a tough one. I have lots of cvd, high cholesterol in my family. I used to be vegetarian because of this. It did not help me with my ldl numbers. My hdl and bp are always fantastic due to all my exercise. I havent done that lipid test yet. I do eat way more white meats some red and lots of yogurt now. My understanding is that the saturated fats are also considered a big issue in CVD they are finding now. Maybe Dr Doug will give some feedback on this. My mom had lots of cvd issues and she had smoked for 30 plus years. Then quit when she developed stable angina. She lived to 93 and her heart was not the cause of her death. At 57 she started excercising a lot in every way. She had a low stress life that helped a lot. She took crestor from age 57. No side effects but it only reduces your chances of a heart attack by 25% I think I read. She ate meat but rarely had red meat or desserts. She lived a pretty healthy lifestyle from 57 on that included meats and yogurts etc. Just rare red meat.

Hi there. @LesleyMowat does a great job explaining this but let me chime in too. I’m about to release a new video reviewing the Twins study on Netflix. This actually explains it VERY well. But in short. Animal protein does not increase your risk for heart disease. It never has. For those that have elevation in cholesterol as a result of the dietary fat there is new and compelling evidence that it may not matter as long as metabolic health is good. This explains why heart disease drops for carnivores and keto adapters with high cholesterol. However, if there is additional concern, eat the diet that supports your metabolism and bones and use nutraceuticals and pharmaceuticals to lower cholesterol if desired or required.

It depends on the P1NP! The ration P1NP/(CTX/1000) will yield a number between 50 and 300. Higher is better. We see the average around 100 coming in and >200 for those doing the work.

Thyroid can be improved but not always. Everything we recommend for osteoporosis could be used for osteopenia. It’s the same issue just to a different degree.

They are both very valid but for those with osteoporosis strength training is critical. Cardio is helpful for other longevity metrics but should not be done in place of strength training.

Resistance exercises strengthen bone density similar to how weight training increases muscle growth

I have watched a lot of your videos but this one I really didn't understand! Really wasn't clear to me what CTX is as this is not a term I have heard before!? Also what are P1 and P2? Usually your videos are not overly technical but this one lost me !

They are bone turnover markers (BTM) in the blood that can indicate whether bone building (increase in P1NP) or bone breakdown (increase in CTX) are happening. It can be used to give an indication of where your bone density might be headed when compared to each other and to the individual's results over time. The net result is either increased bone or decreased, which will eventually show up in a DeXA but that takes longer. He has a short description of CTX in the description for this video, and other videos that go into it more. Basically, if you're looking to grow bone you want P1NP to go up and CTX to go down (but as long as P1NP is raising more than CTX, or lowering less than CTX then could still have net gain). This is a bit simplified, but I hope it helps explain it better. There are other BTMs, but those are considered the best to use (for a variety of reasons). The Masterclass and Bone Foundations course he has also go into these things more.

You encourage people to eat animal rather than plant protein, but how do you explain the Okinawan diet? They consume high levels of carbohydrate, low levels of animal protein, and have low levels of osteoporosis

They eat a lot of pork and shellfish it’s not the postwar era anymore.

I agree with @janedough. They eat a lot of animal product actually. In addition observational studies on large groups have numerous variables to consider. They may have had lower rates of osteoporosis in spite of their low meat intake rather than as a result of it.
