Behavioral and mental health can be difficult to navigate. Dr. Domenick Sportelli and MedCircle host Kyle Kittleson kick off this series on how to find the right mental health professional by breaking down a clinical psychologist vs psychiatrist vs doctor. Here's what you need to know about seeking out counseling and other forms of mental health treatment.
Plus he explains why it's beneficial to go beyond a primary care doctor.
In the rest of this series, he explains how to find a therapist for different types of therapy (such as cognitive behavioral therapy or CBT, dialectical behavior therapy or DBT, and other types of treatment for depression, anxiety, borderline personality disorder, and other types of mental illness.)
This video is also incredibly useful for understanding how to become a psychologist.
Thanks for watching our youtube video and for promoting mental health awareness! Watch more videos on psychology, mental health, and relationship management at MedCircle.com.

I'm kinda disappointed that he didn't really get into the differences between a psychiatrist and psychologist. Basically, a psychiatrist is a doctor, and as such, they are trained to solve medical problems by way of medicines and medical treatment. They have a vast knowledge of medications and their desired and or undesired effects and are trained to use their prescription pads to solve mental issues from a medical point of view. Psychologists are very different in that they are NOT medical doctors. They are trained to apply a therapeutic approach to improve mental health, i.e. conversational therapy, etc. They have a vast range of conversational techniques and exercises at their disposal to help bring about an improvement in behavioral or mental health. Which approach a patient requires is a very tricky concept. I work in the mental health sector and have been informed by many of our clients in psychiatric care that haven't noticed much improvement in their mental health, despite the medication given them by their psychiatrist. Many times they are able to achieve a vast improvement after switching to a psychologist. The reverse, however, is just as true. Many of our clients that have seen a therapist for years also report sudden improvements after switching to medical treatment by a psychiatrist. As a general rule however, most of our clients report that the first reaction of their psychiatrists is to issue a prescription.

@@idkwhattowritehere5104 Depend on your problems. Maybe clinical psychologist is better. If the therapy from them won’t work out, then you might want to try to go to a psychiatrist to get medications. Personally I don’t like getting medications for mental health issues. @@lilyblack1979 "Personally I don’t like getting medications for mental health issues." - Well it all depends on the mental health issue, doesn't it? There's an entire range of mental health issues that derive great benefit from prescribed medication, e.g. schizophrenia, certain forms of depression, etc. Many of our clients at work have reported vast improvements in their quality of life since they started taking medication and wouldn't be able to imagine their lives without it. One client with a severe form of depression that led to trembling hands, violent outbursts, sleeplessness, digestive problems, inability to get out of bed in the mornings, etc., was able to resume normal daily activities after a psychiatrist prescribed medication. Before then, he had been seeing a clinical psychologist for several years without any noticeable improvement. Now he's once again employed and the relationship with his wife and kids has vastly improved due to his lack of violent outbursts and unpredictable mood swings. And that's all thanks to medication.

To become a Psychologist in the United States, you need: - A Bachelor's degree = 4 years - A Master's degree = 2 years - A Doctorate in Psychology or Clinical Psychology = 3 years (or more) - A clinical residency of between 1750 and 3500 hours depending on the state = 1-2 years - Then you must pass the EPP exam So before you can reach the point of working independently as a Licensed Psychologist who has earned the protected title of "Psychologist", it will take about 11-12 years. And I would be remiss not to point out how difficult it has become in most states to even secure a clinical residency position to accumulate the hours you need to satisfy state licensure requirements. It's gotten so bad that it's caused a shortage and has made states like New York hand Psychologist level diagnosis and treatment responsibilities down to Licensed Mental Health Counselors.

What Dr. Sportelli fails to mention is that despite the 12 years of training that psychiatrists have, compared with 11 for clinical psychologists and 6 for clinical social workers, there is not one shred of peer reviewed data that patients are better served by any of these professionals in psychotherapy. This is because psychotherapy, and working with people in general, is far more of an art than a science. Some people have what it takes to develop the art. Some don't. And the bulk of all university training is focused on academic knowledge. Not a good way to learn how to work with people especially in clinical settings.

I know she’s the pro but I also know myself better than her or the stupid eval. Nothing more annoying than a doctor trying to inflict the ego him of “trauma” on some of your past when it’s not as traumatic to you as it seems to be to them. The only thing traumatic about my so called “trauma” is that it’s getting in the way of a proper and correct diagnosis. If she couldn’t diagnose me with adhd because I whale anxiety and depression Amd they have “the same symptoms” she should have told me that before I paid $1800 for the evaluation when I informed her I was already diagnosed with anxiety and depression. Sorry but some 32 year old kid doesn’t know more about me than I do just because she went to college. Js

I get that. In all fairness that’s how it will be for many people, and a lot of times it has nothing to do with how a clinician is trained. It has more to do with the ability of the clinician to connect with you and Vice versa. If there’s no connection there the sessions will not work. Plus, in my opinion, many clinicians stick to providing therapy they’re comfortable with whether it works for you or not. They get on my nerves lol

Ok - folks; the most important thing to understand - is how to navigate channels. > You need a referral to get to a psychiatrist - if you are seeking medical solutions.ie prescription drugs. Only a psychiatrist can write out the prescriptions & monitor you for possible side effects, etc. Make sure you’re doing ok on the meds. The psychiatrist is not a counselor for you, to discuss your issues. > A psychologist - social worker - or counselor : is where you can actually talk through your behavioral/ mental/ emotional / or relationship problems. They are the ones who are going to be w. you in the trenches. . > It’s important to find the counselor that Is right for you. How does that work ? You will know it by how you feel when you’re in session with them. You should feel safe, comfortable, cared for & heard. You should feel validated & supported. That’s for starters. It’s possible to get excellent therapy w/o the use of meds. Meds are for people who have true difficulty functioning - but even then it’s a choice, not a guaranteed solution. > Best wishes to all ! PS - as a reminder. None of these venues will magically solve your problems, Those problems will still be there. Pills or no pills, counseling or no counseling. Just so you know. ...

I will elaborate on the work of a psychologist since it was not addressed during the video. To become a clinical psychologist one must complete a bachelor's degree in mental health (4 years), a master's degree in mental health (3 years) or go directly to a doctorate degree in psychology which can be a PsyD or PhD (4 - 6 years) where we must also do a doctoral dissertation/research (1 - 2 years) and a year of internship in addition to clinical rotations in hospitals and outpatient clinics; and if we want to specialize we must do an additional year or two depending on the specialty (neuropsychology, forensic psychology, trauma). In total, a psychologist studies between 10 to 13 years of mental health compared to a psychiatrist who studies 4 years of mental health during their residency. Both psychologists and psychiatrists can diagnose and treat mental health illnesses. The psychiatrist bases their treatment on medication unless he/she/they is trained in some model of psychotherapy; on the other hand, the treatment offered by clinical psychologists is in psychotherapy unless he/she/they specializes in prescribing medication (in some states such as New Mexico). The literature shows that the best treatment is a combination of psychotherapy and medication for most mental illnesses, so the collaborative work between a psychiatrist and a psychologist is essential in the recovery process for the patient. In addition, psychologists are trained to assess mental illness from a social, biological, environmental, behavioral and emotional aspect. In turn, they also offer evaluations through standardized tests to screen for mental illness (emotional, personality, neurodevelopmental), intelligence level, neurological disorders, among others. This is only a small part of the work that a clinical psychologist does. As a Doctor in Clinical Psychology, it seems to me that Dr. Sportelli has a reductionist view of what a psychologist is.

As a psychologist in Colombia, I´d say if you require medication to manage your mental health, you go to a psychiatrist. BUT, in most cases, this treatment should be accompanied by treatment with a psychologist. Psychiatrists don´t do psychological therapy, just medicinal "therapy". I would say it´s best to start with a psychologist, then that profesional can remit you to a psychiatrist if needed. If a person isn´t stable enough, a psychologist can´t start therapy bc the person just wouldn´t be able to respond, so medication is needed to level the person out first so that they can be present in the therapy.

While medications play a role, there's clear evidence that therapy has huge benefits for many disorders, and even those without a disorder. For those reasons, psychiatrists understand that therapy can be quite helpful, and because of that, many of us psychiatrist learn, understand and know how to employ psychotherapy. A lot of insurance companies require you to go to your primary care physician and get a referral before they will cover any of the very few behavioral health treatments or appointments they actually allow. That is very important, because it is very expensive. Kind of like going to your dentist because you have a toothache. You have to pay the dentist's exam fees, x-ray fees, and who knows what else they want add on to the bill. The dentist tells you you have an abscess and need a root canal. Insurance only covers the exam fee once every 6 months for cleaning purposes. So now you have to wait 6 months or pay another exam fee, x-ray fees and who knows whatever else they want attach on to the bill for the endodontist to examine the tooth again. Then once you pay the endodontist to do your root canal, you have to go back to the dentist and pay exam fees, x-ray fees, and whatever else they want attached on to the bill to get the crown put on and hope that fixs the problem for a few years with three times the dental hygiene practices to keep the infection at Bay.

I am a Doctor of Clinical Psychology. I am very clear about stating that and not just saying the word “Psychologist”, so that people don’t confuse the title of therapist with the the title of “Psychologist”. A Psychologist is a person who acquired a Bachelors, Masters and Doctorate in Psychology =10 years. Prior to obtaining Doctorate, they must complete Pre Doctoral hours, along with a Doctoral researched based dissertation. After completing Doctorate degree, they are then required to complete 1-2 year post Doc hour. Psychologists are trained experts in understanding the mind and behaviors, diagnosing, assessing, treat mental health issues, behavioral issues, hardships and administer testing and assessments. Psychologists can also be therapists or Psychoanalysts. A Psychiatrist is a medical doctor, who went to medical school and didn’t learn anything about the mind, mental health nor behavior. They strictly learn about the anatomy, everything below the mind. Psychology was birthed from Psychiatry in a way, which is the reason the names are similar. Psychiatrist, Psychologist and or Psychoanalyst. Psychoanalyst require an additional training. The first psychiatrist were medical doctors, who decided to flirt with the brain and behavior. From that birthed Psychoanalysis and then Psychology. The first psychological experiment was conducted in Egypt, making Ancient Egypt the birth place. Today’s Psychiatrist for whatever reason has decided they don’t want to work in medicine and go for a specialty in mental health to justify the decision. They complete residency in their specialty, which means training in mental health. They do no focus on mental health in the same way a Psychologist does. They pretty must just prescribe medication for psychosis, depression, anxiety and attention deficit. They are not concerned with the mind, layers and personality organization of the person. They dont care about your childhood, divorce, insecurities, hardships or splitting. They only focus on what they can prescribe for whatever issues and justify it is a means to resolving mental health issues. It is not. Some Psychiatrist become Psychoanalyst, which is a true form of treatment of psychological issues and or hardships. A therapist focused on talk therapy. They receive a bachelors and masters degree. They can diagnose but mostly focus on talking with a person to help them resolve personal issues. A counselor counsels and coaches a person experiencing hardships in life and in need of counsel. I think psychologists and psychiatrists should be categorized into different specialties, like adhd psychiatrist for children (adhd pediologist), psychiatrist for bipolar disorders, and given a specific title, like bipolarologisr, lol,, just like you would see a foot doctor for plantar ficiatis and foot pain and and they are categorized as podiatrists. Patients should be given a psychological test in order to see the correct psychiatrist/psychologist to treat their mental illness.

They do. Clinical psychologists are the only ones who can assess and diagnose people with autism. Furthermore if there is more of a chemical imbalance neural wise then the psychologist more than likely would refer him to a psychiatrist. They don’t need “adhd psychiatrist” and so forth. You go to school for all these years and do internship or residency to learn about these disorders. There are such things as a child psychologist and psychiatrist which is in scope of what you are saying.

It's often in fact vice versa but can depend on the severity of the disorder. Licensed psychologists are as (if not more) capable and educated to diagnose mental health disorders as a psychiatrist, whose first course of action is almost invariably to apply medicinal therapy. That's what they're trained to do--treating disorders with medication over therapy. After obtaining a diagnosis from a psychologist, it'll be established whether medication is necessary, in which case they refer the patient to a psychiatrist. They work hand-in-hand so one should never be dismissed over the other.

@@e.b.4379 Thank your input on this. Psychiatrists aren't trained to use one treatment over another. We are trained to diagnose first and foremost. Understanding a diagnosis is crucial. And to understand a diagnosis you need to be familiar with a broad range of disorders and formulate an appropriate differential. This often times can include medical disorders that can contribute to mental health disorders, such as thyroid dysfunction for example. Psychiatrists then have to know when to use a certain treatment and when not to employ one. We look at risk-benefit ratio, burden of proof of the disorder in question, and a good psychiatrist will collaborate with the patient on a treatment of their choice. Many psychiatrists, myself included, are trained in both psychotherapy and medication management. And as a result, because we intimately know both types of treatments, we have a better understanding of knowing when to use one over the other. While there are, unfortunately, poor psychiatrists out there like any other field, I wanted to instill some hope that there are actually psychiatrists who do much more than just prescribe. Thanks for your input though!

A psychiatrist is a medical specialist who treats mental illness that is categorised as a medical disorder and will usually require medication that a psychiatrist can prescribe. Psychologists, on the other hand, are experts in human behaviour and related to that how the mind and cognition regulates behaviour and feelings. Treatment is based on this, and psychologists cannot prescribe medication. Therefore Psychologists will treat problems like phobias, bad decision making, self esteem, addictions, and psychiatrists treat schizophrenia, bipolar disorder, major depression, and mental illness involving psychosis which will usually require medication for the patient to be able to function.

Whats the difference between a psychologist and a therapist then if you use this definition? That doesn't make sense to me. I am currently a psychology student and there are specific courses for each - Psychologist BSc and Therapeutic Psychology BA - my understanding the is the psychologist is based upon a more scientific structure hence the B(achelor) of Sc(ience) at the end and Therapeutic psychology is a B(achelor) of A(rts) based area. Also I was told that a psychologist works with people with specific mental disorders such as borderline personality disorder and schizophrenia where a therapist deals with more generalised issues such as depression, eating disorders and grief. Both share use of therapy but not both are "therapists" is how I best understand it.

There is a bias here without having a Psychologist speak as well, and he doesn't cover the benefit to seeing a Psychologist vs. Psychiatrist. (Yes, I'm a licensed Psychologist.). A Psychologist has way more in-depth psychotherapy training (in a doctoral program, which took me 8 years to complete after a 2 year Master's program) -- this includes family/couples therapy, child psychology, assessment & evaluation (testing, which Psychiatrists are not trained specifically to do). Yes, a Psychiatrist has a medical background which certainly helps with medication evaluation and so on, but they have very limited "training" in knowing how to do in-depth psychotherapy -- what you heard him say here is that there training in therapy is limited to a one-two year residency/specialty, which primarily includes seeing patients under supervision, not a lot of in-classroom training. This is a huge difference.

First time in my life I felt... Normal. Ofc it's not going to be the answer to everyone but for my specific case, it's what I needed. And those drugs have literally saved my life. The disadvantages are heavily outweighed by the advantages. I reached a critical point in my life where I either take the pills, endure the side effects or I was going to kill myself. Already planned it out and bought all the things I needed. It was that severe.

Many doctors today prefer the patient visit a psychologist instead of taking medication, sometimes it can help but talking to a psychologist about the obvious can be a pain in the backside, anxiety should be treated with medication and some visits to a psychologist, what many do wrong is, banish the medication completely and use the new age response of going to psychologist as the preferred way of treatment, it works both ways and the psychologists should acknowledge this and stop having a negative attitude towards pharmacological treatment.

Or the psychologists who are wedded to a single therapeutic modality because they chose psychology because of their own issues? I'm a primary care doc and fully trained psychotherapist who's appalled at the quality of care from so many mental health professionals. Fundamentally, it's an expensive process of 'auditioning' providers because the therapeutic alliance is everything. Hopefully a primary care doc can help you sort out the dross. The best psychiatrists do not just prescribe medications. The field of psychiatry continues to be divided into fragmented areas of “specialization.” It may be difficult to clarify who does what. You may ask: What’s a MD, DO, LMFT, LCSW, PMHNP, PhD, PsyD, etc.? Who does therapy? Who doesn’t do therapy? What do all these terms mean? Why should I care? The fact is that all psychiatrists are physicians (MD and DO). Many psychiatrists learn, understand and know how to employ appropriate therapeutic techniques and specific psychotherapy modes. Because psychiatrists (MD or DO) understand how to employ the broadest range of treatments of all the different types of “therapists” available, there is a clear advantage to having your therapist be a psychiatrist. While not all psychiatrists practice therapy, if you find one that does, it would benefit you to consider continual engagement with that psychiatrist. The best psychiatrist doesn’t just prescribe. They understand diagnosis, the wide range of treatments (therapy and medications) and know when to recommend one, the other or both.

Not entirely accurate. Psychologists do about 4 years of doctoral training (often 5 years) + at least 1 year of postdoctoral residency/fellowship, and are trained in neuropsychology, assessments, and therapies. There are many specialties within psychology as well, and board certifications which take further years of practice and study. In my experience - and I have worked extensively with psychiatrists in multiple settings - psychiatrists prescribe meds, but have very little knowledge of behavioral health or therapy. It is also noteworthy that in 5 states, Public Health Service, and the DoD , psychologists can earn prescribing psychotropic medications with additional training.

Very misleading information-psychologists being able to prescribe is ONLY permitted under physician (psychiatrist) supervision in the extremely limited scenarios where it is permitted, and when it is, it is due to physicians not wanting to practice much in that state, but furthermore, this does not discredit the reality that a psychologist does not spend a single day in medical school, and does not spend a second studying medicine. Therefore the most important distinction to be made is that a psychologist is not a physician, not a medical doctor, and not fundamentally an expert in human pathology, physiology, or pharmacology in the way that a psychiatrist (physician) is. A psychologist is an expert in their DISTINCT field, which is philosophical, not medical. Therefore the length of time that a psychologist spends studying, whether 5 years or 10, does not indicate that their training is the same as that of a psychiatrist who studies for 10 years because the nature of their training and education is ENITRELY DISTINCT. If I spend ten years studying carpentry, I'm not qualified to be a cardiologist just because we have both studied for 10 years, because we haven't been studying the same thing. Psychiatrists have little knowledge of behavioral health or therapy? Psychologists have no knowledge of human processes or bodily functions because they are not medical doctors. Your comment aims to present psychiatrists as psychologists with special prescribing abilities, but this uneducated oversight completely dismisses the entirely separate education and training associated with the two different professions. Nowadays many psychiatrists refer to themselves explicitly as physician psychiatrists because of this misconception.

So true everyone is trying to be a mental health professional they need to stop. Many people who are calling people crazy is mentally ill and suffering ,emotional and personality disorders severely themselves. You need to go across the country and promote and discourage the importance of non professionals trying to diagnosis mental health. Some of these people are not even graduates of any college thier degrees are in business management, education, some work in law in lay position or law enforcement which only requires a GED or Highschool education and training. I heard a dumpster truck driver calling a woman crazy I'm like this is extremely dangerous. It's slander too I always seek mental health professionals during the times of crisis everyone of us will go through. Such as divorce, custody of the best for the children in divorce,a loved one dying any crisis losing a job it's important to seek help. So it's equally important to discourage the abuse of mental health if you are not a mental health professional shut up. Because new norms are abusing the mental health field it's insane.I don't want to relate to certain mentalities and that doesn't make me crazy because I'm not adopting to your toxic unhealthy view of a certain culture, political view or religious belief. I'm a sound minded individual that knows healthy individuals are capable of thinking independently and differently no matter what there culture, background and life circumstances are. Your brain and my brain are two different brains so focus on your brain health. I'm doing a excellent job on mine dealing with circumstances that many would have really surcome to a mental health break and it has been proven all Ive been through with the assistance of seeking professional Psychiatrist and psychologist. I've never had a mental break ever every psychologist, psychiatrist or therapist I've sought out has never diagnosed me with absolutely any mental health issues. So the only one who is crazy is the ones who continue saying some one who has been medically proven by multiple Doctors licensed and have the Degrees and practices within the State to legally determine a mental health diagnosis in anyone. Who Keeps saying some one Who has on different occasions been cleared on all and any levels of having no mental health issues at all. It's like being negative on an HIV test and some one says you have HIV or Not diagnosed with breast cancer but some crazy person who wants you to be keeps saying you have these diseases.Or some nut saying you're addicted to opioids because you take Advil pm over the counter and you're medical records clearly y shows you are allergic to opioids and have no history of being prescribed anything but Ibuprofen for pain. This individual is crazy Promotion of seeking professional Psychiatrist and psychologist who are competent enough to confidentially diagnose and treat thier patients mental health needs are the only ones qualified to discuss mental health in anyone. You want to be a Psychiatrist go work to be one other wise keep picking up trash off the side of the road. Most people who do this are projecting thier own personal battle with mental health and have set Infront of the same people I have a professional Psychiatrist Psychologist or therapist in the mental health field and they was diagnosed. I have a healthy brain and sound mind I know this because it's been Psychiatrist and psychologist approved by board certified Doctors who are licensed professionals within mental health medical field in my State. Keep serving people your work is needed a sound mind is detrimental to live a independent healthy life . My brain is sane certified who gives one care of what a manic has to say go take your meds please to ensure you don't become a threat to someone

if you are seeing a "psychologist" what is the end goal? Continuous therapy sounds like a never ending cycle and while its helpful to discuss and evaluate your status, I don't see what the purpose is if its just endless meetings. Why do these folks not give the tools to manage your mental health? If you are NOT armed with those skills sets it seems you are dependent upon what ever care giver you go to.....

The end goal is to get you treated and for you to be able to be in reemission. I’ve had a psychologist before and with good therapy I no longer need him unless I fall off track. I dont know anyone who has seen the same therapist for years over the same problem. Sounds more of a “you issue”

Therapy is not supposed to be permanent as a psychologist myself I usually have a treatment plan for my clients and after they have the tools to manage their issues on their own I let them go . Also if my client feels like they are good on their own and I can also see that they are I also let them go

Patients have to be willing to make changes on their own. Sometimes it is important for patients and clients to have reasonable goals. I’m three months in therapy after battling with GAD for 2 years and it’s really working. It’s almost like realizing there is one big knot with a bunch of little knots(issues) and little by little, with each goal being completed something gets unknotted and you’re willing to do it on your own. But as you keep talking about deeper issues you start to realize there are more underlying issues that are uprooted as you self reflect you are able to make some changes in your day to day life. I was not able to see these smaller issues because I was way too focus on the bigger issue and I wouldn’t have seen that unless my psychologist pointed it out. Sometimes you need an unbiased third opinion without someone you know to see behind that brain fog. Other people just need to be heard and need someone to talk too.

There's no clear delineation as the field overlaps. One prominent difference though is that of prescribing medication. Psychiatrist can prescribe, but Psychologist can't. But even that is blurring as Psychologists can also take now additional education/training to be certified to administer medications. Every country too has their differences they may even cover fields where they are not specialized or trained so its really good rule-of-thumb to check their background for certifications, achievements, client feedbacks, etc. With that, you'll have to settle with these tip toe differences to define them. 🙂

Addtional, but less prominent difference is that Psychiatry is usually in hospital settings & Psychologists more academic/private. 🙂
