I laughed when I liked this video. I’ve been having chronic pudendal neuralgia for 10 years. This is a great description. I had mild issues after giving birth, but 10 years ago I had a vulvar tumor removed and have been in pain since. Sigh. Had a nerve block and trigger point injections a few weeks ago with mild results. I have in last had better results. Just tried low dose naltrexone. Was taking at the same time as amatryptaline and one was making me crazy so I stopped both. I definitely need a good muscle relaxer. Currently taking metaxalone. I have tried vaginal Valium but will try again. I’d like to try your suggestions. Makes a lot of sense. Now to get a dr to agree. 😁 maybe I’ll send them this video. I’m sure they’d love that. 😊

When explaining what PD is, I believe it is imperative to understand the Men can and do suffer from PD as well. There is a lot of embarrassment that goes all long with the condition. 3/4 of the battle is getting the proper diagnosis. How many time are you prepared to drop your trousers before getting the correct diagnosis from a doctor who actually cares enough to help?

That is true. Do you mean PN?

Very true. You have to be willing to travel to see someone, typically.

Best explanation I've ever heard. Thankd! An Interventional Radiologist is offering pudendal nerve cryoablation to treat my severe left scrotal pain. I've had two 100% successful nerve blocks done at the ischial spine. He wants to do a block at the Alcock's Canal, then treat there. But that's distal to my previous blocks AND to the most common entrapment location for that nerve - the spot where the sacrospinus and sacrotuberous ligaments cross. Apparently all ablation types for treating pudendal neuralgia get done this way. This doesn't make sense to me, and I can't talk to this doctor until the day I fly there for the block. What am I missing? I'm an Electrical Engineer who spent my career in Silicon Valley developing novel medical devices, including implantable defibrillators and RF generators for performing ablation procedures - which is what i was expecting to be offerred. BUT - if that block doesn't help, wouldn't that verify my nerve issue is proximal to the canal and is likely trapped between the ligaments? Pain started during a six month time period where I had to use a knee cart for a right leg injury. I tried the DRG stimulator, and have had my genitofemoral nerve and the perineal branch of my left pudendal nerve cut.

Great information, and well presented! You mentioned that there are various kinds of blocks, and I know that, in theory, only more distal parts of the nerve are numbed by a block. I also know that even the blocks at the ischial spine aren't easy to do. Ive had 4 and 2 missed. Knowing the various locations on the nerve where blocks can be done would REALLY help PN patients to isolate exactly where the nerve is being irritated. Knowing that would be of tremendous help in deciding on treatment options - especially for surgical approach selection for decompression surgery. Can you please list the different potential locations for blocks and what each block is called, so we know what to ask for?

Ive fractured my coccyx twicd. In the last few years ive had a pinching feeling in the perineum. I first noticed it on a stationary bike, then it started when sitting. Now, it's ever present. Its not debilitating, put a pinching feeling thats very distracting to the point of frustration.

Hi. Can prp work for pudendal neuralgia and/or cauda equina? If so, what location would you recommend the injections? Ive seen several doctors and no one is really helping me. I had a fall and aggravation during PT which led to feet/ankle numbness and urine retention. It progressed to levator ani spasms. Pudendal neuralgia...so a year later and it still hurts to sit (hamstrings & glute pain & tightness and anal/vaginal pain) . I still get deep pelvic floor spasms. Im in daily pain. Im trying to go back to work so i wanna get away from sedative medications (they dont fully help anyway)....any advice please

Another cause for Pudendal neuralgia is when you almost ejaculate, then stop before it releases. As a kid I did that when I first started...you know. But my anxiety made me stop it before it came because I was feeling way too anxious

Thrush, ie yeast infection can cause vaginal and vulvar burning. Hopefully your doctor gave you a prescription to treat your yeast infection.

Cycling can cause pudendal neuralgia. I suggest seeing an expert and having a nerve block to the pudendal nerve at the ischial spine.

There are none of these treatments in the UK. The UK health service is still in the dark ages.
