I enjoy how you talked about structuring each treatment session towards each functional goal and found that helpful. I create goals from the outcome measure if the patient was not specific enough about what they wanted out of their therapy experience. I do think you have a very subjective way of creating your short and long term goals which I disagree with and am not sure how Medicare would justify them for reimbursement in that regard. Say if the patient came in to therapy after just having a knee surgery, I would rather use the LEFS to make a short term goal for the patient to walk the distance of 2 blocks in 6 weeks with stand by assistance to get to their mailbox and back and then walk 8 blocks in 12 weeks independently for grocery shopping as the long term goal. Walking can be measured with a treadmill and stop watch for speed and time in the clinic. All therapy goals must have: a time period, a task, task specifics, be measurable, the assistance level provided, and a functional significance.

Hi. Great points. I like the idea of using min mod max difficulty when describing pts level. So Medicare doesn’t consider that too subjective (as opposed to using min/mod/max A)? This would really help with my Ind yet struggling geriatric caseload POC.
