Always depends on the Mall. A mall where I grew up removed the windows of the second floor walkways to hoist them up. On the first floor you can remove the multiple entrance doors to make a big entrance. There are also fairly big side entrances as dedicated escape routes which can be used. If it's a big mall they'll also have truck bays for restocking.
