Dr. Ebraheim’s educational animated video Part 2 educational video describing the condition known as radial nerve palsy.
Radial nerve palsy results from acute trauma or compression of the radial nerve. The radial nerve originates from the posterior cord of the brachial plexus.
Injury to the radial nerve affects the functions of the wrist and fingers differently depending on the area of injury.
The radial nerve supplies the extensor muscles allowing for extension of the wrist and fingers. Injury to the nerve will cause wrist drop.
High radial nerve injuries usually occurs due to fractures of the distal third of the humeral shaft. The fracture may cause injury to the radial nerve causing paralysis of the wrist and finger extensors. The patient will be unable to perform the “hitch hike “ sign.
Low radial nerve injury usually occurs around the elbow, affecting finger and thumb extension.
A high lesion above the brachioradialis nerve causes wrist drop and loss of finger extension.
In patients with radial nerve palsy, recovery of wrist and finger extension is estimated to be about 1 mm per day. The brachioradialis muscle is the first to recover.
Treatment
•Explore in open humeral fractures
•Observation in closed fracture
•Order EMG and nerve studies 3 weeks after the injury
•The brachioradialis muscle is the first muscle to recover
•Repeat EMG if necessary.
•Radial nerve recovery occurs in about 85% of the cases in about 4-5 months.
•Explore the nerve if there is no recovery
•Tendon transfer may be needed if there are no signs of radial nerve recovery.
•Pronator teres to extensor carpi radialis brevis is a basic transfer
•Splint the wrist in cock up position and order physiotherapy.
The nascent (just born ) polyphasic motor unit activity is the early evidence of nerve regeneration.
Bad prognosis: small polyphasic motor unit 6 months after injury.
Good prognosis: no P wave, no fibrillation in 3 weeks after injury.
Example of a patient recovering completely from radial nerve injury. With improvement od the injury, the patient will also be able to perform the “hitch hike” sign.
Example of a patient partially recovering from radial nerve injury. Recovery of the wrist extensors, but finger extensors are not yet recovered.
Patient example- the radial nerve is not yet recovered but appears recovers. The patient appears to have finger extension. This is an inaccurate examination.
With the wrist placed in extension, the patient cannot extend the fingers = the radial nerve did not recover.

I recovered in 5-6 months ,, got muscle stimulation machine at home and do physiotherapy daily,, even started going to gym by holding the wrist with wrist holder and pumping the muscles with light weight dumbles , it helped a lot ,, now i am perfectly fine. Just don’t loose hope,, it feels like that it’s never gonna be ok but it will be good after some time. Keep in mind the recovery process is very slow,, but it do recover.

Whoever is reading this,trust me when I say it will get better. I met with an accident 4 months ago and it was bad open fracture I couldn’t raise my thumb or my wrist. The doctors said that my radial nerve was probably stretched and it would take up-to a year to get my functionality back. I was doing my physio every day and after 2 months I could raise my thumb and within 2 weeks I was able to raise my wrist. I remember watching this video after the accident and thinking if I will ever get it back but I did so don’t loose your hope

hi guys im feel so sad when i have fracture in my forearm and i cannot able to move my hands and fingers, but after reading all your comments and improvements i feel much better bec. my hands is started recovering after 3 weeks of my operation.. and im glad to see this video.. ty

guh. I fractured my humerus bone pretty badly in November. Had surgery two weeks ago and now I suffer from this. Nice to read these comments and see that you guys are recovering well. I hope I'm equally as fortunate. The thought of never being able to type with two hands or use a controller is horrifying. I can lift my wrist and fingers partially, so that's something I guess. I'd say the movement of the thumb is the hardest to control. There is also an annoying prickly sensation on the side of my thumb. I guess I should just be happy that I can actually FEEL my thumb. Great video. Gave me some much needed hope.

I figured I might as well make an update to this post since it's been two years. My hand pretty much recovered 99% There's still an odd, faint sensation across the back of my thumb, but I mostly don't even notice it throughout the day. I'd say I've regained full control and strength in my hand.

I have a question for you:) after 4 months of radial nerve palsy I finally have basically all my movement back but my hand is still pretty weak is this normal? Did you experience weakness for a while until it regained strength?

I met with an accident on 30th July 2023, broke my 3rd distal humeral shaft right above elbow. Had to go through surgery and doctor added plate on it. Have been suffering from Radial Nerve Palsy with complete wrist and fingers down. It's been 19 days to my surgery. Today is 20th August 2023. I don't know when it will recover but I have firm belief in almighty Allah that it will return in sha Allah. After reading all your comments, my hopes are high and I will definitely keep others posted too on my recovery in sha Allah.

Day 60 post surgery. Better elbow flexion but still the brachial muscle has not fully healed to allow full flexion. Wrist still dropped, fingers still not working. However with the wrist splint, the grip is better.

Hi, I damaged my radial nerve in january and had 100% recovery by May/June. I have been lifting weights regularly for the past 2 months with no problems. I thought at the time it would be like that for life and was so depressed, but it really does just heal extremely slowly. If you have noticed some sign of recovery 90% of the time it will heal completley as mine has. So hang in there and it will recover, also my finger extension took longer to recover than my wrist.

Hi there Aksh. I didn't have any treatment at all because I live on a small island with basic health care. So I just waited for it to get better and tried to get on with things as best I could. It's possible that it would have got better sooner with electric stimulation. The doctors told me it would take 12-18months for full healing (from the time of the break to full recovery) and I am on track for that. Keep positive, it will recover. Helen

Hi Aksh and others, just to let you know that now, two years after my original humerus fracture and radial nerve palsy, I am just about back to normal. Slight problem not being able to point with my index finger very well and occasionally I get a cramp in my hand, if I overuse it. Otherwise I mostly forget that I ever had six months without use of my right hand at all. So keep your spirits up, it DOES improve.

Hi, I didn't see any movement in my wrist or fingers for six months. I went for an EMG after 5 & a half months.They told me there was no reactivation in the radial nerve.10 days later i had my first sign of movement. It started with a very slight movement in my wrist.Over the next few weeks&months, my wrist & finger movements came back.I still don't quite have full finger extension (now I am one year since my accident),but it is hardly noticeable.Be patient,it is frustrating but it will recover!

It's very different depending on how the nerve in damaged, but since yours has started to come back, you can almost certainly count on full recovery within 4-5 months like the presenter of the video has stated. To compare, I have almost all sensation back yet motor function is only now starting to return. All I can say is get a good brace and try to forget about it, it will return in time.

now i can officialy give a thumb's up.. I'm 85-90% healed i cannot extend my index fully when i lift my wrist but i can perfom almost everything like before.Type , write , eat Damaged on Dec 27th 2012 now today is 9th of august.I started to see signs of recovery around middle of april when i could lift my wrist just a little and finger recovery signs around middle of june.Hang in there guys.. We're all gonna make it somehow :)

UPDATE: since 29th Dec 2012 till today 29th July 2013, its been exact 7 months after Humerus fracture and radial nerve palsy and now I m almost 90% recovered. still cant extend my index finger fully but it always heal at the end. 1st sign of extension was in April and since its improving day be day. I can do almost everything normally now like right now typing :) and for those who are new comers of radial nerve palsy :) hang in there, it's irritating of course but its gonna be OK. It will heal

I’m suffering this exact injury following a humerus fracture and op to fit a plate. Currently 8 weeks in with no change at all in movement of the wrist or fingers. Have been getting less hopeful that I’ll start to see any improvement but it’s positive to see the comments on here with recovery, even if it takes months!

​@@aryanthapa9165 Yes, not fully recovered yet but I am at 80% as of now. Recently got EMG and got encouraging results. When I posted my last comment, I could not extend wrist or fingers even by a centimeter. Now I can fully extend my wrist and fingers are getting better and better slowly. I had very severe damage, though. My radial nerve was cut in the upper arm and had to do surgery for nerve reconstruction a year and three months ago (two months after the damage). It will get better, do not worry. But absolutely make sure to exercise damaged hand.

@@thomasrios3905 The first time I noticed movement in my wirst was after 5 months from nerve reconstruction. Ever since it has been getting better slowly. How are you doing now? I bought nerve stimulation device and still using it till this day. Also, I always tried to use my damaged hand in everything possible. I think that's big part of the reason why it has been healing well.

Status update, I injured my radial nerve April 13, it is now July 12 so 3 months almost to the day. Within the last month there has been a lot of recovery of my hand functioning. At about 7 weeks I was able to hold my hand out in a tight fist only horizontal. At 9 weeks I was able to hold my hand horizontal with my fingers extended but I ha to try very hard. Now at 12 weeks I am at about 90% full range of motion in my hand I can open and close my hand, lift my wrist with my fingers extended but

i got this from falling asleep with my arm pressed against the side of a arm chair, thank you for demonstrating the signs of recovery, i can slighty lift my wrist and extend my fingers now but that isnt true signs of recovery. thank you so much

@@affa8757 mine recovered in 10 weeks for mobility. strength took another 2 weeks or so of exercise ( using the hammer or just the bar of a dumbell. ) to get back to pre-injury levels

I wore my spint during the day all day because I couldn't do anything with my wrist flopping about all over the place. I was told to wear it at night but to be honest I found it uncomfortable so I didn't do that very often.

wow...I m so happy...my wrist finally now starting to extend a lil bit..it's been 3 and a half month after my surgery(humerus fracture) and I got first sign of extension......all those who r currently going through tough time...I can only say hang in there, it will come back......

Gurunath Hari Very educational. Great job Dr. Ebraheim. Thank you. I trekked a set of mountains on 15 Feb 2020. Up and down 14 hours. Used a stout stick for assistance. After a day forearms sunburnt. After week and a half since 26 Feb I suspect I have mild to severe right radial palsy and milder left radial palsy. I have mold mild localized pain below the right elbow dorsally. My question: is that how long it takes for symptoms to manifest?

I'm so scared. It's been a week today since I had my injury. I'm getting a little better but I need full recovery since I have an office job and I'm required to type. Hopefully the doctors and Rehabilitation will get me back before pre-injury!

Now can pinch grab stuff but ..full extension of fingers but cant lift themif positioned lying down-my wrist still dropped. What was ur recovery process like? What improvement followed what? Hey Jennifer. Okay, so Ive been completely healed for about a month now. I believe it took around 18-22wks to get total dexterity back but, its true: even if you regain a little mobility, then 90% of the time it heals without issue. The trouble I had was differenciating between whether or not I was actually healing, or just becoming better at adapting to the condition. After a while it became obvious but, for a little bit there I couldnt tell. Anyway, I completely healed in around 4-5months.

Hi Helen....I fell on ice in Poland and hit my hand at the edge of path....it was awful and I didn't damage my radial after fall it was actually after operation as Doctor had to lift my radial up to install 2 plates with 11 pins :) and after operation when I woke up next morning; I realized that I couldn't extend my wrist anymore. I definitely remember when I reached hospital they asked me to move wrist and fingers and I could actually do normally BUT.....

im not a professional, but ive had this condition before, it took about 3 months to recover, now i got it again the same way about 10 days ago. if you are getting some movement back i guess that is a sign of something happening. i remeber the first time i didnt have any movement until after about 2 months. (slept on it approx. 4 hours) this time it was about 1.5 hours. its been 10 days and i can type without a splint, but i cannot lift a glass of water (the muscles arent working yet)

So, I got this about two weeks ago. I was getting over a flu and just passd out in a weird position in bed...hard to believe! I like to play guitar a lot so the first week was so frustrating but it is getting better now. About the third day I tried to pick up a cup of coffee in my boss' office and spilled it all over her carpet! I forgot my hand was dead. It was so pathetic I couldn't help but laugh. Speedy recovery to all! It has been 15 days for me and I feel in a week I will be 100%

I'm a week into this. Man is pretty rough. Fell asleep at the computer. It is nuts you can do this just by sleeping on it weird. I feel like it's made a tiny bit of improvement but that's being measured in mm. I hope everyone gets better this is a drag. I'm picturing myself 6 months from now still dealing with it pretty crazy. At least I've got my right hand still. Idk if i should go to the neurologist i went to the e.r and it still hasn't been a week. I'll let you guys know the 411 take care.

So I got radial nerve palsy just from compression and it's been about 4 months. I can finally move everything basically normally and no longer using my brace but what I'm wondering is if anyone else experienced weakness at first? My hand is still really weak, did that go away for any of you?

hey bro.. i went yesterday to the doctor and he said that im heading towards healing.. my bone was placed at it's best with the help of the plate but the nerv should heal around april .. i asked him abt EMG but he said those are not that accurate and he told me to stay positive to keep doing my kinetoteraphy exercises and start again taking Vitamin b pills(Milgamma).I hope im heading in the right direction:)

But not all the way up, ican now pick things up with my hand but the muscles in my arm are still weak so I'd say within the next month at the rate I'm healing I should be at 100%. To anyone experiencing this just keep your head up try not to get depressed about it, the healing is slow but it really picks up between 8-12 weeks.

Had bicep tendon repair and after 2-1/2 yrs still recovering, had tendon transfer surgery Jan 18, 2019 finally got my cast off yesterday and feel this is gonna be a long road ahead of me..my hand is stuck in an open position now..working on being able to close and reopen🤞

I had fractured humerus in Feb, radial nerve palsy for six months. just showing some signs of movement in last two wseeks. These videos were enormously helpful in giving me hard-to-find-info about radial nerve palsy. can anyone tell me how long it took them from first sign of wrist movement, to full finger extension? thanks.

I damaged my humerus by a break 11 weeks ago. I had surgery 10 weeks ago to fix it and my surgeon said it was intact when he went in to do it. Is it normal to not see progress yet at this point?

Thanks for sharing your story. I've not been operated on as the consultants wanted to see how it got on healing by itself. Surgery is still a possibility, I will have to see.

Hi Mike.... How r u doing? I'd also humerus fracture on 29th Dec 2012 and got operated with a plate and 11 pins...good part that now i can extend my elbow 95% and close around 85%....but still with out extension of wrist...I m gonna have EMG test on 12th March 2013 as I don't see any difference with my wrist and fingers.....have u got some extension with wrist and fingers yet????

My finger extension has recovered first but my wrist is still weak. I can only hold about 3kgs before it "drops" its only been 7 days since i did it. Hopefully 1 more week and it will be 100%. Its such a strange injury!! NO pain what so ever.

Hi guys, just wanna share my experience. I had this last year and suffered for about 4 to 6 months. I fractured my right humerus, went to surgery and got a gift called radial nerve palsy from my doctor. I went to rehab for about 2 months, stopped coz i think there is nothing they can do with my nerve damage. saw minor recovery during my 3rd or 4th month as I can somehow raise my wrist for about an inch i guess. Then follows my pinkie, ring... and lastly my thumb.

Hi Aksh, I am doing great now so just hang in there. I still have funny pins and needles and small cramps in my hand, but most of the time I hardly notice them because I can use my hand and wrist almost as normal. I got my wrist extension back well before my fingers, so just keep patient. I kept manually flexing my fingers whenever I could to keep them moving. My thumb was always the last to do anything and it still feels wierd now!How did you damage the radial nerve?

Okay, so I fell asleep between my table and laptop one night pinning my left arm between the two. Woke up, and had radial nerve palsy...this was 3mon ago give or take. Ive heard that any healing at all, means that 90% of the time it will heal. However, Im not sure if it is in fact actually healing, or if I am just becoming accustomed to it, and learning to live with it. Truth told: Im a little scared because its been a few months...any suggestions?

If you feel alot of tingling does that mean you are starting to recover? No extension of wrist or fingers and it's been almost 12 weeks but do feel numbness, tingling and burning at times

Rachel Pruneau I'm 19 weeks in and barely started to have some movement a few days ago. I haven't had those symptoms, are you going to occupational therapy? Its been helping alot, highly recommend it. Good luck and stay positive

Same here, I'm waiting for my EMG to get approved. No, I slept on my arm for too long. Originally my Dr said it was a conversion disorder and had me thinking I was going crazy but I got a second opinion and finally had it properly diagnosed.

I broke my humerus and the surgeon got the bone reamer caught in my arm and had to open my arm up to get it free. I bet it was very scary for you to just wake up like this and not know the cause. Best of luck to you and thank you for responding

Hi All , i had Biceps surgery 1.5 month ago and my radial nerve got effected , i performed ultrasound and they so Pressure on the nerve , i cant straight my fingers only close them :( , ive been told by my Doctor that it should pass in ~ 4 x month , and i should see improvement signs in ~ 3 x month after surgery........ fill very bad with that i hope ill get my hand back :(

I suffered a spiral fracture to my right humerus just over a month ago, after an arm wrestle. Radial nerve damage has resulted. After 3 weeks of being in a cast I was put in a brace and splint, the latter I only wear at night as occupational therapists encouraged me to exercise the hand as much as possible. Finger movement still limited as per the video, wrist I can move as far as the neutral position. Is it possible that the bone not being healed yet is the primary delay to recovery?

I have the same injury, spiral fracture of the humerus from arm wrestling with radial nerve palsy. did you make a full recovery? at six months in I can also now lift my wrist to neutral

@@payton5233 I did, yeah. Took me about 6 months for the fracture alone to heal (if i recall) and still had some physio a little bit after. So broken in May, discharged from fracture clinic November-ish, physio discharged me early the following year, I think. Then I joined the gym in about March (or May, I forget) but weaned myself in carefully. Just make sure you get what exercise you can (even if it's just walking plenty), eat well and expect the nerves to rewire a bit funny (e.g. when I rub my thumb, I also feel it on the left side of the back of that hand). Functionally, apart from that and getting weird pins and needles down the whole arm at night, I'm doing fine. Everyone is different, though.

Hello Everyone, I want to share my experience I felt radial nerve palsy after broken my arm(humerus fracture) sine Dec 2021 but I make operation on March 2022.. After 3months my fingers are working partially but wrist is still not working and I get depression that I thought It will not recover like a normal and I lost my hope…In July my fingers are fully recovered and wrist is recovery partially and now Nov 22 my wrist are fully recover but I cannot lift some heavy thing due to my broken bone.. As per my experience I would like to give suggestions to be patient and consult with your doctor.

Is it possible to get this for only a minute or two? I woke up in bed and I freaked out when I couldn't move my hand, it had wrist-drop and was numb with pins and needles. After a few seconds of fervently trying to move my hand, it finally moved and it subsequently returned to its normal abilities after a short while; only a dull ache in my tricep is left, really. Was this Radial Nerve Palsy or not?

Tobias Jones, yes! Because radial nerve palsy happens after humerus fracture, Saturday night sleep on your arm, tight watch or a similar object or deep wound on your upper arm. I hope I answered your question!

I am currently suffering from this. I had surgery on May 9th for a detached bicep the button securing the tendon broke free from the bone and was tangled in the nerve causing damage. I have not regained any movement and it has been a month. I am hoping to start PT to regain movement.

I had a distal 3rd humerus shaft fracture and may have damaged or bruised my nerve. I have wrist extension from the beginning and but I finger extension is still recovering. I cannot(only my index and middle finger extends a little) extend my finger when my wrist is extended up. Does anyone have similar experience? I would like to know how much time it would take from now

i went with my friends in the winter at a mountain resort.. I fell stupidly from the sledge and my arm was caught in a pin of ice and my humerus snapped .I had also radial nerve damage instantly but i didnt realise it at that moment because i was warmed up.. it started hurting 10 minutes after when the ambulance came

i fractured my humerus on 27th of december 2012 .. i can extend my elbow about 90% and close it about 70%.. the radial n erve palsy still persists.. atm im doing recovery with galvanic bath..electrostimuation and massage +PT..i hope by the end of march i can see some results of this ...btw my cast was removed on feb 7th..after having surgery performed and attached 1 plate witth 12 screws in my right humerus...i hope i can get well by may bcz i want to finish my canceled exams from college!

hi i also fractured my humerus bone in november. after my operation i cant left my fingures as well as wrist. now after 4 - 5 week my recovery start i am able to extend my fingures and wrist partially.

hi there.. still cant lift my fingers or my wrist...but my forearm muscle tone is a bit better fro the massage sessions... i hope i can get better by may so i can take my exams... btw i can write but in a very ugly caligraphy.. but i guess is still better than nothing :|.. i will talk to my doctor to have an EMG to see how the situation is developing :|..

Hi Mike, i was body surfing in the sea in India and got caught up in a huge wave that slammed me down on the bottom of the sea. So my upper humerus snapped and I had instant radial nerve damage. Had the bone pinned and plated but the wrist drop was what incapacitated me for the year. How did you damage yours?

hi everyone. i suffered from a montaggia fracture in my left arm in december. i had surgery the following morning, which left me with a plate on my ulna bone. i have seen the doctor twice since and he says i have partial nerve palsy. it took me 4 wks to have full extension of my elbow/arm, though i had full wrist extension since the surgery. but my fingers are still dropped.anyone have advice as to how long it takes for full finger extension or an alternative way besides another surgery.thanks

I had this happen to me about one month ago. I have been wearing a splint since then. With my splint on i am almost completely functional bc I am able to grab. In the past week I noticed my fingers can extend but not enough to type, and if I don't have the splint on it feels like I can barely extend them at all. I can however do a partial "hitchhike" motion with my thumb, which I was unable to do at the time of injury.

I would say that keep ur splint on as much as u can...I m even sleeping with it.....and its much better in our case to have wrist in ext. position instead of flexion pos.
