This is a good point, as others have pointed out it could also be referring to Amphetamines or uppers, which are also sometimes forced on workers to make them more "productive." But the birth control makes sense too. I think what's most important is that he gets the point across that what ever the pill is, she will be killed if she doesn't take it.

The story of a smuggled / trafficked illegal Mexican immigrant / religious woman, coming to America.
