I have also read the NTSB preliminary report and I have to say, water in the fuel is a serious oversight. You simply cannot ever skip the fuel check, especially on the first flight of the day, especially after refuelling and especially after rain - even if both fuel caps had their seals intact which, apparently, one didn't. With that said, I have also lost a dear friend in a seminole PA44 twin following an engine failure immediately after takeoff. In common with this accident, the speed was allowed to decay below Vmca leading to loss of directional control and impact with the ground. One big difference in the training between US and European pilots is the requirement in Europe to perform a simulated engine failure after takeoff which (for safety) is performed in the climb above 300ft with takeoff power. The emergency drill includes applying rudder to control yaw, ensuring full power on both throttles, full mixture and full props, identify, verify, feather failed prop and ICO failed engine, gear up, flap up and pitch for blue line (Vyse). Any twin aircraft is certified to climb on one engine if these procedures are correctly followed yet the US training fails to include a realistic practice or test of this exact scenario on the basis that it is too risky. Every European pilot will have done this multiple times and are therefore much better equipped to deal with such an emergency in the air if it should happen. It is tragic that Sam lost his life for a sequence of events that could have been prevented at so many missed opportunities.

Hey! IT technician here. I worked at a flight academy in Madrid for a short while. As much as I want to believe in our system, there are a lot of cracks in European flight education. The place I worked at was a hellhole, and I was in charge of all the funky fake avionic panels for simulators... When I have absolutely no damn clue about avionics, or rather had at the time. What I saw there was corruption, lack of standards, cheating and fraud. I guarantee you this training was NOT performed by any of the pilots in the academy NOR the independent students who took simulator hours in. And yes! That is very much not legal. I tried to submit a few complaints to the relevant authorities and was told "well you're not a victim of a crime so we can't do anything". It boils my blood knowing that this awful behaviour I was witness to can lead to accidents as bad as these. I tried doing my part there but there ain't much more I can do.

You know that the NTSB is saying it was Sam's fatal mistake of not checking for water in the fuel that caused his death. Water in the fuel was present at the crash site. The preliminary report has witnesses saying Sam was "in a hurry" and did not check for water in the fuel tank.@@rebuildrescue

They can honor him by learning from the tragedy and changing the safety culture around what they do. @@400_billion_suns @mustralineabsorbine5082 Tell it to the World War 2 mechanics that took shot down airframes with 20mm cannon holes in them and put them back in the skies to fight again. As with anything, it's about the level of care you take. And with aircraft, the required level of care is EXTREMELY high. The NTSB has made their report. You don't ASSUME that your aircraft is air-worthy before going wheels-up. It was a tragic, but avoidable accident. So yes, the best way they can honor him is by learning from the tragedy and changing the safety culture around what they do. But beating a man over the head with blame for the death of his friend is wrong and you should be ashamed of yourselves. Not to mention that the rubber seal that was missing is a apparently a common occurrence, not something that was due to the plane being "junk", but just due to age, and things like that are why you, as the pilot, check fuel before take-off EVERY TIME. I guess you think all older planes should be grounded, or buying used planes should be illegal. Repairing old planes isn't the problem. If you felt this really needed to be said to him, then do us all a favor and log off.

Preliminary NTSB report says water in the carb. Sam sadly did not do a pre-flight, but a pilot's pre-flight is a last line of defense. I hope if Rebuild Rescue continues with operations, they do the proper self-examination and put safety front and center from here on out. From RBR videos, Sam seemed like an awesome guy and a great pilot, but even great pilots are still human. Frustrating thing about this is, this crash was 100% preventable, Sam did not have to die to learn this harsh lesson.

There are scenarios that would be outside of the aircraft manufacturer's, the maintenance crew, and the pilots control that can have potentially just as tragic outcomes. I think a reaction video to parts of what Blancolirio had to say about this incident, and some of the comments to that video. Water in the fuel is only the last part of the incident, but not how and why there was water in the fuel system. There were also questions about whether the failed engine was feathered. The gasket on the gas cap was apparently missing, potentially allowing water into the tank in the first place. Was a proper review of the engine failure instructions done before takeoff? One engine isn't enough to climb in that aircraft unless the failed engine was feathered. Was that a memory item? It sounds like there was big pressure to deliver the aircraft on schedule to the buyer to raise money for the channel, and a flight to make right after delivery. A slow fuel delivery was interfering with those plans. Many holes in the cheese lined up. I'm sure there are many people considering their part in this incident, and lawsuits may be inbound.

I dunno. I'll reserve my most critical judgements for the final accident report, but the preliminary report is showing that the gasket was not installed on the gas cap. I've been delayed on a trip, because the gasket on my pa31 was the last thing to get to the shop, and the annual could not be signed off on. I'm just a pilot with a degree in aerospace eng, so I am not an expert on the exact procedures/laws regarding this. HOWEVER, this missing gasket does seem like something that should have been looked at while working on his plane before it was handed off to him. And maybe it was mentioned to Sam and perhaps there is not a requirement in the US to have the gasket completed on signoff. Yes there is responsibility of the pilot as well to check the fuel, but still, I feel that theres some responsibility lies on RR here. I will agree with the majority of the experts in the aviation community that the format of fixing planes quickly on the cheap by under-qualified individuals is a recipe for disaster. Having a mechanic inspect/complete your work, but you are also his boss with a show/interests is also not really what these apprenticeship/self work exceptions were designed for. This video, kinda half dedicated to Sam, also reads as really disingenuous, and my gut is telling me something is very off about the demeanor and format of this video. I get that "he would have wanted us to continue working on it" aspect, but, he would have also rather not died. The creator here is obviously in a tricky situation, cant divulge much for legal reasons, and is probably struggled to find the right tone... I think RR is really lucky that the intricacies of this case are too complicated for the average social media consumer to really understand, and why this format should be seriously reconsidered...but more genuine apologies have been eaten alive on the internet, and I understand that this video isnt an apology, but this story and vid is really a ticking time bomb. Lastly, given all of this, I know there were some ideas to have this 401 used for angel flights, donating the plane for transporting kids for intro flights, or providing air travel for people who need /deserve it/cant afford it. I really hope more people aren't killed as a result of trying to get this plane operational hastily, being worked on by people who really are not experienced enough or qualified enough to be doing it, or being flown by pilots with little/no experience on these kinds of twins with little aviation experience. The 401 is a great plane, but like many light twins of this size and era, they are very complex designs (mechanically and piloting), not designed for longevity, not super forgiving, and are for the most part, antiquated. Im sorry if this is rubbing the fans the wrong way, or if this seems like a dissenting opinion here, but I had to put my thoughts out there.
@PegasusWings2310 ай бұрын

Very well said. I followed the channel for a few years and once the side rebuilds started as quick flips I began to feel like it was becoming a bit sketchy and even commented about a year ago I felt like his inspector shouldn't really be the guy who works for you. I took so much heat for that comment, yet here we are. My opinion, work on the 401 should stop, channel should be shut down and Jason go back to tinkering with cars.
@mikegoddard156010 ай бұрын
So what is the timeline you think adequate for 401 rebuild? I mean the first videos are 2 years ago and it still seems 6-9 months out before it's back together. I know they have other projects at various stages of work as well, but to say blame lies with RR while no less than 5 other repairs were done after it left them and RR didn't do the annual inspection seems a bit short sighted.
@pacificdune10 ай бұрын
@@Zappy1210 I have a slightly different opinion, but agree with you in general terms. Safety is a culture, and a choice. If you go back and rewatch most of the RR videos, safety and doing it right is mentioned, but it does not seem like a culture. Perhaps there are legal considerations, and a desire to “just remember Sam”, but he did not need to die to learn these lessons. Plenty of people have learned before them. This seems like a common problem on these planes. Was it left outside? Did it get rained on? This video does not even mention that Sam died in a plane crash in the Cougar that had been prominently featured as a flip on this channel. I find that shocking.

I have seen the video completely critical of Rebuild and by implication suggesting that they are unqualified and sacrifice safety for content - maybe that is just completely unjustified jealous opportunism or maybe the truth lies somewhere in between - BUT - why not wait for the outcome of the Air Safety investigations before making broad subjective assumptions.

@timedward8245 the prelimanary report is in. They had done major maintenance that day with no tedting before the flight. Sam was running late and in a rush to take off and it would appear he did not test his fuel preflight because it was full of water and debris probably caused by a missing gasket on the fuel cap.they deserve all the criticism they recieve for cutting so many corners.

This is terribly tragic and like most GA fatals, 100% preventable. There are many questions here. Did (or how did) this AC get through annual missing a fuel cap o-ring? Were the tanks ever inspected (considering AC sat for so long)? Was old fuel discarded? At the end of the day the PIC determines air worthiness… Did Sam do a pre-flight? Did he sump tanks (I realize that sumping immediately after fueling could be deceiving as it may take time for fuel/water to separate)? Based on eyewitness accounts it appears the answer to the last two questions is no. It is debatable if he did a run-up. Get-there-itis is a killer. As much as I have enjoyed binge watching these videos over the last month or so something hasn’t sat right with me since I saw the episode where Sam and crew were taking off near max gross and riding the stall horn. I think they were sort of joking about it while it was happening. Sam at least looked a bit concerned but also seemed to laugh it off. I just kept thinking they were lucky they didn’t spin in and kill themselves. I’m not a CFI or DPE (and one can correct me if I’m wrong) but I’m pretty sure hearing the stall horn on take off or approach is enough to fail a PPL checkride. Bringing forgotten aircraft back from the dead is a great thing. It pains me to see that half my local AP looks like a GA graveyard. I also like the fact that Jason often discusses doing things “the right way” which I take to mean by FAA/POH/manufacturer standards. However, the reality tv, “rush to get things done”, atmosphere doesn’t align well with aviation. RIP Sam.

I read the preliminary NTSB report on the Cougar crash. Very sad to think water in the fuel and a missing fuel cap gasket allowing water in the fuel tank may have caused it. It appears that checking for that condition is fairly simple but he was in a hurry to get in the air and didn't check. It shows that mistakes by those with a lot of hours and experience flying can have very bad consequences. A good man gone too soon. Slow down and be careful guys.

My grandfather was an Air mail pilot and barn stormer in the 1920's and thirties. My dad, was a combat pilot, and then a commercial airline pilot. During Vietnam, he lost a lot of friends. The air force would just rotate their families out, and they'd be gone, they would just disappear out of our lives. I was born in 1960 and these where my formative years. My dad got out of flying for years after retiring, due to PTSD, (see Tet offensive) But eventually got back into flying and teaching safety to air crews in the middle east. My older brother is pilot for a major airline. and loves fly above all else. My heroes have always been aviators. Sam was living the life, and it is a great life and I will miss him on this channel. Jason and co. my sincere condolences to you, Sam's family, and your R.R. crew. As have many others, I've been following your channel from the beginning of R.R. when the 401 was full of bird stuff. I am a 45 year aviation veteran with an A&P license and a P.P.L. As others have indicated, I was also concerned that you began working on an the aircraft with your extensive automotive experience - only. Granted, you were simply attempting to get the 401's engines running so no concerns there. My primary concern I observed on more than one occasion is to watch inexperienced aircraft owners work on their aircraft with no A&P mech. supervision. My "spider sense" concern was what level of workmanship were you going to apply toward actually rebuilding the 401. Were you going to be a bit heavy handed or were you going to learn - on the job - with your A&P mech partners, to develop the required feel, touch, skill required to return an aircraft safely to the sky. As time went on, I became a very relieved, true believer in your developing, caring, professional aircraft mechanic skills. I've watched you gain the awareness of the life and death necessity of rebuilding the aircraft with --only-- the certified parts and the sometimes delicate skill it takes to perform the best work possible. Since it truly seems as though you and your crew have been doing such exceptional work, so it was heart breaking to be concerned that you may have missed something on the Cougar. I just watched the video of the preliminary NTSB report of water in the right engine carb - which may be attributed to a missing rubber seal on the right wing fuel tank cap. It's easy for me to say that as such a simple thing to miss is, you have learned, cannot be missed. I pray that you all have learned a valuable lesson here but sincerely hope that you will continue what I believe to be a true, concerned effort to return aircraft to the sky for a great purpose. I'm sure that Sam wants you to continue to finish the rebuild of the 401. If you haven't thought about the following, going forward, you might want to consider the following concepts: 1) If/ when you decide to begin working again, you might consider the creation of a more formal plan to --double check-- any work performed. CHECK LISTS!! 2) Possibly coordinate more closely with the FAA during the rebuild process and document everything that is done to the aircraft. For example, have they checked the interior of the 401 to confirm its status? Has the FAA been inspecting the aircraft periodically - step by step? 3) If you haven't been doing it, document everything that is being reworked, rebuilt, and replaced on the 401. 4) Again, double check everything that you are doing on the aircraft - this cannot be emphasized enough. 5) Should you have any additional aircraft "annualed", make sure you yourself become 200% familiar to what is required in the written instruction requirements. Then double check with your IA and review every last detail of what is listed - with him/her. 6) I realize that you run into time constraints. Just the same, isn't it better knowing that you've exhausted all written requirements and then some. You must --only-- release an aircraft that is truly ready to fly or don't release it until it is!!!!!!!!! To lose an aircraft AND a friend is beyond what all of us who are passionately involved in aviation ever want to hear about or experience. Once again, my heart goes out to you, Sam's family, and your team. Steve

Im sorry about Sam and the loss everyone is left with. I think the best thing we can do to honor him is to learn what happened and make any necessary changes to our risk management plans. One thing everyone should do is find out why twins are spinning into the ground. It’s something I’m studying myself right now. And as a 38 year airline pilot, Instructor pilot, A&P/IA, aerobatic competitor I can tell you I am still a student of aviation. I realize an airplane won’t forgive you for a mistake it will kill you for it. My Dad was a WWII B-29 co-pilot and I learned a lot about risk management from him. Let’s all figure this out and make some changes as necessary.
@branchandfoundry56010 ай бұрын
Question: Are under powered light twins like this still produced? Specifically, airplanes which cannot continue to fly as a single in the event of an engine failure without immediately feathering the failed engine's prop? Theoretically, all pilots are awesome and react perfectly to emergencies in an instant. However, in practice it takes a bit to get your head straight and go through emergency check list procedures. Twins that cannot fly as singles--even with only one soul and partial fuel load--fail to accommodate the human aspect. I'd love to hear your thoughts on this point. Thank you

"Get-there-itis" is unfortunately a big killer. Been some high-profile examples of this in GA such as the deaths of Buddy Holly, Ritchie Valens, and more recently: Kobe Bryant. Sam being in a hurry is unacceptable. GA is not meant for on-time transport operations. Even FAR part 121 operators can encounter numerous reasons for delaying a flight. But in GA it's exponentially more likely and cannot be relied on for being somewhere on time. I hope Jason does a video on performing preflight aircraft and weather checks, some of the things to look out for, and push hard the dangers of being in a rush and the importance of always being ready to call off a flight. It would be hard because Sam is his friend. But this is important for potential future aviators watching his channel. It's hard to call off a flight. Many students work a day job, sometimes 2 jobs, and set aside a day or an afternoon for flying once a week. Sometimes it doesn't pan out. I live in a prairie state where powerful squall line storms can pop up out of nowhere. Even if things appear fine but the forecast says otherwise, pulling the trigger to not take off for any flight beyond the pattern is a wise habit to put oneself into. Set strict limits on wind speeds and visibility. If it's 0.5 knots over that... cancelled. That's tough here during the spring since we often have high gusts. Well, I'm rambling on. This further shows the importance of a PIC to take the time to be safe and respect that GA flying is not a mode of transportation.
@badbatch97410 ай бұрын
Time and money pressures are inevitable I’m most cases. You can’t expect everyone to have an unrealistic amount of time and money to accomplish their goals. This is why redundancy is such a huge part of aviation. The fact that everyone is so concerned with inspecting the trouble areas they were working on that the simple common issue slipped past. The incident would only be able to exist do to many combination of factors that were all over looked including the most important, doing a proper preflight inspection and calling off the flight at any sign of concern.

You are right, a proper pre-flight is essential, but there is also the risk of over-confidence (I'm not saying this was the case for this crash). I am reminded of the tragic death of F14 veteran Dale "Snort" Snodgrass who was killed in his Marchetti SM-1019B because he did not check that the control lock had been removed before his takeoff. Regular use of checklists, despite them seeming mundane, boring and irrelevant, saves lives.

@mike it’s dramatized for KZbin obviously. The team is safe and follows protocols but have to make the story ‘interesting’ otherwise it’s just a guy taking forever to fix planes. Fuel gasket was on- but ejected in wreckage as either part of the impact or lost when first responders checked fuel levels- maybe even prying open the cap and destroying it in the process accidentally. The plane was inspected and cleared. Everything isn’t ’black and white’ even if it’s printed that way.

@@timgotshaw7200who are you, Jason's attorney? then explain why the right tank was contaminated with debris and water and the left tank wasn't? How did the water and contaminants get into the tank?... He took on fuel in both tanks... One was ok.. The other not... And the gasket wasn't "ejected" in the wreckage... Or lost.. What kind of fairytale are your telling yourself?? The plane flew in from Michigan ok.. So the water and contaminants were acquired between it's arrival and Sam's departure... If the fuel truck was contaminated... There would be problems in both tanks... Ray Charles could see what happened here.. The fuel cap gasket was overlooked... Sam was in a hurry... A proper sumping didn't happen because of a hasty preflight... The water and debris caused the right engine to lose thrust right after takeoff.. Vmc stall/spin... Thats it in a nutshell.. The NTSB prelim report says it all.. Regardless of who is responsible for the gasket not being there... The lack of proper preflight is ultimately the cause.. Had the right tank been stumped till clear of water and debris.. It most likely would have been fine... Its just a bad situation all around.. But nobody should be condoning what's happening at rebuild rescue... Amateur shade-tree mechanics doing most of the work being barely supervised by actual A&P/IAs.. Sure, the right signatures are getting in the right blanks... But is quality/safe work being performed in every instance?? Doubtful... Now, I will say... Things have come a long way... More and more operations are being done by the right people.. And Jason and his guys are getting better... That's all good... Over time I think they have realized some of the things they are doing was way way wrong... But overall... The entire operation is still way too dangerous and if things don't change Sam is only the 1st of many casualties that will be suffered due to this type amateur operation...
