I had an ischemic stroke when I was 38 years old and I was bedridden for months now I'm proud to say that I can walk without an assistive device outside, I drive, I go to the mailbox... no it doesn't look "normal" but I couldn't even sit up in the beginning! I had to learn EVERYTHING all over again and your videos helped me so much!!! It will be 2 years on Valentine's Day and I live by myself now and I can make my own food and everything now I don't have a Mobility Scooter or a walker anymore! I used to have assistance to transfer to a wheelchair and have somebody stay here with me all the time for meals etc... I came so far because of you!! Watching your videos made me never give up! I never settled for what happened to me...I'm still tryin to improve every single day. Thank you from the bottom of my heart! Seriously.

Do not think about the future ..just keep moving forward and before you know it you will experience goals that only you can celebrate because you remember how hard it was to achieve!..small goals make me so happy! You know what my biggest accomplishment was this month?? Stepping up onto a curb. seriously I could not do that for over 2 years ....don't give up!!!!

This November will be 3years. Haven’t had PT since my first year bc insurance fell through. No dr or anybody I have seen has helped me like your videos. They act clueless and have helped me to stay in the mindset that there is nothing I can do. I am going to start doing these workouts and going to try to set up a workout routine. Mentally depressed and drifting backwards thinking nothing will get better. I did just buy the sleep afo. Thank you so much hoping I can find the inner me again to help myself get better

Are you on Social Security Disability? If so, you are entitled to MEDICARE! They will allow you to return every year, with a doctor's script, for 2-3 months at a time. Check it out. You may want to make adjustments to your plan. They will, also, cover an AFO, as long as you have a script from your doctor. A General Practitioner can write it out for you. Ask about Baclofen, too. It's for Neuro spasticity.

Any improvements with your husband? What were symptoms did he have before and after stroke? After stroke I am currently experiencing numb hand and unable to walk. Cannot bend the ankles or move the toes. No toe movement is what worries me the most. Will I be able to move it later on? Any exercises I can get my sons to do to help recover?

Such an important video and think a lot of people underestimate the value. Strapping my legs and a ramp made a huge difference. I can hold my hips now but still strap my legs for some things to prevent my leg from wondering out. And most people already have a walking belt or sling they can use for it. 👍🙏

OMG! I'm a little over a year out from my stroke and this whole video made so much sense to me. I have been dealing with this for a while now. I had 3 weeks of inpatient rehab, home health pt/ot, and have completed 2 rounds of outpatient therapy as well and none of this was ever mentioned when I spoke of these exact concerns.

I have been in the same boat, only 35 years old currently and trying to overcome the ramifications of the stroke which was 7 months ago. I have been on medications, and professional physical therapy sessions (stretching, balance, aqua). When I research spasticity due to a stroke there seems to be no cure, only symptom management. I'm an "unusual" case for the doctors I have seen and they haven't given me much hope or options. Trying to take it upon myself to retrain the brain.

Hello I have had³ 3 strokes and know I am going to walk again even though they don't think I will. After my first stroke t 26 I was hemiplegic but I could still walk and talk okay. After my second stroke at 47 I could still walk and talk but my balance not that good. Now after third stroke at 57 I cannot walk and my speech is impaired. I try to put weight on my left foot but I can't. I stand half hour daily and then I do exercise for about 2 hours with some resistance bands . With my AFO on. Should I take it off and try to stand without it?

My biggest problem is my ankle rolling. Can you imagine how bad it is when I drive? I always have to pull over when it is acting up and adjust it, or ask the person sitting next to me to reach over and “Please fix my leg!” Thank you so much Dr. Tobias. IYou are the best. I am your biggest fan. 😀❤️

Your presentation is very useful in one’s recovery from spasticity. A lot of medical people talk about but few give good advice for overcoming its affects. Last time I’ll comment but thank you so very much for this information! I tried this stretch twice tonight with a foam wedge my mom had for her neck from her chiropractor. I used my afo while doing the stretch. Did the stretch maybe 1 minute twice tonight and when I walked again after I was actually able to step more towards the inside of my foot as opposed to the outside of my foot. Instant relief to foot problems I’ve been having for 2 years now. I had thought for a long time it was pain from my afo but after trying this stretch I realize it’s not my afo at all and you’re the only person I’ve encountered that ever spoke on it being from spasticity and offering a possible fix for it. Get to a Neurological Physical Therapist. Ask before you book, if they have any on staff. The nervous system is VERY TRICKY! The scientists (and patients) are learning new things ALL THE TIME! Think them as being your team, as well as yourself! Believe me, they love the input. A two-way learning experience.

I am so so glad that this video is helping. However, you are the angel sent from God helping your son with such dedication. I have not worked with pediatrics in several years, but when I did, I thought these little humans had so much potential given that their brain was still developing. Generally speaking, as kids (with movement challenges) develop, it is good to give them opportunities to explore and interact with their environment (and whenever possible) being there to help them move with "near normal" movement patterns. You described my symptoms exactly. Ankle rolls ,I step and lose balance. The issue you didn't cover was how to strengthen a weak ankle. I'm going to search some of your other videos. I haven't fallen in about three years but I've come close and its always the ankle supination. Thanks for your videos they are far and away the best on you tube. I don't wear my brace because I don't want my ankle to get weaker as I depend on the brace more and more. My toes curl a lot too. Some days less than others and I've found I can "walk that off" usually.

I did try the motus hand mentor, while they told me I was gaining strenght I never saw the improvement they said I was making,I did get a little extension after 3months, but i had so many issues with the units they sent, soft wareissues andhardwareissuesI ordered some of the saebo equipment, the mesh glove and some contact electrodes, unfortunately the tens unit I need to make use of the things I actually recieved was on back order I've been waiting for it to arriveI'll update my progress when I make some, my wife and son tell I'm not as patient as I used to bewhen it comes to spending money disability doesn't make up for what I used to make, it may seem I was hard on motus nova,I paid $199.00/er month for the use of the stuff they sent, and the clinical support was minimal

Hello, I am so glad I found Your site.I have gotten more information and help from your videos than from my Neurologist.I have spasticity on my left side, and all the problems that go with it. So, after 5 MRI's about to be 6 and 2 EMG's etc. He still doesn't really know where it came from.I have been dealing with this problem for two years! My wife normally uses an AFO brace when she walks outside, mainly to protect her from rolling over her ankle from uneven surfaces and things that can create a problem. Dr. Tara, my wife is experiencing so soreness with the muscle that causes the foot to bends towards her, located at the top side of her foot that stretches up to her shine muscle. For myself, I experience a crap with that same muscle if I over-exercise, would you have any therapy that can address our problem.

My son R Ganesh is having spasticity in the left leg ankle. I find no tool is available in India for the improvement. I find the Norco Fasciitis Night Splint will be of more useful. But I am unable to buy this since the cost of the material is very much on the higher side. By seeing that image I started trying to make it on my own. I really believe I have this in both feet. I had a severe army accident and I’ve had it ever since. But I had no head injury I’m aware of? Also every person I’ve seen always just says, they have no idea what is up with my. Both my feet just hang from my leg, but I can lift my feet ok. The pain in my calf’s etc is so bad constantly. I’ve had those for years.

You’ve stayed in many different videos that wearing an AFO is important. However, I used my AFO religiously when I had little kids. Yeah I worked for 10 years and whenever I took my shoes off, I fell frequently. I always had a horrible gate that made sense with my right foot no matter what Pt I went to. Moving to Florida I need to learn to walk on the beach . So barefoot or with light weight, then shoes I walk to the beach. The past two years I barely wear my AFO and I’m able to walk with shoes and I do have a foot drop ankle hook that I use (have several different types for different shoes) But I found for the first time I’m able to wear sandals now without an AFO and not have an accident be falling . Therefore, I really disagree with many physical therapist, saying you need to learn to live with the AFO . I did for 10 years and it didn’t work. Everyone is different . So please try something safely for yourself.

Thank you for this! My PT told me I will be in an AFO for the rest of my life. I refuse to give ip hope and will keep trying. I hate the AFO

Hello Tara, my foot is over pronated thanks to spraining my baby toe 4.5 years ago. I have had MS for over 27 years and unfortunately, that entire leg is now weak. I watched the hip drop video...which is one of my challenges... super helpful! Hoping you can do a video on overpronation. My foot of course turns out...which makes walking very awkward and unstable!! Thank you!!

Thank you for the advice! Do you recommend practicing walking without the AFO at some point to alleviate the foot ankle spasticity so the stroke patient does not become dependent upon the AFO for life?

@Bella,Ella! Wow, not good! It only weakens the ankle more! I love the wedge. My first outpatient PT used it with, and I right away saw/felt the benefit (they were made of wood vs foam). My husband made me a set. I used it along with my BIONESS training mode all through the pandemic lockdown. THAT COMBO WAS A GODSEND! When I went back to outpatient PT, (yes, I go back every year, 2-3 months, as the insurance will allow) my PT was amazed I hadn't lost any of my acquired range!

Hi Tara. Firstly I appreciate the flow of good information. Bless you my dear. Do you have any recommendations for those suffering Gout. Also was a recent patient(4 mths ago) for triple bi pass. The care afterward was less than stellar if not any at all. No physio for either. The experiences had me believing it was all a new Stephen King concept. Any help would be appreciated. Contact a PT that specializes in orthopedic PT. Anything to do with bones and/or joints. She can help you, however, her speciality is Neurology PT.

My daughter has a brain injury and per your video, her issue is spasticity and causes her right foot to roll outward. Serial casting helps alot to stretch her so she can get her foot flat on the floor. She also has the Bioness 300 which works well at lifting the foot but if she is super tight, it can't clear her safely for walking. She started with an AFO with SMO insert as you suggest and no doubt she is stable in it. But I don't like it because I feel she is locked up in it and doesn't have a chance to stretch and get stronger. She also gets Botox treatments in her leg. Clearly it is all a balancing act. Can you speak to using the Bioness versus the AFO for someone who does struggle with spasticity.

Hi Stephanie, that is an excellent question. It is best to work with the person who made your base so they can help you choose a shoe that will work best. In my experience, I have tendency to lean toward new balance (not minimalist sneaker) but the actual "traditional" old school new balance. They have a wide toe box (to fit a brace), pretty stiff leather (adding stability), and you can get them in velcro or lace up. I hope that helps!

I humbly request you to make a video to fix Delayed response on movement. Lately. I was in a passenger seat on a motorcycle and while getting down I was finding it difficult to extend my knee and abduct as my hip was arrested on seating position and as my knee was bend for a long time. Kindly advise on a fix and recovery tip for this issue. When I took advice from my therapist. He told me this will get better in practice regularly as it's a new movement for the brain.
