Fascinating. I’m using my first cpap and it’s resmed AS 11 auto. This video helped me understand how to interpret my data so much more than I was doing. I’m still adjusting pressure and trying to find a mask that’s tight for me, so my data looks erratic. I hope I can get it dialed in like you show. No one is helping me except for you. I’m so thankful for your great content

@@NYEliteAlleycats-NAL Yes I did. You are looking at a LITE version of one manufacturer's machine against a FULL Blown version of another. is the algorithm used in the Resmed mobile version the same as their FULL blown CPAP machines? Is the performance the same? I have a database of over 300 patients using Dreamstation 1 and I've not seen the issues that are shown here. I have no axe to grind I've found both Resmed and Respironics offer excellent units and have meaningful though not perfect algorithms. Would be interesting to know if the algorithm and performance on the Resmed AirMini is the same as the AirSense 10 Autoset. Would also be interesting for patients who use a Dreamstation 1, 2 or GO to upload their data to sleephq and use sleepyhead to see if they are observing what is shown in this video for themselves.

Great video, and since I was diagnosed with apnea in June, I binged your channel because you always have the best cpap reviews and advice on KZbin. I finally received my Resmed Autosense 10 and P10 nose pillow mask at my fitting last week, and went from an AHI of 37 to 0.23 in a week. I haven't noticed much of a change in sleep quality, and am still tired a lot, but I have a clearer mind at work and feel more productive. The Resmed reports and myair app are very basic, but I get the details I want quickly and easily, my nightly scores are 99s and 100s. Thank you for all of the info you share with us!

Thank you Nick for the great channel. I've been on CPAP for 14 years. I started with the old Resmed, then bought a Phillips dreamstation in 2013, then it got recalled last July and I bought RESMED AirSense 10 in a panic after the recall (used my old 2008 Resmed while waiting for the new one to arrive). Now Phillips are fulfilling the recall and sending me a new Dreamstation... So, I have my original Resmed (circa 2008) that still works, I have my carcinogenic Dreamstation (circa 2012), my new'ish Resmed 10 and the new replacement dreamstation arriving monday. Unfortunately, the Phillips machines aren't quite heavy enough for a boat anchor! Will stick with my Resmed's. I really value your clear instructions and explanations of the data to help me with adjusting my settings. I just signed up to SleepHQ and have been looking at my data which appears to look pretty good. Going for the O2 ring as well just double check O2 levels. Keep up the great work!!

I am using ResMed AirSense10 and after watching this I’m glad that I am. When I first investigated therapy my test showed 52 events per hour and sleep quality wasn’t that great. Since going onto therapy I have got that down to somewhere between zero and one event per hour which is a vast improvement. My pressure is set to start at four and from what I understand it doesn’t move from that for the whole time so something must be going right. I’m feeling much better to all thanks to this device. Thanks for the great video Nick.

Thank you Nick, I'm a respiratory therapist and sleep technologist, I setup PAP equipment daily. I totally agree with you. The recalled DreamStation had superior reporting, wave forms etc., that's why I favored the pre-recalled machines, they also had a feature called Opti-start which moved the pressure gradient forward so that reaction to an event would take less time. I witnessed the effectiveness and it was remarkable. Those pt's that have the DreamStation 2, I'm sorry, it's a terrible machine both quality and algorithm efficacy. If you have these machines, instead of having a straight pressure, which might be uncomfortable for some pt's. I suggest shifting the pressure gradient, i.e. 5-15cwp, try 8-15cwp. Hope that helps.

@busigh
@busigh 2 жыл бұрын
@@dimiterprenerov8683 You would have to look at your software version. I think the last version for the DreamStation 1 was 1.1.8. If its the same then i think the algorithm is probably the same I had to remind some of my patients when they got their replacement units is that the machine has to relearn their breathing. So it typically starts off at the lowest pressure of their prescription and work its way up. Each night after starting a bit closer to the pressure the machine used for most of the night before. Your old machine went through this exercise when you first got it. After a few days of use your new machine will start at the pressures your old unit did 😀

@@busigh Thanks for that comment. The DS1 says it's software version is 1.1.8. I have it since March 2022, so if it didn't learn my breathing so far, I doubt it will do any further. Maybe the old one was with older fw version, but I can't check this as I returned it to DME.

Thankyou, thankyou, thankyou. After watching this I went straight out and replaced my Phillips Dreamstation with a RESMED Airsense 10. The difference has been incredible. After more than 18 months with the Dreamstation, I was still waking up tired. I assumed this is because my resmed P10 mask was falling off (or I was unconsciously taking it off) during the night. Now with the Airsense 10 I usually wake up in the morning feeling refreshed and its rare now for me to wake up with the mask off. There is absolutely no doubt that that the RESMED Airsense 10 has been infinitely better and its changed my life as I wake up not feeling tired, no headaches etc.. This is life changing for me. Thanks so much Nick, these videos are so important, please keep this channel going.

WOW! Thanks Nick. So glad I'm using Resmed Autosense 10. After my initial four week titration study with 2 weeks on Auto and then 2 weeks on fixed pressure, I preferred the Auto setting, which I've stuck with. Glad I did, because my diagnosed upper pressure requirement was initially 11.2 and stayed around that consistently, but it has steadily increased over the past six months to around 12.6 or 12.7, sometimes up to 13.5 now. When I talk lying down on my back (mask off) the sound is a bit strangled and nasal, and my throat feels like it has closed up a bit. This has worsened over the past year, so I wonder if that explains the need for my Auto pressure to increase too. Does anyone else experience this? The joys of getting older haha!

Thanks for this info. It is really aggravating to have paid $1,000 out of pocket for this machine and have so much go so wrong. One thing I don't get it why these machines don't get updates to improve the algorithm. I'm not specifically singling out Phillips. I mean most of these have SD lots and many have bluetooth, Wifi or cellular. These manufacturers could send over-the-air updates or post files online so we can make use of these things.

Had my first, well most recent I should say, sleep test last night. A home test from Deeper Sleep Bathurst. I've tested positive before, about 7 years, and did nothing about it, but now I'm older (49) and restless leg is getting worse, it's time to do something. I found your channel recently for research and I'm glad I did. Your videos are entertaining and informative, and even better, offer an Aussie perspective, unlike most channels. I noticed you had some videos singing the praises of the AirSense 11, about a year old, and the AirSense 10 getting a little old now. I haven't seen it on any local CPAP reseller sites, or even on Resmed's site. What's happened to it? I'd rather get a newer machine than an 'end of life' model, as per one of your videos.

I think what David needs to do is tighten up that range. I get one or two OA's per night with my DS1, but my range is only 10-13. Any lower and I start getting more OA's; any higher and I start getting CA's. 10-13 seems to be the sweet spot for me. Looking at my data on SleepHQ, I see it mostly stays at 10, but often spikes up briefly before going back down (in the space of ~5 mins). So maybe rather than switching to fixed pressure mode with the DS, play with the range until a sweet spot is found. Great video Uncle Nicko, thanks! And I'll get a ResMed just as soon as I bubble up my provider's wait list. Meanwhile I'm using a foamless air chamber (thanks to you), so I'm doing okay for now.

I borrowed the money from my partner to purchase it - and am I glad I did! Find a way to switch over to ResMed because my AHI is now under 1.0. I want nothing to do with Philips as a company. I will NEVER buy another product of theirs as long as I live. I used to be in sales and the way Philips handled the recall was unconscionable - plus they've allegedly known about the cancer foam for at least 7 years and didn't bother to come forward. I had a Dreamstation One Bi-Pap for about five years before the recall happened. No serious complaints. Pressures of 13.0 and 8.0. Once I got the refurb replacement it wanted to use Auto Bi-Pap mode and kept the intake pressure at 10.0, which felt like I was suffocating. So I found the developer menu by holding the Ramp and Dial button together for about ten seconds, and switched that puppy to normal Bi-Pap mode with fixed pressures of 13.0 and 8.0. My AHI last night was 0.5. No, Dreamstation, you don't know what's best for me.

Auto gets trashed a lot. What also matters is which machine one is using as algorithms and made equally, there is no standard and manufacturers are secretive about it. I like the auto on my resmed but i have also tried fixed pressure for a while to see which one is better for me. I feel like i get a better sleep on Auto then fixed even with the same AHI results. Maybe allowing the pressure to drop when it isn't needed allows the body to get a break from the higher pressure which is constant on fixed.

Thanks, I am not surprised but I do know that the Dreamstation 1 algorithm does increase because the Dr set my pressure at 8 to 18 and when the pressure hit 16 my mask would leak like a sieve. I took some advice from our friend Lanky Lefty and narrowed my Auto CPAP 13.5 to 15 and it works great. So a narrow Auto range is another option for some people who are stuck with a Dreamstation. My average is usually around 13.8 with 90% at 14.5

Just by way of a tip, I use a Resmed Quttro F/F mask and I often use wipes to clean it, but what I have found out, alcohol or spirit infused wipes, at the very least discolour the silicone in mask and I have a suspicion it lessens the lifetime of the rubber insert.(I have a ramp of eight and a pressure of seventeen)and this means my silicone insert only lasts 12-15 weeks, which is pretty expensive, but like a lot of sleep apnoeiacs I just cant do without my CPAP machine!

I already had to replace that thin piece of crap tubing they sent with the larger one I had because I could tell it restricted the airflow. Thanks so much for your expertise!

This is very educational. I just had an at home sleep study done but I'm not really sure how bad it is. I am going tomorrow to get fitted for a cpap machine. Below I have posted some results if anyone has any information on how bad mine is. Thanks to anyone who knows about this stuff. Recording Time: 432 Minutes Snoring was noted, there was a total of 376 respiratory events. Of the events, the total number of apneas was 71 (55 obstructive, 11 mixed and 5 central) and 305 hypopneas. My respiratory index was 52.2 events per hour of study time. The mean oxygen saturation during the study was 87% with a minimum of 73%. I spent 275.1 minutes at oxygen saturation measuring less than 90% (63.7% of recording time) And 206.8 minutes at oxygen saturation measured at or less than 88%

So glad I found you, and sub’d!!! I’ve been on ResMed autopap since 2003 and didn’t know anything about anything, as I am finding out watching your channel!! I was told I have severe OSA. After a sleep study 13+ years ago the doctor said I should be on a CPAP at a steady 12 pressure. Previously I was on 5-15 as needed. I asked him why I should go down in pressure if I was experiencing apneas that needed 15 pressure. He said, “ because you sleep better at 12”. I still didn’t understand, but that’s the only reason he could give me. I stayed at the 5-15. Does this make any sense to you? My ResMed 10 has been giving the message “ MOTOR LIFE EXCEEDED “ for 7 months. So I bought, but haven’t received the ResMed Mini and it’s way overdue. BTW here in USA insurance won’t cover the Mini, because they regard it as a LUXURY!! I’m a senior and paying 1114.00 is a lot, but I travel and need less weight and want the compact style and ease of no distilled water. Wow that’s considered a luxury!!!! Now I’m not sure what the right machine is for me, but when I get it figured out I’m DEFINITELY getting your Sleep HQ app. Can you advise me in any way, would love your input? Thanks so much. ❤❤❤

I noticed this myself when I switched to the AutoSense 10 (because I didn't have a year to wait for my replacement DreamStation). Good comparison, and also underlines how important it is, if at all possible, to incorporate Oximetry Data. I do not think you get the whole picture without seeing both CPAP and Oximetry Data. The Philips results above are absolutely appalling, and almost suggest the user would be better off without it. However, I agree that switching to Fixed Pressure would be a sensible workaround to overcome the effectively useless Software issue. One question this does raise for me, is the issue of Software/Firmware versions. I have a few ResMed S10 units to play with, and the Firmware is dependant on when made. There is no sign that any were updated after manufacture, despite the fact that most would have had their Modems active during use, so should have been updated as they went. Or at least should in an ideal world, given the fact that the Manufacturer can communicate with the devices if the Modem is active. My wife recently received a new ResMed S10 AirSense AutoSet via the UK NHS, and whilst that was functionally identical to her existing ResMed machine that I had acquired for her lightly pre-used (I fully disassembled it and sterilised it from end to end before use), I noted that the Firmware was different. Her results are also, not surprisingly, very slightly different. Nothing major, as we had everything well monitored and under control but, looking at her Data via OSCAR as I do every day, the later machine does seem to operate slightly better. I also think her results would be even better with a ResMed For Her model, but the UK NHS only supplies the Black S10 units that do not have the female-geared algorithms (faster response to breathing issues, raising the session working pressure higher if needed, and keeping it at the higher level for remainder of that session, and a few other changes that I cannot now recall). I gather the ResMed S11 units now have the female-geared option as standard. The point I am making, is that all apparently identical machines are not necessarily equal, depending on the Software/Firmware. I am totally against the way that ResMed and others protect their Software and Spare Parts business models, because this protectionism makes sensible and performance related updating totally impossible and, likewise, sensible maintenance and repair. Both are almost totally impossible (albeit if you look, you can obtain some limited Parts via the usual channels if you look hard enough). In terms of Parts, most ResMed S10 used Models could have a second life just by clinically cleaning them, and changing the Pneumatic Block (AKA the Air Pump Motor Unit), which is the only moving part. But, that said, it would also be preferable if the Firmware/Software could be updated at the same time. I am not in the slightest bit persuaded that this protectionism is for health reasons, the machines are not that complex, and there are many active users who understand how the machines work, fully exploit the Clinical Menus, and take a lot of time to monitor their Data. There should not be any blocks put in place to deny users the ability to update their machines via identical Hardware spare parts, and via later Software.
@CPAPReviews
@CPAPReviews 2 жыл бұрын
Great comment Keith. You raise some great points all of which i agree with. Just so you know the AS10 does not do 'over the air' updates. The AS11 does. Cheers
@keithharrison1453
@keithharrison1453 2 жыл бұрын
@@CPAPReviews Thanks for letting me know about the AS11. I knew the AS10 did not update, I was just saying that that should have been possible, but they elected not to add that feature. That was crazy given how they do update the Firmware, so older versions may not be as good as later versions, even on virtually identical Hardware. I do not know about the AS11, but another minor issue on the AS10 is the MotherBoard BIOS Battery is soldered on, it's a standard CR2032 Cell, but of the type that has fixed pins for legs, that have to be soldered on. That may cause issues if that Cell loses Voltage below the working range, and then it cannot easily be replaced unlike, say, on most Computer MotherBoards. I am going to replace one on an older well-used test unit, as I have noted the Voltage on that one is below 3.0 Volts, and it tends to forget Settings if left for a while, which I think is the low Voltage unable to hold them for long enough before next powered up. I'm not sure how the machine will react, it could in theory lose all configuration settings, so it will be interesting to see how it reacts to a complete loss of BIOS back-up Battery power. If it bricks, then on better units I may need to solder on a by-pass supply when changing the Battery, to maintain 3.3 Volts during the swap out.

I have a problem I do not hear other people reporting. I have a resmed autosense 11. The machine shuts off as many as 20 times per night. Then I can’t breath and wake up to have to turn it back on. This does not seem any better than having apnea. This is maybe an issue of mask adjustment but the difference between adjustment which works and what does not is very fine. The latex ages so what works one day does not work a week later. The settings on my machine can only be changed by the doctor which means in practice that they are never changed because the doctor does not listen to me. They just tell me it is my fault. I would just like the machine to keep blowing. Then it would be fine. I think it is a bad design for the machine that it switches off.

I know this is purely anecdotal but I have noticed that if my Dreamstation and Airsense are set at the same pressure, for example 10cm, the Dreamstation feels quite a bit lower. Also Dave may have better results with his Dreamstation if the auto settings weren't so far apart. Because his ave. pressure is in the teens he might want to bracket it much closer to his average. Instead of 5 min 20 max he could try 13 min 18 max. Not sure if he could tolerate 13 at the start of his therapy though. Cheers.

Great video as always. Have you ever reviewed the Prisma smart max? I’d be interested to hear your thought on it. Long story short I moved from one part of the the UK to another and they changed me from the resmed 10 to the prisma. The prisma app (since getting a good mask) shows 1-4 AHI which sounds great but I sometimes wonder how accurate it is.

The reason for my concern is I was recently diagnosed with atrial fabulation and an enlarged right ventricle. After lots of tests they found it has been years of sleep apnea untreated that has caused it. I’m only 40 so it’s been a bit of a shock, hence my concern to know the machine so working well. I’ve always stayed in reasonably good shape but heavy weight training has also be an adding factor.

Fascinating information. Have to wonder if anyone at Philips has any idea or even cares.

Hi Rob, i doubt they have even looked at it since they took over respironics back in 2006. I don't think they've even tried to improve it. No one apart from yours truely is looking close enough. Cheers mate. Nicko

Hey mate, i just do it on www.sleephq.com which i developed with my best mate. It's free so give it a go. Just grab your SD card, insert it into a card reader and drag the files across into sleephq. DONE!
