You are just amazing. In our country we study "la médecine" in French. So i always prefer to search any information in French because my English language is not so good . I have had a Dr prescribe me 40mg Prilosec since Dec 29, 2018.. could this be causing my b12 defiency? I have been easing off the PPI by doing it every other day, plan on quitting it completely and taking b12 supplements.. is this the right approach?

@@wolfdoggiestyle It could, but I would think that it wouldn't happen so quickly since you should have years worth of B12 stored in your liver. Also, from my understanding, only protein-bound (dietary) B12 would be affected by your Prilosec (unless you also have bacterial overgrowth in your intestines). If you take a B12 supplement, I think you should be able to absorb it while still taking the Prilosec. I could be wrong, so you should check with your Dr.

It's a great topic, i enjoy watching your videos, and i keep commenting them ☺Besides treating the patient with vit B12 in (IM)which is symptomatic treatment we can search for the cause to treat it , because else the problem persists, and there is many other cheap labs and methods to do it through labs to search for eosinophilia or neutrophilia and biochemistry to dose Amylasemia and Lipasemia, inflammation markers for crohn disease if you suspect it (it attacks the ileo-coecal junction mostly).

Here you must stress about main point... If there is presence of vitamin b12 in urine, it means that vit b12 has been absorbed through gut and made into urinary circulation due to saturation of transcobalamin levels in blood that carries vit b12 due to IM injection. Hence if there is less amount of vit b12 in urine, it means that absorption from gut is decreased and excreted in stools...hence next we ll give intrinsic factor, antibiotics

Is it possible to have normal B12 in my blood, but my body is not utilizing it? My B12 & folate blood tests come out as "normal" when I'm not supplementing. But when not supplementing, I have all the symptoms of pern. anemia. The worst of them are neuro symptoms; I cannot function like a normal adult. But when over-supplementing with oral hydroxo & oral methyl, my bodybrain is much more functional. And the one time I got a methyl injection, wowza... I was a fully functional & energetic human. How do I further my research on this? I need to do this myself because several med practitioners I've questioned about this are not knowledgeable, so I have to lead them.

Actually here you might be in seronegative stage not seropositive that's why the Autoantibodies which they test comes out to be negative and your serum B12 and B9 levels are normal or slightly above normal

It must be genetics then. Body cells cannot use b12. May be mutase deficiency, or homocysteinemia or b7/carboxylase deficiency

In my textbook its given when trypsin enzyme is deficient, vitamin - cobalamin complex would be excreted in urine. But in this video why is it said only free cobalamin is excreted .

I had unfortunately had Gastric Bypass back in 2011 & have had nothing but problems since. Not sure if my Lupus plays a factor or not. I inject Cyanocobalmin 1,000 mcg/ml once mo. Still I have so many malabsorption issues hence blood tests always show low in this, iron, etc. I do have meds I take for a grade C ulcers caused by bacteria in my stomach. Had a gastro perforation twice. (Not fun). Wonder if my kidney stones factory, stomach issues, 45 to 50 lbs underweight & can't gain, fatigue & brain fog (was dx'd with Fibromyalgia could be that), ended up with 3 back surgeries/fusions, osteoporosis, tooth decay, chronic all over pain. I am 46 & thought I was in menopause but had my fsh levels tests - they say I should not be in menopause yet even though I've only had a cycle 2 x in 1.5 yrs & not often the other 2 yrs prior. I wish I could find out what the root issue is & if they are all related or caused by 1 single factor. I need to get me busy body lifestyle, former intelligent mind, focus & energy back before I continue to deteriorate. Which is how I feel my life's progression seems to be on a downward spiral. Any insights would be much appreciated. Note I do take meds for the stomach (Carafate & Pantoprazole in am, Prilosec in pm). I live in Rural Iowa & would gladly be a Guinea pig to an intelligent Dr such as yourself ~ anyone who could help me get the root cause(s) figured out so I can be a better mother to my young kids. What energy, brain & physical stamina, motivation I do have is focused on my sons 1st before I expel it on any other important life factors. I was an over achiever employee who could work 50 hours a week while still being an extremely active parent, co-mingled often with parents of my children's friends, held play dates, cooked all healthy meals, raised my own fruits & veges, active outdoors as much as my Lupus allowed, volunteered in many organizations, coached little league teams, OCD, very clean & tidy, on the top of everything 8 to 10 yrs ago. Now I feel they get 35% +/- of the above depending on the day. I don't socialize anymore whatsoever & even am unable to attend my immediate family functions. Worst of all I went from 3 to 4 meds to at my high was taking 23 along with numerous expensive vitamins, supplements & nutritional products. I've exceeded my $6,000 out of pocket & 2,800 deductible every yr since 2010. Have a son heading off to college in 1.5 yrs but haven't been able to save a dime for his education because I'm behind on medical costs that pile up yr after yr. I'm sinking in debt & definitely not sure how long I can keep my head above water. 6bof my Drs kept telling me I should go on disability & ultimately after missing so much work I had to humble myself to do so because this brain with 2 master's & worn out, chronic painful, dead tired body was unable to function at a decent level for any employer. I wound up having a min of with 3 major / massively invasive unanticipated surgeries ea. yr since Lord knows. The 2, 3, 6 weeks off to recuperate was not taken kindly by my last 2 employers not to mention 3 to 5+ Drs/Specialists/Pain Clinic I have to go to every mo. which around here is a 3 hr. commute round trip. Hence that's at least a day every wk I had to take off work since the jobs were 45 min - 1 hour the opposite direction. (I live in probably the most rural areas in the US where there aren't any part time jobs or any job for that matter). I want to use my higher education again before I hit 50 because I am becoming harder & harder to be among the desirable candidates employers typically consider. Plus I'm still paying an & a leg for all my degrees & will be when my 16 yr old heads off to college. My credit is shot due to excessive annual medical costs. I can't afford my copay Narcolepsy meds this new yr because my Cobra premium (health insurance alone is $1300 a Mo). I've drained my 401k just to keep my kids & essential necessities taken care of. So as you can see I'm open to any ideas, new tests, new medical trials, just about anything so I can not be digging myself deeper in not only in the financial sense but certainly the continual medical issues that keep getting added to my list of ailments every yr. Otherwise is anyone reading this been or going thru the same issues? I don't want to seem like I'm having a pity party, I am motivated & don't want to give up. I have to keep the faith that 1 yr I can find out something or find a team of physicians who genuinely care rather than forget who I am from 1 month to another offering their rural subpar lack of care/knowledge/ambition/etc. Any suggestions would be great, I think I've seen every specialist type we have in our 2.5 hr radius. I just don't get it because I am not a lazy person, my mind user to be sharp as a tack, well educated, had good careers, someone who can afford to stay living a wage on a disability income & much more. I've been to cognitive therapy, I seen neurologists - they all tell me it's NOT in my head - my medical charts clearly have real diagnoses (I didn't even go into half of them above). I'm so thin, weak & frail - a person who once was 185+ lbs at 5'9" - I barely can keep myself over 100 lbs now. Who would think being this thin would age a person 15 years but it does when your cheeks sink in, ALL your bones stick out, eyes look like they are popping out, my pants literally fall off if my rubber bands & pins start to shift, every single person who sees you has to make a comment or ask me if I'm ok because I'm so thin! Or Drs wanting to make you wear permanent feeding tubes that will run down my face onto my nose - meant to wear 24x7. My once naturally curly full hair is now falling out & looking sickly limp, even my skin & pigmentation looks worse than a hormonal teenager. I feel like I was hit by the ugly stick & someone who looks like a hobo because I can't find appropriate clothes under a size 0 or XS in the traditional sections a 46 yr old female would shop. Not one person in my ENTIRE FAMILY has refrained from making rude, nasty, condescending remarks on particular clothes I wore saying it isn't something my generation should be wearing. Or because I'm a tall woman obviously the jrs & kids sections outfit bottoms run short even on them because that's unfortunately the "style". It takes me hours to go look for pants or shorts that are the longest length in the entire store or department. All the haters & criticism I have to endure based on what clothes I have on has taken its toll on me. It hurts me to constantly have to hear at least 2 or 3 over spoken family members criticize my outfit plus made me extremely self conscious. I can't be too choosey because I'm on a limited budget. I'm forced to shop in the Jr section & even the level below that (young girls section where girls up to 10 yrs old still shop). If my arms aren't entirely covered people tell me to go put on a jacket because my arms are grotesquely thin to be "shown in public" so I have to wear a light jacket even when it's 100 degrees. My boys refuse to be seen with me in shorts & a t-shirt. The past 2 yrs my boys & soon to be ex said they refused to go on any short summer vacation or even go places locally or out to eat with me because they "are embarrassed to be seen with me when my bones protrude out". Admittedly I do have Pectus Carinatum (rare chest wall deformity that causes the breastbone/ribs to push outward rather than flush hence looks similar to a pigeons broad outward chest). Makes finding a 38AA bra nearly impossible, go figure! Having chest bones that protrude outward combined with having an extremely large bone structure I can't even purchase a young girls bralets due to not fitting a 38 chest circumference. Heck even undies are hard to find even in the Jrs section. Yes I'm now that self conscious that even undies are a long drawn out challenge to find to fit my overly large boney hips & yet not sag or fits snug enough so people don't see saggy panty lines. Also because I have a 20" waist somewhere above the ginormous hips but have NO tosh left to find an affordable size & age appropriate undies. Lol Dang if a person wasn't depressed before just the constant remarks on clothes would get you. Even the clothes I typically wear to hide it that are baggy. Well then I look like a poor hobo I'm told. Not like a Narcoleptic person can drive the 3 to 5 hrs ONE way to go to the bigger City mall stores - my choices are limited on my budget, inability to drive, family members who ridicule don't offer to take me to areas where I have more stores & options. Online shopping is not affordable nor the hassle of returning because nothing fits this overly odd frame, tall stature + overly underweight body. I try (tried) to eat 6 to 8 times a day - even then I get hassled from my unemployed soon to be ex because it annoys him or I'm being too loud in the kitchen (making a ham & cheese sandwich mind you), or he can't focus on watching his endless hrs of tv when I think I have to eat that often. Having Gastric Bypass muffled with complications, large bleeding ulcers, upset stomach, etc. a person like me can only eat golf ball sized servings to begin with or else I will be puking it all up. Gastric Bypass patients really are limited to what we can eat & how much in 1 setting or else our bodies go haywire which feels like a sudden flu that lasts for hours. We can't drink fluids until a half hour after otherwise can as long as it's a half hour before because that fills up the stomach making a person not eat the true nutritional golf ball sized meal that our malabsorbed bodies desperately need most. Definitely sweets are out because you go thru dumping syndrome, without explaining that let me say dumping is prob the worst even for a normal bypass patient who has no stomach, intestinal, or ulcerative problems. Wow! Finally someone I can relate to! I am SO SORRY you are going through all of that. I had gastric bypass ( the roux n y procedure which sounds like what you may have had as well) Dec 2001. You are literally telling MY story except that I did not get skinny or underweight. I was 5'8 and 291 at the time of surgery. I was 34 years old and got down to about 185 within a year. I also became pregnant with my 3rd child at that point. Before the surgery I already had Fibromyalgia and Chronic Fatigue Syndrome. I had Hypoadrenalism. I had suffered from a couple of concussions and whiplash from some car accidents I had been in. After surgery and weight loss and the pregnancy ( in which I was still trying to learn how to incorporate solid foods back into my diet...and vomiting profusely daily for the first 5 months of the pregnancy..lost 20 lbs first 5 months then rapidly gained 43 lbs the next 3 1/2 months) ...My health problems really went nuts and ran the gammot. 3 weeks after my son's birth I developed heart problems ( which I later found out I was born with) Seizures, Pancytopenia, Anemia, Gastric Bleeding Ulcers, Perferations, Duodenal Ulcers, Hiatal? Ulcers, Malabsorption issues, Pelvic floor collapse and had to have a hysterctomy...Blood Transfusions More Concussions due to Seizures where I fell and hit my head on everything from the oven door handle to a cement floor. One seizure caused me to strike the back of my neck ( just below my head) really hard on the back of a steel bar ( very sturdy lawn chair at a public pool while holding my 8 month old son)... That seizure was caused by a rapid drop in my blood sugar ( dropped to 34 according to paramedics) and my electrolytes were dangerously low. I had eaten just before leaving for the pool. I also have 2 other children that were teens at the time that were with me. I had my protein bar with me....my water..some juice. I'm constantly having B12 issues even in 2020. 18 years since my gastric bypass ...hoping taking the weight off would "cure" all those fibromyalgia/ and other fatigue/chronic pain issues I was always being JUDGED for by ( mainly family) I go through living with guilt for even being alive sometimes. I can relate to how you feel. On top of making some of my health issues worse, I have now developed Rheumatoid Arthritis and am scared to try the actual treatments for it due to their severe sounding side effects. With the malabsorption issues and possible lower immunity from that, I know the treatment is going to be risky. But ..Ho hum...my right hand is nearly crippled up into a fist and I'm awaiting surgery. Like you, I feel like I sound like one big pity party but i am stating facts. I am the one trying to lift other's spirits ...always had lots of motivation. When I do have energy I try to maximize my day but there's only so many hours in a day to catch up on everything I had to let pile up when my brain fog, fatigue, depression and severe pain set in again. Oh yeah.......did I leave out A.D.H.D? Lol. Thats almost to be expected after numerous concussion. It's hard wanting badly to be a contributing member of society but if tbe people judging us only knew how little we would be able to contribute with the neurological and other issues we have. In those physical and mental states we aren't contributing....we're making one mstake after the other. I worked as a hairdresser/ makeup artist before I developed these things. How can oral B12 come in urine? Until and unless it comes in blood and to get transported or to move in blood doesn't it require transcobalamine ?

it requires transcobalamine to remain within the blood(transcobalamine catches and holds the B12, says u stay with be right here in the blood), if it doesn't get bound to transcobalamine, then it gets wooooffff outta the kidney

I am not a doctor but I am struggling with a diagnosis. I have had 2 blood tests in around 4 years, I had to request both. They showed pernicious anemia and para mega haemoglobin, after a course of folic acid, they did not request a follow up blood test. I have had brain fog, feelings of constant negative thoughts without depression, feeling like getting through a day was tiring then having insomnia. They prescribed SSRI which made me feel worse. I have now come off. After telling them my problems are not caused by mental issues and asking for another blood test, that again it was the same and to take 5mg of folate daily. Recently I have been having mycolonic jerks and had a seizure to check for stomach problems due to gastrointestinal issues. They found erosive gastritis. I had a fit during the procedure and it was cancelled. The episode went on for 15 mins I was told after coming to. I am now at my wit's end. Can anyone help me in directing what may be affecting me. My doctor seems clueless as to my condition and has again mentioned it may be mental illness related. Why will my body not absorb folate, in eaten as part of a healthy diet or supplement form? I feel if my body can absorb the b12 I can feel better Any help would be appreciated...

first u need to confirm whether u actually have b12/folic acid deficiencies or not, if u have b12 deficieny and u think that ur body just cant absorb b12 then long term b12 IM injections are the go to options since they dont require ur body to absorb b12 into blood, b12 is being directly injected into blood. if that doesnt help raising ur b12 levels then u may have a problem with transcobalamin which carries/binds b12 and helps it stay in the blood by not allowing it to pass into urine.. problem with transcobalamin could b due to autoimmune causes where ur body produces antibodies which destroy ur transcobalamin. hope this helps. although im sure u already would have recieved b12 shots. then there is no problem with ur b12..

i take oral b12 (methylated) I only take 1/4 of 5000 mcg sublingual tablet daily ...and lately I forget more than I remember. ...for some reason my doctor did B12 test probably looking for low but I just got phone call ...too HIGH..."are you taking supplement? yes? ok stop taking" Point being it was NOT injected but is not simply going out in urine like you suggest ...why? Because it's sublingual? and/or because it is methylated?

it means that you are not getting it into your blood stream (because you took it orally) once it is in your blood stream it is easy to measure through urine because all the transcobalimine is occupied so if its free you will pee
