Agree 100% Scrappykat! I have a few seb-hype spots on my face, and my derm has a "don't give it another thought" attitude about them. But because they are visible to everyone who sees me, I do wonder what are my options to remove them. Thanks for the great video!!

Fun fact: I went to my derm because I thought I had a basal cell cancer on my face. It turned out to be SH. But it was my first visit so he did a full mole check and I had a mole on my back that was positive on the edges for melanoma. It was caught very early and no issues now. Moral of the story: don't diagnose yourself and don't skip your derm visit.

I have taken your advice and want to share with you, and everyone, that using a retinol works so well to shrink my little sebaceous hyperplasia “donut.” It has been at the side of my nose for twelve years, and this is the first thing that has worked for me! @@shireen175 I have had success with Neutrogena retinol cream. Never put your fingers in the jar, because it will degrade the active ingredient. Plus, even if your fingers are clean, you can still contaminate the product. I use a clean Q-tip to scoop out a bit of product at a time. I use it on my entire face; then, I leave a little glob of it on my SH donut. I sleep with it like that. It has never disappeared, but the appearance is less noticeable.

Hi, I my dermatologist discovered my first sebaceous hyperplasia "spot" when I was a teenager. He described it to me as an over reactive oil gland. Over the years, as I've gotten more, my dermatologists never really look at it as a priority. They do bother me, after watching this, I'm definitely going to speak up. My Dr. is wonderful but never shy about offering an expensive solution. A few zaps are most likely in my future :). Thanks for your great videos and have a great weekend!

I've been taking an estrogen blocker for breast cancer for 4.5 years. The SH has gotten much worse during that time. I have been using strong retinol cream for a few months and it seems to be melting, peeling the lesions away. They are deep so I'm going to keep going with the daily application of retinol on my face and upper back indefinitely. Happy that it works.

I have sebaceous hydroplasia on my cheek and I'm so pumped to get surgery. I tried using the laser but it made it more red in the area they did a biopsy. I have had mine since birth. I have been taking zinc supplements for about 2 months, and even though I can’t comment on its effect on sebaceous hyperplasia, it has helped to reduce oil production slightly. I’m planning on discussing with my dermatologist on the use of anti androgens alongside a laser treatment :)

Dr. Dray, I'm not sure if you mentioned that people with oilier skin are more prone to sebaceous hyperplasia. I have super oily skin and started getting SH in my twenties. My esthetician of 15 years, learned how to remove SH by a doctor years ago. She creates a tiny "flap" using a sterile needle to remove the "core" (the deep gunk in the hair follicle and gland) on each one. She uses sterile fine point tweezers and a lighted magnifying glass to remove the "core". They actually ARE extractable and she says if you don't get all of the "core" out, they will return (which is why if you've ever picked at one, they don't go away). I've never had one return in the same spot. I put vaseline or bacitracin on them and cover with tiny Clear Spot bandaids overnight or two to prevent scabbing and for faster healing and smoother makeup application. They heal nicely, quickly, and never scar with this treatment. The longer the SH are there, the larger they can get. They can be as emotionally devastating as having acne. It's TRAGIC this treatment isn't taught. My esthetician charges $60 (plus I tip) for 1 hour and can remove several within that time.

@@cnathan1 I don't know what the actual technique is called. I highly doubt many, if any others offer it. If you've ever watched Dr. Pimple Popper, you'll know that most doctors and maybe estheticians won't take the time to remove them or other skin ailments. There is skill involved as well, to ensure the core comes out. I do what she calls the Enzyme & Extraction treatment. She applies an enzyme mask and steams skin for about 10 minutes to open up the pores. Then she'll extract SH, milia, black heads, zits, etc. You can tell her you'd like her to focus on something, such as SH.

I’ve have atopic dermatitis since I was little, visited dermatologists all my life but gave up and stoped going because the treatments didn’t work or make a difference so I was used to live with this uncomfortable condition for decades. Believe ir or not, your common-sense minimalist approach has been life changing for me. I stopped using corticoids (which I have used non-stop all my life) about a couple of months ago just by following some of your recommendations (routines, advise and products). You inspired me to start Retin A with your very detailed videos on Differin (since I am 69 I thought Retin A would be more appropriate!). I have on my forehead what first appeared as a freckle, then over time began to rise a bit and get darker. Being a sun worshipper of the 70s and 80s (I know!!) I was quite concerned and had a dermatologist take a look. It is a sebaceous hyperplasia (thankfully!!). I am so pleased with the diagnosis that cosmetically it does not bother me (whew)!! I have noticed though, that with the use of tretinoin it has diminished considerably in color and size. Thank you, Dr Dray for your kind spirit and willingness to share your knowledge to better educate so many!! Wishing you a wonderful weekend!!

I notice that mine start off as zits. And before the zit has time to run it’s course it’s already headed over and formed a sebaceous hyperplasia. I have to hurry to reactive the zit with sulfur… it stimulates the oil production and the zit “unheals” and can run its course. If I don’t use the sulfur soon enough the zits heals over with lumpy skin in the form of sebaceous hyperplasia. Permanently scarring my face 😕

I had a cluster of 3 hyperplasia growing into one big one on my forehead. I went to the dermatologist and he wanted to scrape it off. It didn't feel like the right thing to do, so I left telling him I would think about it. What I ended up doing is I got a monthly 10% salicylic acid peel directly followed by a derma needling pen treatment of the spot. I rang up the dermatologist and he prescribed me tretinoid cream. The results are stunning. In two treatments the spot is less than half the original size. I use 0.05% tretinoid cream 3 times a week. I use about a small pea size that I dilute with about twice as much regular night cream for my whole face. It makes a world of difference.

@@ericmartin9569 no, tretinoine cream is something your skin needs to get used to. What I ended up doing is put about a peasize of cream on the back of my hand and then take some regular night cream and put about 2 pea size next to it and mix it together. That I put on my whole face and the leftover I rub on my hands. Tretinoine cream is also the best anti aging treatment out there so everything benefits from it. I started this routine in February 2020 and my bumps shrank to nearly invisible. In 11 months time I had 6 peelings and derma pen treatments (had one 2 days back) and my skin looks amazing.

Those little bumps are annoying. I have quite a few. I’ve found a few things that keep them looking flatter but nothing has taken them away completely. I’m getting more used to then just being there. And good tip to have them looked at by a derm. Mine always studies mine each December I go in for my yearly mole patrol lol.

I went to the dermatologist and has been confirmed I have sebaceous gland hyperplasia. I was also recommended electrodessication (I agree that it's an inexpensive procedure) and offered accutane (but not recommended). I already use bezaclin and differin for my acne. Surprised I still have this condition. I do agree they bother me and keep multiplying more these past few months and years. I do feel like I want to get rid of it. I did notice two was flatten out recently and was not sure if my facial visits help. At my facial sessions I had the aqua peel (hydra facial) done which is a machine with a tip that cleanses and sucks impurities. I will continue to monitor and see if I go the electrodessication solution. Thanks for this video Dr Dray

I have had great success with radio frequency treatment through both the Skin Classic and Lamprobe devices offered in some dermatologists' offices and med spas. Both devices emit a radio frequency current that basically evaporates the lipids comprising the sebaceous hyperplasia papules upon contact. My first treatment was expensive - about $400 - because I had about 50 hyperplasia on my face, but I'm down to about $100 for a few maintenance zaps once or twice a year when new ones pop up. I don't know if links are allowed in comments, but if you search both companies and sebaceous hyperplasia, you'll probably find a provider in your area.

I had 3 bumps pop on my cheeks 3 month ago. One is quite large maybe 3 mm in diameter and raised so with ceiling lights it looks quite apparent. After doing a bit of research, the cause of Sebaceous Hyperplasia is testosterone ( or DHT). There is a new treatment for acne which blocks the effect of androgen on the skin only, its called Winlevi cream, the active ingredient is Clascoterone which is a topical anti-androgen with no side effects (I am a male). I have been using this on my Sebaceous Hyperplasia bumps once a day for the last 2 weeks and the bumps have decreased by 50%, including the large one. At this rate they will be gone in one month.

@@wombatlittle1 hi a plastic surgeon did it in his office. it is called radio frequency. it actually burns the skin a bit and dissolves the oil deposit. it did hurt tho, my guy did not freeze skin. i also had some moles removed. the healing time however seems long especially for the moles and im afraid it may scar

I have had a handful of these. I always run to the dermatologist when they are very small. They have always used the hyfractor on mine and it works every time. My father had many large SH. I have used tretinion since I was 15. I am 58.

Great explanation for a common issue. I had about 20 small ones of these on my face and my dermatologist put me on Retin-A at night and spot treat of BHA in the morning. It was a slow process but after about a year they are virtually gone or basically so flat that they are hard to see with the naked eye. Since I’ve continued to use the Retin-A I have no new ones either. Yeah!

I just had many of these cautherized at the dermatologist's office. Quick, slightly uncomfortable, but so worth it! They only charged me $10/spot. At the same time they treated a few cherry angiomas on my face. I wish I would have asked about getting them treated years ago!

I would highly recommend the Evologie Intensive blemish serum ( I do not work nor associated in anyway with this brand ), I went to the dermatologist and she tried to burn one sebacious hyperplasia spot off (electrocautery), but it left a scar and pretty much came back. But I started with this serum and it diminished my spots significantly that they are barely visible. I will continue using it and see if it gets even better. Apparently, it has salicylic acid, tea tree oil and azelaic acid. Try it and see if it works for you.

Sebaceous = oil glands Hyperplasia = overgrowth Meaning because of your oil glands, there is an overgrowth of cells causing bump like texture on your skin. Basically meat bumps on your skin caused by overgrowing cells of your oil glands. Common places would be oily parts of your face like nose, T zones.

Mine started at the age of 18 instead of the usual teenage breakouts. And theya re on my chin only. I do have an oily tzone though. But they are only on my chin.

I can't figure out my skin issue. I have these bumps that aren't acne or cystic acne (I've struggled with both), they're not really that red, they're not white or yellow or black, they're not even that hard, they're not painful.... They're just raised bumps and I can't find an answer lol. Pictures online don't help

Hey doc I wanted to share that I recently started Benzoyl Peroxide lotion 2.5 and the sebaceous hyperplasia got smaller and even seems to be drying put and getting flatter. Why is that? I have used this lotion before and the results were not that dramatic. Maybe my skin had gotten "inmune" to it. What do you think? Thanks!

I know this is weird but I did a 10 day fast years ago (Master Cleanse---I know, I know, young and dumb), and they "dissolved" away. Autophagy? They came back eventually, though.

I have sebaceous hyperplasia and adult acne. I was on Accutane 3x in the past and it cleared the SH completely, but after the med stopped, it returned. I've also had freezing, cauterization, topicals, but the isotretinoin is the only thing that worked. My question--can isotretinoin be used in small does indefinitely to maintain results? Thanks for the informative video.

Isotretinoin (roaccutane) is also available as a topical cream treatment, perhaps that could work. People seem to have seen an improvement using tretinoin too.

I ve had ellman radiofrecuency (electrocauterization) and the SH dissapared, but I got 2 scars 😢, they have some weeks, and I dont know what to do. My derm told me they wont leave scars... but I am watching other thing, I am stressed

I have been trying to figure out what exactly it is that I have under my eyes. It’s shinny dots. It looks worse when I wear concealer to cover it. People always think I’m sweating under my eyes but I’m not. I completely hate it. It’s getting worse. I now wear big dark glasses because I can’t stand it. I cannot find a Dermotologist that deals with this in my area. They won’t go near the eyes.

Hi Dr. Dray, Thank you for the information regarding sebaceous hyperplasia. I had a dermatologist zap them off my forehead, approximately 1 dozen but unfortunately they came back. I will now be starting treating my skin with Retin-A. Hopefully this will help.

The dermatologist I saw for this was horrid. She pranced in, said it's sebaceous hyperplasia, nothing can be done" and walked out. So frustrating.

I have had great success with radio frequency treatment through both the Skin Classic and Lamprobe devices offered in some dermatologists' offices and med spas. Both devices emit a radio frequency current that basically evaporates the lipids comprising the sebaceous hyperplasia papules upon contact. My first treatment was expensive - about $400 - because I had about 50 hyperplasia on my face, but I'm down to about $100 for a few maintenance zaps once or twice a year when new ones pop up. I don't know if links are allowed in comments, but if you search both companies and sebaceous hyperplasia, you'll probably find a provider in your area.

Unfortunately, I have SH 50+ lesions that I’ve had cauterization, laser, but still come back. It’s starting to effect my nose as well. The only time they went away was with Accutane ( pulse therapy) but came back after stopping. Now at 52 , can I go back on Accutane for short time if my liver isn’t 100%? A doctor once said that intestinal health is involved as well with my case? I’m just so sad about my SH problem.

Thanks for the great advice.. I am subscribing!! I suffer from SH and have had NUMEROUS treatments. I am always scared my sunscreen will irritate or clog my pores and cause more SH, so I don't use sunscreen as often as I should. What sunscreen would you recommend? I'm 40, combo skin and still have acne.😣

You must use sunscreen everyday because you can get more from sun exposure. I have tons on my face. Nothing gets rid of them: Retin A, adapalene, Salicilyc Acid, AHA, not even electrocautery. I have an appointment next month with a plastic surgeon. I will ask him about getting laser done on my face.

I had abdominal cancer and theyntook all my girl parts and my spleen. Since then, I started getting sebaceous hyperplasia on my forehead. I use to have beautiful skin. Now I lool like I have acne. Still super oily skin. Using Neutrogena Stubborn Texture daily cleanser and Neutrogena rapid wrinkle repair regenerating cream. It seems to reduce the appearance.

Dr dray I have a question regarding androgens. I had high blood levels when I was younger and had cystic adult acne. I was cured for the most part by taking aldactone. My understanding was that it binds with the free testosterone and that had the affect of reducing my oil production. Whatever it did was a miracle. I could never take birth control pills of any kind and antibiotics and retin A did nothing for me. Here’s my question. I am now 55 years old still break out on occasion but have now developed many sebaceous hyperplasia lesions. I still take the aldactone. I am wondering if my age is affecting androgen levels naturally and I am at the same dose of medicine which could be lowering them more-would I notice improvement if I decreased the aldactone. Is there even a connection? I now use coconut oil to wash my face and am able to tolerate retin A and that is helping some with the hyperplasia but new ones are developing. Thank you for your time! I will admit it has been a while since I have seen a dermatologist and I am self-diagnosed with mine. In addition to that, now that you mention it, there are some spots that appear they could be something else more threatening. With that, I am going to set up an appointment ASAP and get these checked, so thank you. One question I have...will my retin-a affect any of these if they are NOT Sebaceous Hyperplasia, before I can get in?

I wonder if you could perhaps do an informational video on something I believe I have under both of my eyes but never knew what it was called. I refer to it as looking like "chicken skin" but believe the correct name for it is Syringoma. It is becoming much more noticeable as I age (soon-to-be 54) and would like to know what options are available for treating and/or getting rid of it. Thank you in advance!

Great video Dr., thank you. I have had numerous tx fro my SH and nothing has provided lasting results, freezing, burning them off, but they return. I was however, on Accutane 3x and the disappeared every time, but returned after tx stopped. Can Accutane be used in a small dose to keep them at bay long term? My dr. does not seem very receptive to this, but it is the ONLY thing that worked for me. I am so discouraged. Thanks.

Accutane was a miracle for me and my extremely oily skin and SH. I wish I could get it again. (after 2 rounds of 6 months each, they said no more due to risk of liver damage.)

Hi Andrea, I have one more question (I posted a bunch today under various videos). I have a bit of vitamin D deficiency and started reading about which vitamin D to take (D2 or D3). In the meantime, I found that the skin needs to be exposed to UVB light (the rays that cause wrinkles) to produce Vitamin D. So if we wear sunscreen all the time, how will we produce enough Vitamin D?

uvb is not the wrinkling ray it is the cancer causing ray...which is why there is zero amount of sun exposure to raise your vitamin d without raising your skin cancer risk

Dr Dray I did not understand your answer. The point is that at least some amount of damaging rays are necessary to produce vitamin D. I came across info that among the people who are at risk for vitamin D deficiency are those who overuse sunscreen.

Also, I picked up the neutrogena hydro boost gel cream for night time. I have never really been sensitive to any skin care. This burned my skin going on, and took about 10 min to go away. My whole face is bright red. I wonder what is in this That my skin is so reactive to? I love the texture of it...but probably shouldn't use it again. I'll stay on the search!!!

Hi Dr Dray. I was diagnosed with sebaceous hyperplasia 1 yr ago. Dr prescribed tretinoin and glycolic acid. They have helped a lot, some even disappeared. I am wondering about products with silicones or oils, should I avoid them? Can they make it worse?

It was a cream from a compound pharmacy where they mixed tretinoin and lactic acid. However, I have since changed I now use in the evening a toner with niacinamide after that I use salicylic acid (not every night) then tretinoin. In the morning I use the Ordinary niacinamide serum. I also use other products but these things have made a huge difference. I stopped using anything that has oils in it but silicones are fine. I hope that helps :)

@@lisag8442 don't worry. Thank you very much. I have been using The Ordinary Niacinamide and Zinc too. I have noticed a difference as well but I've been doing a bunch of things in the last 3 months: Blue LED light Theraphy (shrinks enlarged oil glands), Tazorac or Adapalene at night (speeds up cell regeneration to help fix damage to the cells and reduces excess oil), dermarolling once a week with .5 microneedles (plumps skins which helps to make the bumps less visible), The Ordinary Peeling solution and Salicylic acid mask (surface exfoliation helps to flatten the bumps and deep exfoliation cleans out pores to reduce hardened sebum) and use SPF every single day. I purchased am electric needle but the bumps came all back. I was able to extract a few of the bumps for the first time. The pore dilated and I was able to get all the hardened sebum out. I could never do that before.

Spot treating with BHA significantly shrunk the one I have on my face. It increased in size again though. I need to spot treat it again.

I’m 52 and have developed these pesky bumps. I’ve had 3 accutane treatments in my life but that was years ago. I take Adapalene now but it doesn’t really do anything for it. I really want them off my face. But I don’t want scaring.

I just recently got this diagnosis from a dermatologist. I am 35 and i have 14 over my face. I got my first one when I was a teenager. It's starting to really bother me. In the UK I am struggling to find someone to treat it. Can the lesions be shaved off?

They can, I’ve had them shaved off, however they have all come back. I’ve had moles etc shaved off and none of those have come back, but the hyperplasia has always come back.

I have a mole removal pen for home use that I use to get rid of skin tags (works quite well for those) and was wondering if I could use it to get rid of enlarged pores as well. Mine look like acne bumps (no depression in the middle) but I was told by the derm that it was enlarged pores and that she could burn them off for me on my next visit. Can't afford it right now, so would the mole removal pen work? Retin-A is not helping and my handheld home dermabrasion device is not helping anymore.

I’ve been getting Agnes RF treatments from my plastic surgeon and have found these treatments to be quite effective. It’s relatively new and would encourage anyone who is dealing with this condition to research whether or not this could be a viable treatment option.

I just started menopause in January and I read that sebaceous hyperplasia can come right after menopause starts so I've been dealing with this for months now and I'm really frustrated... I'm wondering if skin tag removal medication will do anything for these at all? I've been using that as well and haven't seen anything yet but I do have Retin A 1% that I just started using. Also what can I take to lower androgens?

My nurse practitioner wants to “burn them off”, is that the only option? I have using Differin for about three years now. Would switching to RetinA be more helpful?
