Medieval fantasy often links to Greco-Roman (Aquaducts) or Italian (venetian sewer-canals) aesthetics. And if one of those is the dominant culture, all "civilized" areas within a few hundred miles will likely also be copying the style, especially if they're in the same nation or close allies, because it's a tried and true style that lowers sickness and increases public clenliness. Or in other words, fantasy sewers make perfect sense, as long as you're aiming pseudo-mainland-European in your setting. Now if your setting is Olmec, Chinese, or Japanese... Or if it's a modern setting with modern plumbing... Then yeah, giant multi-storey sewers makes not one lick of sense. But Eminance is pretty European, so it gets a pass for Venetian sewer-catacombs.
