USDA mentioned that it should be 2hrs max: ask.usda.gov/s/article/What-is-the-2-Hour-Rule-with-leaving-food-out# So (A) is a answer according to USDA.

Bones in fish is physical contamination when fish have bones?

Yes. The bones can become a shocking hazard or cause cuts. It is always good to be vocal about there being bones present even if it is common knowledge
