I am a health care provider. Pulmonary and sleep medicine is my specialty. I see a ton of pre and post menopausal women complaining of sleep issues. I routinely tell them it’s likely due to hormonal fluctuations and send them to their gynecologist for follow up. I am shocked, SHOCKED at how many very fancy MDs don’t know the difference between formulations. Thank you so much for your time, content and presentation style. I am truly appreciative.

@karolynchang8559 Thanks for pitching in! Sleep is absolutely the most ignored and the most problematic menopause symptom. Hundreds of women have told me they sleep 2-3 hours a night. @@Jess33-w1o The answer is . . . YES I am a pharmacist but NO I don’t prescribe hormones. Pharmacists are allowed to prescribe a limited number of drugs like birth control but only in a few states. I’m currently licensed in Utah and Colorado, neither of which would allow me to prescribe HRT. That’s OK with me because I really emphasize patient education. I’m developing a database of providers I can refer patients to. Thanks for asking though.

Medroxyprogesterone acetate was the progestin used in the WHI Study (Provera). This was the substance that elevated the breast cancer incidences in the Premarin group. If your body never ever ever made medroxiprogesterone aceate, why would you give it in HRT? Unfortunately this is the first progestin type that is prescribed in my country, not real progesterone. I am in perimenopause and I was just prescribed this with estrogen gel. Going to another doctor this week!! I am so greatful for all this information about HRT! Without youtube videos, I would have probably started taking this MPA and put myself in serious danger! Thank you ❤!!

Was unable to swallow 100 mg (bhrt) progesterone round capsules (smallest they make) given 2.5 mg of medroxyprogesterone... What do you think of the combi-patch??

How is this supplemental progesterone taken? Would that be topical or oral micronized progesterone? I am in Europe and have been prescribed a combi pill of estradiol and nomegestrol acetate (Zoely), which I think you said previously is akin to medroxyprogesterone, as they're both progestins. I am 51, in perimenopause, and would like to get off this particular drug, as the side-effects have been awful (insomnia, incessant dark thoughts, weight gain,,,, and NO decrease in any of the symptoms I was prescribed it in the first place: hot flashes, night sweats, migraines), so I would like to be fully prepared to discuss this with my gynecologist. Is it oral micronized progesterone I should ask for? I love your videos and appreciate you SO MUCH!!

@catmaclaggan The most effective form of progesterone is oral micronized progesterone. It CAN be taken in other forms, like vaginal suppositories or transdermal creams. But those dosage forms are not very effective for sleep issues, the primary benefit of progesterone.

@aprilepp74 Great question. Premarin is a brand name for conjugated equine (horse) estrogens. That’s the horse urine derived estrogen that was used for decades. It’s technically a “natural” hormone, but not the same as human estrogen. Medroxyprogesterone is a progestin. That’s a progesterone-like hormone that is synthetic - made in a pharmaceutical laboratory. Medroxyprogesterone is often given along with Premarin. The hormone combination we use now - estradiol and progesterone - is much cleaner and safer.

@yensounta6501 Thanks for watching! My course doesn’t exactly address primary ovarian insufficiency (POI) but there’s a lot of overlap between POI and menopause. The use of hormone replacement after menopause and POI will be pretty much the same. POI is essentially menopause, just early.

Thank you @@SimpleHormones . The issue though is that with POI, the physiologic demands of lost estrogen/progesterone are much higher than a menopausal woman. With POI, we want to replace what we would normally make (e.g., in 30s vs 55+). This is where I'm lost. I don't know if a .1 estradiol patch and 100mg micronized progesterone will do. Still feels like it's not enough as I remain symptomatic. Any insight would be greatly appreciated! I feel like this population is massively underserved and our lifespans are decreased from the neglect.

The evidence from the medical literature shows that women with sub-optimal hormone levels after menopause (especially estradiol, progesterone, and others) are much more likely to experience: - osteoporosis - insulin resistance and type 2 diabetes - cardiovascular disease - Alzheimer's Those diseases are absolutely huge risks. Not to put too fine a point on it but . . . The last 3 are in the top 10 of diseases most likely to kill you. Heart disease is number 1 by a mile. Osteoporosis is likely to give you a broken hip, surgery, time in a rehab facility, a bed in a nursing home, and maybe even kill you. So . . . Yes . . . these long-term health risks are even more important than the awful, life-changing symptoms so many women have to suffer with.

@calimaree30 That’s one very good and safe combination. The key is estradiol + progesterone. There are always advantages and disadvantages to every dosage form. Many providers I work with prefer oral estradiol and oral micronized progesterone. I’ll have to make a video about oral vs. transdermal estradiol sometime soon. Oral progesterone is definitely the most effective form to help with sleep.

Great video, im doing my best as a husband to help my wife with her symptoms she’s started having in the last year, she still has her monthly periods but they’ve become irratic and other issues, her doctor put her on what we figured out now is continues hrt, her estrogen is a tablet that is only .675 mg and her progesterone is only 2.5 mg, we’re so confused as we dont see this small of a dose on any videos or podcast?, is this very low dose something we should still cycle progesterone 12-14 days once we figure out her cycle?. I appreciate all your videos. I'm curious. If a woman takes 200 mg of micronized progesterone continuously in peri-menopause, will that prevent ovulation and thereby, prevent any estradiol being made by the egg?

@GiGi-fd6om Great question! The answer is yes . . . and no. Progesterone does suppress menstrual periods when it’s given continuously. I’m not sure whether it Will completely suppress ovulation at that dose. A higher dose may be necessary. During perimenopause, ovulation starts happening less and less until it finally stops at menopause. Estradiol is made by BOTH the ovary AND the corpus luteum. That means suppressing ovulation may have some impact on estradiol but won’t eliminate it. Perimenopause is marked by dropping progesterone levels, caused by not ovulating consistently. It’s also marked by big estradiol swings, very high and very low - all over the place. Taking progesterone during perimenopause often helps level out the roller coaster.

Is there any advantage to using Medroxyprogesterone as opposed to a bio-identical version of progesterone for particular uses? For example, endometriosis or endometrial hyperplasia? My wife just got prescribed it for hyperplasia, but if the bio-identical version could serve the same purpose, she'd rather go with that. Also, regarding the doses. The prescribed Medroxyprogesterone dose is 5-10mg, whereas I believe the typical dose for the bioidentical version is 100-200mg. Does this have something to do with the Medroxyprogesterone being more potent?

I was on the 50/250 combipatch and was taken off and put on the Estradiol 0.05 patch and oral pill of Medroxyprogesterone 2.5 cause of post menopause bleeding from the combipatch patch. I have osteopenia I’m worried about being on this pill what progesterone is safe?
