This is literally the specialty I’ve been looking for! I liked ortho but I didn’t want to be a surgeon, liked plastic because of the in clinic procedures, liked anesthesia but it didn’t feel personal enough, and wanted to do something that worked directly with patients. I’m a super patient and optimistic person! This sounds perfect. Thank you for making this video!

Oddly enough, I think I would have to agree with you. I'm just a patient, but I've had the misfortune of needing to see a bunch of specialists in different disciplines - physiatry, physical therapy, psychiatry, psychotherapy, neurology, dermatology, ophthalmology, urology, immunology and (obviously) internal medicine. The only bad encounter I've had with any MD was with a psychiatrist who was likely in the middle of a severe case of burn-out. The physiatrist, however, was a fantastic doctor of exceptional ability with an extraordinary bedside manner. I genuinely liked her from the moment she introduced her. (I'm usually the kind of patient who cares more about an MD's abilities and credentials than their bedside manner.) Then, she looked at my medical history and various diagnoses and decided to take the lead in trying to coordinate care, "move beyond being treated as someone with a mysterious syndrome" and cut back on the multi-pharmacy that had built up around me

well, you have to present yourself as the "laidback" doctor because your patients are either in pain or disabled. The last thing they want is to hear bad news.

This just gave me so much clarity. I'm a third-year DO student, and I really hadn't had that "aha!" moment for what I want to pursue. I've just been coasting through rotations. I know what topics I like and the lifestyle I want, but this seems to be the speciality that encompasses everything I'm looking for in a career and with patient care. I’ve seen a physiatrist every 2 weeks for a year now for a back injury I’ve suffered at my job. He’s a complete gem of a human and even though my progress is slow and I’ll never be back to 100% he keeps my spirits up. Definitely a speciality I’d look into if I got into med school

wow, I've been struggling with what I wanted to do for a while now, I was always in between psychiatry, neurology, or optometry because I wanted to become a specialist that utilizes neuroscience, yet I kinda felt like the three didn't fit quite well with what I exactly wanted. I've been told that I should be a PT, one that subspecializes in the nervous system, but I wanted to be a medical doctor and go to med school, not PT school. However, I was doing some career researching and I stumbled upon this specialty, and I think I know what I want to be now! Every aspect of this job seems very fitting with my passions and capabilities, so it makes me so happy that I'm finally certain that I know what I want to do. Thank you so much for this video, you made me realize what I want to pursue!

I fell in love with PM&R in my 5th year. Still loves it but it has been slowly gaining popularity and it's now more competitive than ever. Still gonna try my best, see where that gets me.

This seems like a very rewarding career field. Chronic pain and sudden handicaps effects so many people. Not only that it's seems a bit more laid back. My mother has spina bifida and has had to see a few of these Physicians in recent years.

@@ovans Hey ! I guess my own experience biased my comment ! 2 years ago at least it was a big deal because you can branch into Pain management/Injections, etc making 400K a year. Was very competitive at the time. :/ oh well !

I would at a private practice for a PM&R Pain doctor (didn’t realize it was rare lol) but also everything else was super accurate including that it’s extremely tough dealing with chronic pain patients every day! Rewarding for sure but it can weigh on you! Great speciality though for combination of clinic and procedure days, which I think is the best part!

You don’t have consider one over the other. If you with PM&R you can then do a sports medicine fellowship after your 4 years of PM&R training.

Both physiatrists and orthopedists treat similar conditions, mainly musculoskeletal injuries, but orthopedists have the additional training to provide surgery as a treatment option, whereas physiatrists do not perform surgery. Many orthopedists and physiatrists do work in the same office.👍

You made it sound like physiatrist is an orthopedists w/o surgery skill. Imo their point of views are very different. This video doesn't explain enough about that. Orthopedists focus on musculoskeletal anatomy, PM&R focuses on function. While that sounds abstract, it's actually the only thing that the patient cares. For example, when total knee arthroplasty is performed, the patient did not really care about the fit of the prosthesis, they care about when will they be able to walk normally again without pain. PM&R focuses on that. With that being said, the interventions vary A LOT as long as the goal is to increase function, which means, it includes not only MSK cases, but also neurology, pediatrics, geriatrics, sports injury, and even cardiopulmonary problems. It certainly doesn't have the fancy vibe of the surgical specialties, but its patient-centered approach is second to none among other specialties.

Actually we do perform rehabilitation surgery - tendon lengthening, tendon transfers, osteotomies, joint replacements, burn contracture release, skin flap construction etc here in India and other parts of the world. We don't do surgery for acute trauma. Rehabilitation Medicine essentially treats any person with temporary or permanent disabilities. The Physical Medicine is the non-surgical component. And what he didn't mention is Rheumatology. We do a ton of that. It's normal for other doctors to not really get what we do or confuse us with therapists with whom we work closely. I think ABPMR and AAPMR and other resources will give you a better idea. Or you could speak to a Physiatrist.

Im an undergrad and I work at a Physiatry clinic. Since I live in an opioid/bad health epicenter of the US, its the busiest clinic in the area by far.

While trying to create a specialty branch...the tree wont work at all, truncated. In any national emergency, a doctor must be able to function within all 20 plus specialties... So I know the specialties or rather residencies were by hours and subject matter gained and covered and viewed (must, electives and optional, and revisits for the missed). Waiting 4 years to see 1 case or having missed all of them is not the best idea. Why, because you can still be working on another residency and then come back to the 1st or 3rd to see what you didn't get there. Then it's topped off, I think by revisiting the specialty you will practice....so doing all 20 or 15 residencies in the past was about 2 to 5 years up to 10 to 20...because the content of what is normal and abnormal and arely seen was always measured.

Hi, my brother is in PT school and he said that PT’s are educated on manage pain, spasticity, and neuromuscular dysfunction which was not included in the list. They actively engage in improving that patient.

I am a Physiatrist, and while this video is very good it has a blatant inaccuracy.....physical therapists DO NOT prescribe physical therapy! Only physicians can do that. A physician decides whether PT is appropriate, then a PT develops the most appropriate therapy plan and works on it with the patient. Granted, some states do allow a bit of leniency unfortunately in this regard as there has been gradual creep in scope of practice (some states allow PTs to start working with a patient and work with then for a short period before requiring physician input), but technically physicians PRESCRIBE.

Well said, Doc. Just to add this tangent: While I prescribe PT for a great many of my patients, it is only one tool that a physiatrist can use to help increase a patient's function. Other tools include occupational therapy, speech language pathology, medications, acupuncture, injections (for muscles, peripheral joints, and spine), osteopathic techniques, and diagnostic tests and imaging including Xray, MRI, CT, EMG/NCS (basically a nerve test), and labs (blood tests). I'm sure I'm missing some stuff but that's a brief rundown.

PM&R PGY4 here: actually, it’s super competitive. For the last 10 years or so, every PM&R slot in the match has been filled. You can’t SOAP into it. Step scores on average get higher each year. I did very well on Step and had a super strong application, but I wouldn’t be competitive for the program I’m in now (I matched in 2018).
