I wanted to ask something; My child was initially feeding very well. he got sick and was hospitalized for a few days with the flu. now he's home but not feeding as well. I thought it would go away but its been about two weeks. He just plays with the bottle in his mouth, but does not really eat his food. He just turned 4 months and is eating a max of 19 oz, that is with force feeds throughout the night. please let us know if you have any thoughts on the matter. Thank you.

It’s normal to have a reduced intake after recovering from an infection. Avoid force feeding, allow the baby to recover and if concerns persist, consider starting weaning foods. Review these (the aversion part of the first article)

You could try giving 60 ml every 3-4 hours and monitor weight gain (current) after 1 week. Decide based on this gain, as comparing from birth weight at this stage is difficult. Don’t force feed, take precautions to reduce reflux (review my videos on reflux)

You are supposed to prepare it at 70 deg celsius (to kill any bacteria), and then cool it to 37 deg C or so by leaving it to cool or letting it float in a container with water (if needed sooner)-you can test (should always test) by dripping a drop or two on the back of your hand, where the skin is sensitive (to avoid risk of giving it too warm and burning baby's tongue)

Your baby is growing well, and is probably starting to refuse feeds as a result of earlier overfeeding. Allow a longer gap, let baby start regulating intake (don’t focus on volume, keep the gap even if a smaller volume taken unless baby demands). It will normalize over a few weeks. Hello sir, my baby had a poor weight gain so the doctor suggested formula feeding.. we made a mistake of introducing bottle feeding to the baby who was only 3 weeks.. now at 6th week, they say that the baby might have nipple confusion and is not feeding from my breast (they didn’t check the latching or how she feeds).. we have increased the formula feed and I’m expressing my milk twice a day to feed her.. (I get 60 to 110 ml based on how often I feed her)..at early hunger signs when I breastfeed her she latches and drinks but starts crying in 10 to 15 minutes so I give her formula in a bottle..but when she is completely hungry and crying she does latch and drink milk and sleeps for 2 to 3 hours..She still has not reached her ideal weight… she also has acid reflux and spits up after every feed…Now I’m scared to breastfeed her as she may loose more weight..How can I overcome this situation and exclusively breastfeed her and ensure she gains proper weight.. How do I know for sure that she has nipple confusion.. will meeting a lactation consultant help?? (to check the latch) Please, I request for your guidance..

Babies go through phases and she is learning to regulate better. Weight gain is good but possibly more than the trajectory, so allow baby to regulate, don’t force feed, allow spacing and don’t be too focused on volume as long as weight pattern is ok (review this below)

Yes, that is a good plan to increase the solids so it helps you manage-try to express milk at work and you can use it during the day instead of formula (that will maintain your output to feed at night, otherwise it will drop)

Hi, feeding for 1 hour doesn't mean milk transfer is adequate, on the contrary, it may negatively affect by tiring both of you. Feed for 15-20 minutes on breast followed by expressing to empty the breast, you can then use this milk to top up soon after the following feed (the top up should be soon after you remove from breast, otherwise baby will be restless and will refuse)-you can avoid formula if this works (in terms of expressed milk. Good afternoon doctor My baby is 22 days old now.. my breast milk is too low. So how much formula milk will be sufficient for my baby... Right now I m giving him 30 ml (3-4 times) a day along with breast feed in between. The most important step now is to focus on improving your output (try to use breastpump after each feed, empty both breasts and use that to top up, avoid increasing formula for a few days till you see the improvement in your output). Hello Doctor I have a 32 week preterm baby. Her birth weight was 2.1 kg. Which reduced to 1.7 kg after photo Therapy. Her weight increased to 2 kg on 04-05-2023. Today she is 24 day old. My baby fully depends on formula. Initially she took 30ml every 2hour. Now 40ml every 2hr. Her day intake is ok.. & she sleeps properly. But at night. She demands 40ml every one hour. Since you are fully on formula feeding, try to space more and feed 40-60 ml every 3-4 hours, use pacifier to delay feeds so baby can take more

Hi, in some mothers, it takes a bit longer to establish feeds, you need to ensure adequate latch, with supervision if needed, and use breast pump to empty after each breastfeed, at least 3-4 hourly. If you are getting 10-15 ml each time after expressing to empty 2-3 times, it is likely to increase rapidly and no need for formula (check weight loss of baby, monitor urine output every 4-6 hours as indication of output). You could consider formula as a bridge till this succeeds (next 3-4 days) if worried, don’t give more than 60 ml per feed after trying above, and stop soon after your output improves (if your aim is exclusive breastfeeding)

Hi Dr, it was pleasure meeting you at the hospital. This is baby of Aisha mom if you still remember us😅. My baby is one month old now, should I continue waking her up every 2-3 hours for feeding. Also, I have noticed that the amount she drinks from time to another is not the same, sometimes she takes 120 ml and vomit some, sometimes only 60 ml and goes to sleep immediately.

Very nice to hear from you. She can sleep 4-5 hours now, some variation in feed volume is expected but with a better (longer) gap, she would regulate better. Monitor the weight regularly as well

Hello sir.. i have 28 weekers premature baby who is 6 and half months now. His birth weight was 800 gram and current weight is 5.1 kg. I used to feed him every 2 hourly. He is not willing to take more than 60 ml and if I try to give him he vomits. Reflux is also an issue for him. I am very tenst as I am unable to manage his feeding schedule. I also tried to increase the gap from 2 to 3 hrs between feeds, but he still don't take more than 60 ml His total milk consumption for 24 hrs is 600 ml only. Sir plz guide as weight gain is a big issue.

Thanks alot sir for your reply. Yes, he is on lower growth curve but following his growth pattern. He is also developing feeder aversion, in this case starting solids will be helpful? I am concerned what if he also developed solid food aversion. His corrected age is 3.5 months now and weight is 5.1 kg. Is that OK?

It is a similar volume given at a more frequent schedule maybe due to colic. Try to wait longer as possible and establish the 3-5 hour pattern, use pacifier to try and delay feeds

Babies appear more hungry at this age due to colic etc, if you keep increasing the formula, your milk will drop off and stop-try to express regularly to improve your production, but keep the formula the same unless your milk output is dropping even after expressing and weight gain is dropping (plot on centile growth chart to monitor)

It’s possible he still has nose block or ear discomfort. Use frequent nasal spray before feeding, don’t feed lying down, and consider expressing and giving by cup if still refusing, for 3-4 days
