As an audiologist (who also has severe tinnitus), there is a monetary reward attached to tinnitus for veterans, which is why many do not want to learn how to cope with it.

Well the metaphor is the practical part. The idea it presents is to keep going regardless of the struggles (which will be your negative thoughts beating you up) so you accept the thoughts are there but you don't have to conform to them (and who says you have to?).
