Exactly what im going through. 2 years of muscle spasm. Pts telling me my muscles are weak again and again. Found a bulge 4 days ago.

This is the most frustrating thing as it happens so often, claiming it's "just muscle spasm" is the most unsophisticated diagnosis :( sorry you're struggling but do check out the channel as we've loads that will help you and the membership on the website if you need more guidance :)

Very useful information about the potential real causes of pain rather than just a one-size-fits-all solution. Thanks for posting. The focus on many internet resources is lower back spasm. If the spasm is elsewhere (e.g. thoracic) is the regime applicable or would that need a different set of rehab and strengthening?

Its a pleasure, i think at it's root working on the foundational work is essential, movemetns like the hip hinge will work the entire posterior chain as a rule and so some additional work perhaps with things like Mid rows should really do the job. ALTHOUGH, it is common that if the "spasm" is above the mid back i.e. bra strap level, then it can commonly be related more to the neck than the midback itself.

Would regular road cycling - which puts the back in quite a pronounced rounded position - be just an extreme example of "Modern Lifestyle Impact: Prolonged sitting and chronic stretching of weak lower back muscles often lead to more harm than good"? If so I am guessing preventative strengthening would be sensible even if currently asymptomatic (no lumbar spasms).

You're on the right lines, Road cycling = sitting at a desk on steroids. it's just really not good for the back. Its not so much the muscles in this position that i take issue with because that can be compensated for, it is the impact on the ligament balance that the duration of the fixed posture has. It's quite unlike many other things in that you spend hours in the saddle practically not moving at all. and do this for sometimes hundreds of miles a week. (hours).

Very important what you are saying too many exercise regimes are flexion flexion biased and people with lower back pain are often flexion intolerant. Exercise with a neutral position of the spine. A question would you recommend using the towel under the lumbar spine at first for the dead bug if they cannot hold neutral pelvis /spine ?Matt the pilates teacher

Do check out the deadbug tutorial video, but yes using a smaller towel under the low back is a great tool to act as an extra "sensor" for lumbar movement :D It helps the individual feel the pressure changes on the towel as a surrogate for movements in the spine :)

Specially the muscle spasm as im a gym person and love sports and it's annoying me🥲
