I am a retired computer programmer. I've spent a few years writing "rom-able" code, where the initial values of variables are stored in rom (read only memory). It sounds like the car has an after market computer, and two variables are not initialized on power up. One variable caused the problem to switch banks. The other variable is responsible for misinterpreting the oxygen sensor.

Ivan, You produce some of the best videos on KZbin. I never worked on cars professionally, only as a hobby, but I spent 43 years in enterprise computing, and there are several truths shared between them. The first is that abnormal problems can often have abnormal causes. In that light, did anyone check the hidden wiring for evidence of rodent damage or the car being flooded? Another truth is that if you are looking at a wiring diagram and the symptoms don’t make sense or they appear to be moving, it may help to start looking for a missing or intermittent ground. It is really great to be able to sit in the comfort of my living room, watch you on a big screen TV, and not have any pressure to fix anything. I love it!

Since it's vvt and dual fuel system. Possibly the timing parameters are separate depending what fuel mode it's in.

Swapped O2 sensor connectors could possibly explain this, if the computer was adjusting fuel trim on the wrong bank. Also, some systems with a wideband upstream sensor apparently use the downstream sensor to determine the center point, so a problem there could conceivably cause issues.

About Toyota D-4S dual injection system: It does not switch between port and direct. It uses BOTH all the time. It has 3 strategies: 1. Cold start: Inject half of the fuel from port, open intake, piston down, intake closes, compression and near TDC direct injector sprays -> stratified fill, mostly lean mix except around spark plug, late ignition timing -> generate a lot of hot exhaust gas to warm up the catalysators ASAP 2. Low and mid power same as 1. except normal ignition timing 3. High power, port and direct injectors inject at the same time during intake stroke -> more time to get fuel vaporized & gasified to create a homogenous mixture, maximum cooling of intake air, less risk for engine knock/pinging. You tested only the port injection as you thought that D-4S switches between systems on-off, it does not. Correct terms for diag tool would be Stratified and Homogeneous

Sounds like either you are incorrect or you just nailed the problem. In the vid when the computer switches to direct FI his scope flatlines on the port injector pulse which means it gets completely switched off. Where are you getting the information that it does not switch off the port injection and why does the scanner differentiate? I have read the Toyota propaganda about how the systems work during part throttle etc.. but have not had a problem like this to actually start looking at the systems and troubleshoot them. Is there a strategy that would shut off the port injection for a problem like this?

Welcome to new luxury car diag. I've done 3 years at Lexus, 3 at Audi, and now at Mercedes. I've seen so much mind-melting stuff on the newer cars. The dealer is probably the best place for something that wacky. Chances are at least one tech at the dealer has encountered it, and if not, dealer techs have access to the technical help desk and field engineers if it's squirrelly enough.

I am not a diagnostics,, but i love watching these videos from all the people that are. Seems to me they all follow some logical progression to solve the issue. Some better than others. Using Fuel, Air, Power,Grounds, and Known Values from documentation found in various places and tools to check them. In this case it was misfiring, but it did fire sometimes. So then you decide what does work all the time, and narrow it down. The mistakes of not finding the problem must be from not following a logic pattern of steps/procedures,, and so that leaves what step was missed, and what caused that missed step or confusion.. In this case bank 1 and 2 intermediate firing confused the logic progression . So the answer must be what caused that... Heck if i know, but it's very interesting to watch.

Assuming the AF sensors are functioning properly I would check to see if foreign material is inside the fuel rail moving around from side to side restricting flow.

And I thought all the haunted ones came into MY shop! I'm of the opinion that there is moisture~slash~corrosion in a multi-connector somewhere. I find that problem in states where they salt the roads in winter... like Montana, where I live. Was this maybe a Louisiana Flood Recovery Vehicle? Electronics can't swap symptoms around like this unless acted upon by an outside change of some sort. I'm going for water...... somewhere.

it does see odd like "water" intrusion issues, but they did change scan tools.... perhaps one is listing the banks from in front of the vehicle looking at it, whilst the other is listing them from drivers seat (likely targeted at self diagnosing folks) I hate when they're all scrambled up like that, but a quick unplug of injectors or coil/s while running would tell you fast, in the world of computers and electronics, NEVER-EVER trust any data or documentation, no need to ask how I know :))

The question is do you trust the reported AFR? If so we have a serious mystery. I guess unplug both up upstream sensors, see what it does with default strategy.

My guess is some sensor is shorted on the 5V line causing noise and anomalies in the signals as viewed by the PCM. The Air fuel sensors likely share the line with something else that is causing one to read pegged as viewed by the PCM.

this sounds like a ground problem to the ECM. A faulty ground can cause the ECM to do all kinds of crazy things.

Exactly.. I was thinking the same thing... Check powers and grounds at the computer. I bet that just by messing with the plug and moving things around at the ECM--- he would stumble onto a loose ground. The fact that it switched banks really narrows it down to only one common denominator... that being the ECM-- either a bad computer or bad grounds to it.

Hi Ivan, I going to go out of the box with this one and suggest the Hydraulic Lifters are the culprit. They could be sticking slightly open. This was my fist thought when I saw the first video and this problem only seems to occur on warm up. BMW are common for this. This could also explains the misfire movement from cylinder to cylinder, bank to bank.

Not logical...first one side then the other? Rusty, crusty, ground, etc do not behave like this. Only commonality is the computer (integrated circuit)...logic board...whatever you wish to call it. Internal logic malfunction...one side then the other...sometimes both at the same time (when it stalls). Replace the computer!!!!!!!!!!!!!!!!!!!!!!!!

I would look at strategies in the computer to see if the voltage is somehow dropping out momentarily and look into why the sensors are not being read correctly. I would look at ohm resistance issurs. On another subject, on the 98 Contour, I can't find a good wiring diagram for the tail light/brake light circuit at the light itself. I need to figure out which wire is turn/brake and ground on both sides. Need to tap into both.

I had a Honda CRV that ran rough during cold start and warm up, it turned out to be an Air Idle Control Valve. Can’t say that it’s relevant to your situation, but in your Lexus as in my CRV after warm up, the car ran fine. I also realized that low coolant level contributed to an early demise of the Air Idle Control Valve.

I would look at what the 2 banks share as far as power feeds and ground before condemning the ECU. Also more info is needed on how the system works. How is fuel routed to the 2 injection systems, is there a valve that switches between the rails? Does the direct injection and muti port injection share a rail? Could the car be switching fuel to the direct system on 1 bank before the computer commands those injectors?

My guess is that the O2 sensors were recently renewed or checked and the connections got crossed over between the banks. If swapped over then a lean condition from the O2 sensor in bank 1 would result in the ECM extending the injection times (richer) of bank 2 and vice versa. The slightest difference in O2 readings between the cylinder banks will be made worse by the ECM trying to resolve it.

I wish you would've expanded on your thought process behind testing the VVT with the scope and showed us that. From what I understand, the VVT shouldn't work in port FI mode. looks like the strategy is mainly clean the valves at idle when in gear, I guess red lights and stop signs are good for something after all and fools warming their cars :)) what happens in a hot climate with all round-about's? I guess it will toss a mix of both together to clean itself (or try)

Egr could be the issue. Egr opening before it should will cause the engine to shake like that. My 1997 Camry had these symptoms. Shake so hard when trans put in R or D. P not so bad. Got all new engine mounts. No codes So I got to thinking, what will make the engine shake that I can't see. Vaccum leak revs up the engine. Egr lowers the rpm. Pulled the egr assembly and found the spring for the pintle wasn't holding the exhaust pressure when the car was in idle. Egr lasted 230,000 miles.

At this point I think the wiring would have to be the culprit especially since it does it only when motor is torqued (in drive) like possibly both side oxygen sensors wiring are able to short but it happens to be one side or the other. Meaning maybe somewhere in the harness where both circuits may run together in harness there is an issue happening while motor is torqued in drive.

Having worked at a couple dealerships, I bet they've tried an ECM. Next thing I would do, is pull connectors from ECM and trace all wires, power and especially grounds with an actual test light. Please keep us in the loop, I'd like to know what finally fixes this car.

I reckon the AFRs being switched between visits is a strong possibility. Then whoever switched them didn't catch that the problem had switched banks. Ivan didn't realize it until unplugging bank 2 injectors stalled the engine.

Maybe you can get it to stay in 'cold run' mode by subtituting a variable resistor in the Engine Coolant Temperature sensor circuit. That could give you more time to 'catch' the problem.

This makes me wonder if another sensor is affecting the computer's decisions. I'm thinking maybe an engine temperature sensor like an engine coolant temperature, cylinder head temperature, or intake air temperature. If the engine has a CHT sensor on each bank, and both are faulty because they came from the same defective batch, that might explain why the issue was able to jump to the other bank. Introducing a rich mixture is a method that can be used to cool the engine by reducing combustion temperature.

Ivan, do you think someone may have reversed the upstream O2 sensors. I've seen similar behavior on a BMW that had been to an exhaust shop do work, and someone reversed the O2 sensor connectors so the computer starts dumping fuel down one side but it still reads lean from the other side, and then it will reverse and do the same thing on opposite sides. I don't know if it's even physically possible to reverse them on that car though. I've never been under one.

Look at the fuel distribution system to the two banks. Is there a T or Y? I'd back flush the fuel rails all the way back to the common line that leads to the fuel pump.

In the DI mode I assume the engine runs more like a Diesel engine with a trend of wide open throttle body to reduce pumping losses. And when in the conventional mode, the throttle body to operate in a "normal mode" . The question I have is how the engine transitions between the two modes AND does VVT get involved with the switch? I bet you have an air flow problem, not a fuel delivery problem. At first I thought the O2 sensor wires were swapped...I hope you get another chance to work this problem.....

I wonder if it's a can bus problem with a terminator or a failing component somewhere connected to the bus. That causes some really weird side effects. I'm not even close to an expert but I wonder if the ECU is having issues with some other component on the can bus that is causing it to miss-read the signal from the O2 sensors. The ECU is supplying and reading the voltage that the O2 sensor changes (if I remember correctly). The other thing I'm wondering is if the ECU stopped firing the entire bank of cylinders for some reason. I may have missed you doing a test on that but it would match the pull a plug and nothing changes if it's on the dead bank while the injector pulses were happening. This one is a definite stumper.

Yes I suspect wiring as well especially because of it happening when torqued (put in drive)

I havent seen part 1 but if it hasn't been looked at check coolant temp sensor. See if injection changes at certain temp. If so, maybe put decade box on the cts and see if something is corrupt in ecm. Kinda funny it switches banks,,,

Swapped wiring somewhere like rear O2 sensors? As you said a computer reading problem or the even a software problem.

I'm going with ECU firmware problem, maybe a software bug? Doesn't sound mechanical or electrical if the problem moves around like that. Really wanna know though, if the problem changes how are you suppose diagnose it?

Not knowing anything about these injection systems, I have to ask: Is there ever a time, during normal operation that calls for only the direct injectors to contribute, and deactivates the port injectors?

Sure, actually most of the time the engine is running solely on the Direct injectors. The port system is only used for cold start and occasional "intake valve cleaning" I think.

hey ivan could the problem be a small manifold leak? all the numbers stayed the same till it got warmed up then it seemed to run better or could it be a warped intake manifold seeing it changed sides after you leaned on the engine?

You need to make flow charts,, did you check if the oil was saturated with fuel as in other videos. Seems to me this should be checked in and order,, do you have fuel, is it good, is pressure good,are o2 working, and so on. In the end it comes down to what is not working.
