In my opinion pills are worth it as the keeper. The health down is the most dangerous one and simply saving pills for when health upgrades are available counters that and allows you to use them as health upgrades the rest of the run.

To be fair, at least in Boss Rush he gets a choice between 2 or 4 different items, rather than the standard one which he might not have any chance to re-roll. Higher chance to get something worthwhile.

TheDarkChaplain there are indeed more items to choose from and during this video he still needed boss rush with the keeper, but lately NL isn't playing that well as he did during the afterbirth days, he makes so much minor mistakes and takes so much damage just from his impatience, he's exploring the minimum and at the end he asks "what did you want me to do?" and it's making his runs worse, I just want to see him play well again and rushing through causes him to lose the runs.
