Being new to ER, I get the impression that his videos, while not extemporaneous, are improvised around a core set of ideas. In this case, he knew to address his hair, and he knew what candy he had. Here and in any other videos, I hear him searching for words all the time, which helps with pacing and slowing down. A script would probably be more difficult to pace out.
