The "look at the look on her face" line is so funny to me because it's like he's trying desperately to make other people see what he sees. In the same way that everyone else couldn't persuade Tony that his mother was evil, he's trying to show the doctors that she's a total demon. It's hilarious because the doctors would never believe it - there's probably a million medical explanations for why she might be smiling - but we know the truth.

She's totally self-destructive, and the idea that her own children love her simply tortures her inside - she doesn't believe she deserves it. Seeing Tony fall apart like that is a confirmation of her beliefs, that deep down even her own son despises her. With that said, she also smiles when Tony falls down the stairs. I think there's an argument to be made that she just loved inflicting pain on her son.

My dad is exactly like Livia, so cold to the point that he probably pisses ice cubes, no empathy for others, very controlling and they're like leeches they suck you dry emotionally then move on to someone else and its always someone else's fault they're always the victim. The only way to keep your sanity is to stay as far away from them as possible the longer you keep them around the worse they get Tony being the prime example as Carmella put it both of his sisters left New Jersey so fast you'd think they had contracts out on them Barbara especially stayed away even when Livia had a stroke she stayed out of it and let Tony handle it she knew distance was the only way to stay sane
