To be fair, 1 heirloom at level 600 if you're completely free isn't too bad with average luck.

Can you get shards from a pack you get from leveling up because it says it a “rare” apex pack do you need to get a specific pack or is it 0.045% chance regardless of the pack?

The free packs you get from levelling up count. That applies to the chance to get heirloom shards but also to the 500 pack guarantee. same goes for Epic and Legendary packs you earn. Examples of packs that don't count are collection, milestone and thematic event packs.

no it didn't, but there was a glitch where some people got it reset when they shouldn't.

ah yeah, sadly if you're going purely off level in theory you'd need to reach close to max prestige (which I think works out at something like level 2,500). In reality you should also be earning packs from other places like events, so you should get it before then.
