The first one was really sad, because it could’ve been avoided outright if the staff had just listened to her when she said she didn’t feel safe. In fact, most of these could’ve been completely avoided if people had just used common sense.

Also the saddest thing is, the family sounded like they added the fuel on the fire post accident. Saying that they only hear 1 click instead of 3 click. I've never been on NTG/IR before. But, does that restrain supposedly didn't make sound AT ALL since it has hydraulic restrain? I've been on coaster with hydraulic restrain, and legit it didn't make sound, to the point that "Is it really? Where the click?". Which also explain why the operator just dispatched it once the restrain light is green (which again, that woman also had obesity)

Doctors used to not wash their hands before performing surgery and didn’t understand why their patients kept dying from infection. I’m pretty sure there’s a lot more common sense floating around these days. People make mistakes, it doesn’t mean an entire generation of people is stupider for it.

Chimera’s accident is a huge example of park negligence. The park barely had proper maintenance on this thing and it was running much faster than it was meant to. You can see in POV videos of this ride that it goes *flying* through its midcourse break runs, and an accident on this roller coaster was unfortunately inevitable by the way it was operating.

Regarding the smiler, the reason the 5th train wasn't noticed is because the engineers put it on, but didn't inform the ride operators. When it valleyed, the ride ops checked the location of the 4 known trains, and once located cleared the ride for dispatch and overrode the safety system. Since then, staff are now required to check all cameras to ensure the track is ALSO clear, regardless of if they have located the known trains

Electrical engineers weren't informed about the 5th train and the mechanical engineers had left for a break so the Electrical engineers were left to fix the ride so when the 4th train got stuck on the bat wing the Electrical engineers (knowing that there 4 trains) saw 3 trains on the break run and one on the 1st lift hill.

As someone who works for merlin, this is what I can say relatively confidently about the smiler incident. Ride ops will have given the ride to engineering who will have added the 5th train, tested it and given the ride back to the ride operators for another set of testing. The empty train will have valleyed on the last test that the ops sent as guests were sent on the one directly after. After the train stopped on the lift hill, the ride op will have given the ride back to engineering (without seeing the valleyed train) to figure out what was wrong. The engineer called will likely not have been the same engineer who added the 5th train as the engineer thought that there were only 4 trains in operation. The engineer then checked for the other 3 trains and found them. He then will have assumed all was good and that a sensor on a block section had been triggered by something other than a train (things like leaves hit these sensors very often) and he will have overridden the alert and sent the train around the first section which the empty train had been sitting in. This is then when the incident happened If you want to see what the engineers saw, the camera footage was released and is public on yt. - I don't want to excuse what happened as there were still a lot of mistakes that happened but thought I should give a better picture as to what happened that day *I also want to note that I am basing this off of protocol that is happening today which might have changed since the incident*

Any idea why the protocol today for Smiler wouldn't have the engineer checking the siding where the 5th train would be stored if it wasn't out on the track? Why wouldn't protocol include verifying the location of every train installed on the track instead of counting to four? Maybe you should suggest they add that to the protocol, it wouldn't be hard to do. EDIT: bullet point 56 on the last page of the document in my other comment should have been addressed by now but that might not correspond to your role at the park. I appreciate your insight into the organizational factors. The failure to handoff information from one group of employees to another was indeed part of the problem. The case report lists several other things that could have been done better to possibly avoid the injuries.

There's no indication that the engineers attempted to look at the CCTV screen and saw what was in that camera footage that was released. But I do believe that failure to handoff information was the main cause of the accident as well as control and engineering being sloppy with their jobs and cutting some corners (control decide whether rides run in harsh weather conditions and I know first hand that they can be slow at responding to changes).

Part of why I feel New Texas Giant is overlooked is there is still feuding over fault. Gerstlaur blames Six Flags for unsafe ops, while Six Flags blames Gestlaur for the trains whose safety system gave the all clear before dispatch.

For further clarification, the accident on the Smiler was caused by a miscommunication between the ride ops and maintenance. Maintenence added a 5th train to the ride, but didn't tell the ride operators. When that train valleyed, the operators visually confirmed the locations of all 4 trains on the ride. Not knowing a 5th train had been added, they assumed that it was a ghost train. Ghost trains are common since the sensors on the ride are pretty sensitive. That is why they manually overrode the computer system which stopped the following train. I'm not excusing what happened. I think knowledge is power, and the more clarification we have on what happened, the better

Adding to the Quimera accident, the investigation concluded that the ride wasn't properly maintained, and thus there was negligence in the part of the park. Because of that, the Mexico City Government closed permanently the park and revoked the concession of the operators of the park.

Everyone probably knows this but I wanna clarify it just to be sure, The Smiler has always been the safest coaster at ATR and has been even safer after the crash (The Overiddes panel has been locked until someone higher than a ride op or engineer unlocks it) The machine itself did everything it was supposed to. 7:16 you're right from what I can remember. there was a lack of communication between the operators, and the operators in the station did not know another train had been added to the track. they assumed there were still only 4 trains, so dispatched the other train. (correct me if I'm wrong, just going off what i remember)

more information on the smiler the crew that bought the 5 car forgot to tell the maintenance crew about it then stopped at the bottom the second train that stopped at the top went down because of the staff did not tell the maintenance crew about the 5 car the maintenance crew had a look at the ride and saw four cars so they thought it was a ghost car so they released the cars and then it happened

The ironic thing is, the Quimera accident is pretty similar with Mindbender accident. Just with different kind of fatality Both were had the whole bogie assembly just come off. Both were happen on the most rear train Both were mainly because of lack of quality part Both ended in fatality (Despite one is Final Destination 3, and another just slam into the support structure) And both of them are Schwarzkopf Triple Looper, which like only 2 in the world.

Wow I've never heard as much detail regarding any of these before. It got me wondering about the superman coasters I've been on, the checkers all wizzed by slapping the restraints with no pause to lift it, so maybe I could see the amputee guy overlooked. Also after visiting SFOT I never realized how few Spanish speaking visitors understood English and maybe that was a reason for tx giant. I never heard that she told them she was not secured. And finally, Indiana beach has not opened triple loop yet, so I think it's more than paint going on and the state will have to inspect it, etc. Good video though, as always, but more toons!!!!!

If you think about it, roller coasters are actually the safest form of transportation. Roller coasters are the only form of transportation where the vehicle is directly attached to a track and at the same time the passengers are directly attached to the vehicle.

Smiler is one of the best rides at Alton Towers. It was a tragic accident but the ride has been made safer with barriers in front of the seats. I’ve rode this many times in the last few years and they are extra extra cautious. Don’t let it ever put you off the ride

a family friend went on a coaster, (idk where) and basically their restraints didn't lock and they tried telling the employees but the employees didn't notice, and so they started going up the chain lift, before the employees noticed them being worried and so they stopped the ride when it had left the station by like halfway.

As a rollercoaster enthusiast and former ride op I can say that the most dangerous part of riding a rollercoaster is driving to the amusement park. Coaster accidents are always tragic when they occur but thankfully they're pretty rare. Safety is very inherent to ride design and I've noticed a theme that most rollercoaster accidents can be attributed to 1. Ride operator error. 2. Poor maintenance practices. and less common 3. Manufacturer design flaws/manufacturing defects. That being said amusement parks should be a place of fun where safety isn't a concern for guests and I find it unacceptable that this keeps happening. Parks need to invest time and money into maintenance and ride op training and manufacturers need to take the extra time to ensure their rides are without design flaws or manufacturing errors.

In the case of the NTG accident, the ride should not *physically* have been able to dispatch. It sounds as if this was a case (like the Icon Park drop-tower accident) where the restraint was able to latch, and register as closed, even though it wasn't in a position where it could actually protect the rider. That's a fatality waiting to happen. You can prevent it with seatbelts, but you could also prevent it by the restraint system refusing to latch in that position. The trains were manufactured by Gerstlauer, not RMC, and it caused bad blood between Gerstlauer and Six Flags that continues to the present day, which is why you haven't seen Six Flags parks putting up Euro-Fighters or Infinity Coasters in recent years--though they bought a lot of Gerstlauer spinners before this happened. More recent RMCs use RMC trains.

...now in the case of Icon Park, the restraints had been modified to fit larger riders in such a way that they were unsafe, but here, I don't think Gerstlauer had that excuse--this was a design flaw.

@@MattMcIrvin It was an override, literally nothing changed about the trains after the accident, because the trains themselves are perfectly safe, and are rated as class 5 restraints by the ASTM (the safest possible) And to be class 5, their has to be a redundancy system, whether it's a seatbelt or something internal, and hold your pelvis bone. If it was the case that the train was able to dispatch while the restraint was locked but not holding the rider correctly their would be many more similar accidents with these trains. The common theory is that she fell on the overbank(a partial inversion) but the other coaster with the same trains has a full inversion and has never had an accident before , the odds of this accident being the trains fault are minuscule.

The smiler is only supposed to hold 2 carriages at once. The maintenance workers let a 4th carriage go, even though the 3rd carriage was stuck, as it came down that path, it crashed. And they closed the smiler for 7.4 months.

Good video, very informative and I think you are being super respectful of the victims while explaining what went wrong, how/why it went wrong, and even giving insight into how it can be avoided in the future. i think it was 2017 at Carowinds riding the hurler something broke off a wheel of a car IN front on the train i was in and it made the cars jump like a whip it hurt my neck and my brothers we told the ride ops in the station something broke on the car and they looked at us like we crazy and loaded up the train and sent it back out

The smiler accidents haunt me. If you watch a video taken by someone stood watching the ride, you can hear the screams of joy turn to pure pain and fear. You can see images of faces covered in blood. Horrifying, and it's left victims suffering for life, all because of human error.

Once on nemesis inferno I had faulty restraints when the guy pulled it done before the operator said “it’s a faulty restraint, good luck” and it left quite an unnerving gap between me and the restraint, but I didn’t want to change seats incase they thought i was gonna stay and dispatched, so I was caught in two minds and decided it was safe enough to stay because it weren’t that deep and I could just hold on and be fine. I was alright and it was more so an inconvenience I felt I couldn’t put my hands up and enjoy the ride after waiting for 90 minutes but it’s alright I guess. Btw I’m not a gp, it genuinely was a fair gap considering I’m skinny but centripetal force probably would’ve kept me in.

How do you send a new train round not even once before you send a train with a full train of people??

For those of you who don't know on rollercoasters a block zone is a section of ride that only one train may occupy at a time. At the end of a block zone there is a method to stop the train, in case the block zone ahead is still occupied. This is the safety system that prevents rollercoaster trains from colliding with one another.

I remember this one time I was going on a ride in my city, the seatbelt was on right but when it started going (if your from Canada, idk if it the same in other countries, it called the crazy mouse) it goes fast and up and down, my buckle or something lightly came loose and when we turned the sharp corner I literally almost fell off the ride my friend literally had to grab my hand because she thought I was gonna fall off and I felt as if I did and I told the worker about it turns out that the seat I was on had to be closed but the worker who put me in didint know that, because that seat had to be looked at and fixed because the belt wasn’t tightened and if it came any loose I would of fell. I was 14 and now I’m 15

@@girrl88 i was in a dark tunnel during the loops that happened on the last coaster i was on. i thought they were really intense corkscrews until someone told me we actually were upside down for two of them and then someone else asked if i was felling sea sick, that was the only way i ended up finding out those make me motion sick

@@girrl88 heartlines arent as bad, in some cases, cuz they go fast enough that i’m upright before the motion sick gets to me, if that makes sense? but like full loops and the ones where they hold you upside down on a heartline or a loop i can’t do without getting nauseous
