Idk why but I feel like the protagonist's name is Pedro. After mashing that banana in his head and started killing mindlessly, his subconscious created the banana friend Pedro to justify his murderous acts as if the banana made him go on a rampage, and not make him feel bad about his actions. Basically something to make him maintain his sanity, but because his memory got fucked up, the only name he remembers is his own.

Let me try and explain the story a bit better as well. You see there's two pedro's in this story. We have the Player Pedro, son of mitch, eldest brother to Denny and Ophilia, and the ego created by the broken son of Mitch, the banana PEDRO~! The banana is the ego state the original version of Pedro that was left behind by himself trying to retain something to keep his mission going, the new self of him has no memories of what he must do, so Pedro and PEDRO~! set off to start doing in the evil deeds of his family.

I actually just completed this game, and there was a comment I saw saying that they think that the man himself IS pedro and the banana is just his sub conscious. That seems very possible. At the beginning of the game, when he kills one of the guys, the banana pops up and says, guess you'll have to kill them all now, maybe this just meant now everybody wants to kill him so he'll HAVE to kill them, or maybe it was the sinister banana controlling the guy, making him do evil things. All lore/story aside, this game was amazing, had so much fun with the combat.

I would say something more along the lines of schizophrenia, I mean in this game you have a talking banana who floats in the air and constantly disappears and disappears and then you explode his face in another world. I don't know much about schizophrenia, but I do know it can certainly cause hallucinations, and this seems right up the alley of hallucinations, he might not even be able to distinguish reality from the 'Pedro's World' that he created, explaining why his face looks exactly like Pedro's. He might even have a form of Multiple Personality Disorder, which his schizophrenic mind interprets as the said banana he tried to jam in his ears, in order to justify the voices in his head he might be hearing as well as why hes killing his own family, he has a seperate personality in the form of Pedro, another reason as to why their faces probably look the same

Maybe pedro is just some god, and has no actual form since hes just some type of living energy, and the protagonist makes a deal with pedro in which he would erase his memories and replace all he knows with a sertain goal to kill his family (although he doesnt know its his family). After that pedro would proceed to kill him after the deed is done, but the protagonist ends up resisting and surviving this force in his own mind because pedro tells him the truth, which then kills off the living energy inside of him killing pedro(since he is the living energy). The reason pedro is a bananna is because thats what he wants to apear as to the main protagonist so he would look more friendly and harmless which would make the protagonist trust pedro more.

i have a bit of an issue with the sewers part. first gaming disorder is a thing, and its not merely gaming. its gaming to a level that intrudes and harms other aspect of personal life. its also never an isolated issue, resulting from a need unfulfilled in real life satisfied rather in video games. other than that, unless theres some deeper narrative i missed its pretty weird to have an anti violent games section in a violent game. its just a bit weird, but still really fun. overall great game with some trippy story and aesthetic. also the part where he reveals his face to be the same as pedro's is symbolic more than anything else. the way i see it it shows how much pedro is not just a product of the protagonists imagination, but rather how its an integral part of who he is now, and maybe how pedro would never truly die out of the protagonists mind.
